4.Locking of the Metacarpophalangeal Joint of the Thumb: Report of Two Cases and an Anatomic Study of the Heads of the First Metacarpals.

Locking of the metacarpophalangeal(MP) joint of thumb in mild hyperextension is relatively uncommon problem. The most frequently recognized cause appears that proximal palmar ligament or volar plate were ruptured tranversely with its distal part riding over the volar prominence of the radial condyle of the first metacarpal head. We experienced two cases of locking of the MP joint of the thumb due to volar plate injury. Closed reduction was failed in both cases and open reduction was done by cutting the constricted ligament hundle over the radial condyle of the first metacarpal head. Our description of this mechanism is supported by expeimental evidence that we obtained from the cadeveric specimens. The height of radial side condyle of first metacarpal head is higher than ulna side of that by 1.76mm in both hands of 16 cadeveric specimen. So we are going to report these cases with a review of the literature, and suggest that the preferable location of surgical incision in open reduction is radial side of metacarpal head.

6.MARGINAL FIDELITY ACCORDING TO THE MARGIN TYPES OF ALL CERAMIC CROWNS.

Poor marginal fidelity resulting in a large marginal gap increases plaque accumulation, gingival inflammation and dental caries. The purpose of this study was to evaluate the marginal fit of three different cervical finishing methods of prepared teeth. A stereomicroscope was used to measure the space between the margin of restoration and the finishing line of prepared tooth. The results were statistically analyzed using the ANOVA and Multiple Range Test(Tukey`s HSD). The results were as follows : 1. There were no significant differences concerning the types of tooth and position (P<0.05),whereas the differences were statistically significant in case of cervical finishing methods (P<0.05). 2. There were statistically significant differences between before and after cementation (P<0.05). 3. In comparison according to variable margin after cementation, the gap discrepancies were increased in 130degreeshoulder margin, chamfer margin and 90degreeshoulder margin in ascending order, and there were significant differences between 90degreeshoulder margin and chamfer, 130degreeshoulder margin 4. In comparison according to variable margins, the gap discrepancies were increased in chamfer margin, 130degreeshoulder margin and 90degreeshoulder margin in ascending order, and there were significant differences between 90degreeshoulder margin and chamfer, 130degreeshoulder margin 5. This study demonstrated a better marginal fit with all-ceramic crowns fabricated on chamfer and 130degreeshoulder margin compared with 90degreeshoulder margin.

7.CT feature of bile duct invasion in hepatocellular carcinoma.

Intra- and extrahepatic bile duct can be invaded by hepatocellular carcinoma (HCC). This is infrequent in HCC, but it can directly affect the clinical manifestation and prognosis. We present eight cases of HCCs with emphasis on the incidence and features of bile duct invassion on computed tomography (CT). Over a period of 22 months, abdominal CT was performed in 186 with HCC patients. Out of these, five cases of bile duct invasion by HCC were confirmed in our hospital and three in other hospitals. The eight cases were evaluated for the type, size and location. CT features of intraductal mass and ductal dilatation were evaluated. The incidence of bile duct invasion in HCC was 2.6%. Infiltrative type of HCC was seen in seven cases and six of these had mass 5-11 cm in size. The characteristic CT findings of bile duct invasion in HCC are mass in common hepatic duct with bulging contour(8/80, multiple intraductal masses in the intrahepatic ducts (5/8), and diffuse dilatation of intrahepatic ducts (7/8).

9.THE EFFECTS OF SURFACE TREATMENT AND THERMOCYCLING ON THE MICROLEAKAGE OF COMPOSITE RESIN CORES.

The purpose of this study was to compare the microleakage of the composite resin cores according to surface treatment, dentin bonding agents, and thermocycling. For this study, 120 extracted premolar teeth were used. Flat occlusal surfaces were prepared with diamond disk, and treated with air-abrasion, acid-etching, combination. The composite resin core was built with Z-100 after application of Scotchbond Multi-Purpose and All-Bond 2. Prepared specimens were thermocycled for 2,000 cycles. Specimens were immersed in 1% methylene blue solution for 24hours at 371C. The microleakage was measured with a inverted metallurgical microscope(BHS313, Olympus, Japan). The following conclusions were drawn from this study 1. The microleakages in the groups treated with air-abrasion and with acid etching were greater than that of the groups treated with combination method before thermocycling(p<0.05), the microleakages of the groups treated with air-abrasion were greater than that of the groups treated with acid-etching and combination method after thermocycling(p<0.05). 2. There were no significant difference between groups using Scotchbond Multi-Purpose and the groups using All-Bond 2. 3. Thermocycling didri t affect the change of microleakage in all cases.
