1.Heterotropic Ossification in Paraplegia: Report of of 4 Cases

Heterotropic ossifications have been known to occur around joints in many pathological conditions of the nervous system mostly in paraplegia and is affects most of the hip joint. Recently, 4 cases of heterotropic ossification in 18 cases of traumatic paraplegia have been reviewed at the orthopedic department of Kyungpook National University Hospital from April 1965 to July 1968 and the results obtained were as follows: 1. All cases were found in male of 3rd to 4th decade. 2. Heterotropic ossification affectected around the hip joint in 3 cases and in the other one case around the medial femoral condyle of knee joint. 3. Three cases affecting around hip joint expressed pressure sores on ipsilateral great trochanteric area, but the other one case affecting around medial femoral condyle had not any pressure sore. 4. In all cases, the heterotropic ossification was noted on X-ray film within 4 to 6 months after paraplegia.

3.Bilateral Slipped Upper Femoral Epiphysis: A case report

During s period of rapid growth in adolescence, weakening of the epiphyseal plate and shearing stress of incumbent body weight may cause the femoral head to become displaced from its normal relation to the femoral neck. Slipped upper femoral epiphysis was firstly described by Ambrose Pare in 1572, and many cases have been reported in Europe and America, and extremely rare in Korea. This particular case is a 15-year-old boy. Slipping was evident on the initial X-ray taken at a local provincial hospital approximately 1 year prior to come to this hospital. Owing to the capital epiphysis subluxated with coxa magna deformity, we performed Herndons operation as the first step of treatment. And schedule to do subtrochanteric wedge osteotomy for correction of residual deformity on right hip later on. Varization of left hip is also considered.

4.Bone Metastasis of Malignant Melanoma: A case report

The following case report concerns a rare bone metastasis of malignant melanoma occurring in the 4th and 5th lumbar spines and adjacent iliac crests and sacrum. A 42-year-old woman was admitted with a 6 months history of severe lumbago ar right flank pain radiating to right lower extremity. Preoperative X-ray studies revealed stag horn stone in the right renal pelvis as spondylolisthesis of the 5th lumbar spine with bilateral isthmic defects. On physical examinations, dark brown colored nevus was noted at lower lumbar regio which was removed and subjected for biopsy. Surgical exploration demonstrated a melanoma metastasis to have invaded the spine process, lamina and transverse processes of the 5th lumbar spine, adjacent L4 and sacru and iliac crests. Surrounding soft tissues, however, were apparently normal. Postoperatively course of the patient seemed to follow a rapidly terminal clinical cours.

5.Arthroscopic Notchplasty in the Treatment of Flexion Contracuture of Early Osteoarthritic Knee ( a preliminary study ).

Although causes of restriction of knee extension in osteoarthritis are thought to be contractures involving the posterior capsule and the hamstring muscles, intercondylar notch stenosis, osteophyte, loose body, and displacement of ruptured meniscus, few studies have investigated incidence and treatment. The purpose of this study is to consider intercondylar notch stenosis and anterior impingement as major causes of flexion contracture in osteoarthritic knee and to assess their relationship through arthroscopic notchplasty. We performed arthroscopic notchplasty and debridement in sixty patients (sixty-eight cases) with more than Sflexion contracture for early osteoarthritic knee and compared flexion contracture before and after operation. The average flexion contracture was 13degrees (5degrees-35degrees) before operation and 4degrees (0degrees- 25degrees) immediately after. An average of 9degrees improvement was shown, with 29 cases ( 48% ) showing improvement of over 5degrees. Our study indicates that flexion contracture in the 29 cases (48%) improved by arthroscopic notchplasty was caused by intercondylar notch stenosis and anterior impingement. Arthroscopic notchplasty may beneficially affect those with flexion contracture in early osteoarthritic knee when conservative management of this disease has failed. This is a preliminary study on the immediate postoperative outcome, therefore, long-term follow-up and recurrence rate should be investigated in future studies.

6.Clinical Study of Conservative Management of the Acute Epidural Hematoma.

The authors present 31 patient with acute epidural hematoma who had been admitted from January, 1987 to June. 1989 managed by consevative treatment. The results were as follows ; 1) The age of patients was distributed evenly, most patients were male (26 cases) and the most common mechanism of injury was traffic accidents(14 cases). 2) Sites of hematoma were mostly the temporal and parietal area and the maximum thickness of hematoma was not different from each site. 3) On admission, all patients presented GCS score above 13 except 2 cases. 4) All patients did not undergo delayed up, due to the enlargement of the hematoma or neurological deterioration. 5) The maximum thickness of hematoma was under 20 mm in all patients. 6) The hematoma was resolved within 30 days in all cases. This was confirmed with brain CT scan.

