2.Extraskeletal Neoplasm Resembling Ewing's Sarcoma: Case Report

Ewing's sarcoma had never been described as a primary tumor outside bone, although other malignant skeletal tumors, such as osteogenic sarcoma & chondrosarcoma, are known to arise from extraskeletal soft tissues. In 1975, Angervall & F.M. Enzinger reported 39 cases of small, round or oval cell sarcomas occuring in the soft tissues and considered histologically indistinguishable from Ewing's sarcoma of bone. Recently, We experienced one case of extraskeletal neoplasm resembling Ewing's sarcoma of bone which it was located deeply in the calf area of young female patient and the case review has been followed until the death, approxlmately 10.5 months after removal.

3.A Case of Acute Fatty Liver in the Second Trimester.

Acute fatty liver is rare but life threatening complication and usually manifests late in pregnancy. The mean gestational age onset of symptoms has been reported 35-37 weeks of gestation. Recently, several reports revealed acute fatty liver can develops in the second trimester of pregnancy. Recent reports suggest that early diagnosis and delivery decreased the mortality of fetus and mother. We have experienced a case of the second trimester onset acute fatty liver, in pregnancy so we report the case with brief review.

4.Multicystic mesothelioma of the peritoneum: case report.

Cystic mesothelioma of the peritoneum is a rare benign neoplasm that occurs predominantly in young to middle-aged women and tends to recur locally. Pelvic viscera is the most common predilection site of cystic mesothelioma. Authors report a case of multicystic mesothelioma of the pelvic pelvic peritoneum. The pelvic ultrasonography showed a large, confluent, and thin-walled multilocular cystic mass occupying the pelvic cavity and lower abdomen. The cystic mass was flattened and elongated. Each cyst of variable size was separated by thin septa and filled with translucent fluid. Cystic mesothelioma should be included in the differential diagnosis of the cystic peritoneal masses, especially in reproductive-aged women.

7.Psychogenic symptoms in patients with noncardiac chest pain.

BACKGROUND: Patients with chest pain are common among outpatients. Several researches shows that chest pain patients can suffer from psychiatric illnesses such as hypochondriasis, depression, anxiety, somatization, panic disorder and so on. This study aims to examine the pschogenic symptoms in patients with noncardiac chest pain METHODS: In this study, the 98 people who have undergone treadmill test for chest pain during the three months from June 1st to September 30 of 1998 were selected, and the 98 patients were asked to respond to the prepared questionnaire and SCL-MPD(symptom check list-minor psychiatric disorders) before they underwent treadmill test. And then, they were classified into two groups. One group consisted of patients with noncardiac chest,pain, and the other group of patients with cardiac chest pain. These two groups were compared in 10 measures, and the compared results were analyzed by t-test. RESULTS: Among the 25 patients with cardiac chest pain, 15 patients were males and 10 patients (38%) were females, and among the 44 patients with noncardiac chest pain, 31 patients (70%) were males, 13 patients (30%) were females. The average age was 47 years old. The average age in patients with noncardiac chest pain was 48, and the 46 for those patients with cardiac chest pain . In the statistical analysis between the patients with cardiac chest pain and those with noncardiac chest pain, significant difference(p<0.005) was shown in the measures such as somatization, depression, phobic anxiety, obsessive-compulsive, neurasthenic, hypochondriacal, anxiety, anger-hostility, interpersonal sensitivity. CONCLUSIONS: As in the analysis the patients with noncardiac chest pain showed significant difference in the symptom check list-minor psychiatric disorder compared to those with cardiac chest pain. We conclude that primary care physician should take more interest in evaluating psychiatric symptoms in patients with noncardiac chest pain.

PURPOSE: To describe the abnormal gyral and sulcal patterns obtained by means of three-dimensional (3-D) surface-rendering MR imaging in patients with cerebral cortical disease. MATERIALS AND METHODS: Thirteen patients with cerebral cortical disease [M:F=9:4, aged 8 -55 (median, 26.6) years] underwent 3-D surface-rendering MR imaging. Seven had cortical dysplasia and six showed gyral atropic change, conditions which in all cases were pathologically confirmed. All were the subject of conventional brain MRI imaging studies using the MP-RAGE (magnetization-prepared rapid gradient-echo) sequence, and the resulting 3-D data sets were processed on a commercially available workstation. Abnormal gyral and suleal configurations were reviewed. RESULTS: Abnormal gyral and sulcal patterns were seen in all patients. In eight cases these involved the frontal lobe, in three the parietal lobe, and in two the sylvian fissure. In four patients with cortical dysplasia, conventional MR imaging revealed no cortical abnormality, but 3-D surface-rendering MRI indicated that the configuration and orientation of affected gyri and sulci were abnormal. In nine patients in whom an abnormal gyral pattern was revealed by conventional MRI, 3-D surface imaging confirmed the presence of a thick and enlarged gyrus, or that the configuration of affected gyri was atrophic and abnormal. CONCLUSION: In patients with cerebral cortical disease, 3-D surface-rendering MR imaging detects a high rate of abnormal gyral and sulcal patterns.