7.The Correlations between Cardiorespiratory Fitness Levels and Body Mass Index, Metabolic Syndrome Risk Factors, Homeostasis Model Assessment-Insulin Resistance and High Sensitivity C-Reactive Protein in Male High School Students.

BACKGROUND: The purpose of this study was to analyze the relationships between cardiorespiratory fitness and body mass index (BMI), metabolic syndrome risk factors, homeostasis model assessment-insulin resistance (HOMA-IR), and high sensitivity C-reactive protein (hsCRP) in male high school students in Korea. Furthermore, the effects of the cardiorespiratory fitness levels on these four factors were examined. METHODS: The level of cardiorespiratory fitness, BMI, percent body fat, waist circumference (WC), systolic blood pressure, glucose, triglycerides (TG), high-density lipoprotein-cholesterol (HDL-C), HOMA-IR, and hsCRP of 227 male high school students were measured. Analyses of multiple correlation and simple regression were applied. RESULTS: The results showed that the level of cardiorespiratory fitness correlated negatively with BMI, percent body fat, WC, TG, and HDL-C, in addition to HOMA-IR and hsCRP, and strongly influenced BMI, percent body fat, WC, TG, HDL-C, HOMA-IR, and hsCRP. Comparative analysis of our subjects showed that higher cardiorespiratory fitness levels had positive effects on BMI, metabolic syndrome risk factors, HOMA-IR, and hsCRP. CONCLUSIONS: This study found that there are close correlations between cardiorespiratory fitness and BMI, metabolic syndrome risk factors, HOMA-IR, and hsCRP. It also showed that, compared to lower levels of cardiorespiratory fitness, higher levels had beneficial effects on BMI, metabolic syndrome risk factors, diabetes, and cardiovascular disease risk factors.

8.Comparative Studies of the Effect on the Cat Brain between Intermittent Brain Retraction and Continuous Brain Retraction.

The authors compared the changes of morphology, blood brain barrier alteration, pathology, arterial blood lactate content and cerebrospinal fluid lactate content between an intermittent brain retraction group and a continuous brain retraction group in 56 mongrel cats. The results were as follows ; 1) Microscopically, hemorrhages were punctate in 15 cases among 25 cases in the intermittent retraction group. However, there were multiple or large hemorrhages in 13 cases among the 25 cases in the continuous brain retraction group. 2) All cases of the intermittent retraction group showed 0~25% Evans blue staining of the coronal section crossing the retraction site. However, 8 cases among the 25 cases of the continuous retraction group showed 51~75% Evans blue staining and 4 cases of this group showed 76~100% staining. 3) With photomicroscopy, the authors noted small hemorrhage and cellular swelling in the intermittent retraction group instead of pyknosis, hemorrhagic necrosis, vacuolation in the continuous retraction group. 4) The change of arterial blood lactate content was from 1.22+/-0.24mmol/L at preretraction time to 1.42+/-0.26mmol/L at 90 minutes after release of retractor in the intermittent retraction group(p<0.01). In the continuous retraction group, the authors noted a change in the lactate content from 1.20+/-0.38mmol/L to 3.15+/-0.97mmol/L for the same time as above(p<0.001). 5) The change of CSF lactate content in the intermittent retraction group was from 1.39+/-0.29mmol/L at preretraction time to 1.43+/-0.23mmol/L at 90 minutes after release of retractor(p>0.05). In the continuous retraction group, this author noted change in the lactate content from 1.37+/-0.28mmol/L to 2.11+/-0.52mmol/L for the same time as described above(p<0.01). From the above results, the superiority of the intermittent brain retraction was demonstrated as compared with the continuous brain retraction. Also the possible utilization of this experimental method was discussed for other wxperimental studies on ischemia.

9.The Effect of Mitomycin on the Experimental Filtering Surgery.

In tiis study, an expenmental model of glaucoma filtering surgery in rabblts was used to evaluate the effect of single intraoperative application of Mitomycin Full-thickness filtering surgery, posterior lip sclerectomy, was performed after application of 0.5mg/ml Mitomycin-soaked Week-Cel(TM) sponge for 5 minutes in one eye of 15 pigmented rabbits and was irrigated copiously with balanced salt solution(BSS(TM)). The same procedure was done in the contralateral eye as a control, using BSS(TM). In Mitomycin-treated group, the filtering blebs were more prominent with sparse vascular tissues and were maintained through postoperative 8 weeks or more. Histologically, there loas was amount of inflammatory reactions in Mitomycin-treated group. The fibroblastic activity and proliferations of the collagen were also suppessed markedly in Mitomycin-treated group. These results suggest that a single intraoperative application of Mitomycin can be used adjunctively to improve the success rate of glaucoma filtering surgery.
