1.Prognostic Factors of Physeal Bar Resection and Fat Graft Interposition in the Treatment of Partial Physeal Arrest

This purpose of this study was to evaluate the possible prognostic factors of physeal bar resection in the treatment of partial physeal growth arrest. From January 1979 to October 1993, 23 skeletally immature patients (16 males, 7 females) underwent physeal bar resection and fat graft interposition. The age was ranged from 1 year 8 months to 16 years 9 months (average 8 years 9 months). The follow-up period was ranged from 12 months to 10 years 6 months (average 3 years 4 months). The results of the surgery were categorized into 4 groups on the basis of relative growth ratio (RGR), spontaneous angular correction after physeal bar resection, disappearance of converging growth arrest lines, and the viability & proximal migration of the interposed fat verified by follow-up MRI. The RGR was assessed as a percentage of the contralateral limb segment: change in length of operated limb segment divided by change in length of unoperated limb segment multiplied by 100. The angular correction was calculated as the difference of the degrees of angular deformity between the preoperative and the latest follow-up visit. In the sixteen cases which required concomitant operations (osteotomy, Ilizarov method of for lengthening or deformity correction), their contribution to the angular correction was excluded in the calculation. The etiology of partial physeal arrest consisted of fracture (17), infection (5), and leukemia (1). In 10 of 17 fracture cases in which initial radiographs were available, the Salter-Harris types were assessed. There were type II (2), type III (2) and type IV (6) fractures. Distal femur was the most common site of physeal arrest (13), followed by distal tibia (7), proximal tibia (2), and distal radius (1). There were peripheral (9), combined (6), central (5) and linear (3) types of physeal bar. The nine variables including preoperative limb length discrepancy and degree of angulation, age, onset, etioloty of physeal arrest, Salter-Harris type of epiphyseal injury, site of the arrested physis, type and size.

2.Contact Urticaria - Relationship between Blocking Effect to Wheal Formation and Antiinflammatory Potencies of Some NSAID.

This study was undertaken to investigate the response of non-immunologic contact urticaria(NICU) test before and after ingestion of cyclo-oxygenase inhibitors such as naproxene, ibuprofen and mefenamic acid. Forty patients who showed positive reaction to 5% benzoic acid (BA) in petrolatum by 20 minutes closed patch test were chosen and divided into 3 groups. Group I was consisted of 13 patients who were taken naproxene 250mg bid, group II, 14 patients, taken ibuprofen 600mg bid, and group III, 13 patients, taken mefenamic acid 500mg bid. All the patients were tested with 5%, 2.5%, 1%, 0.5% and 0.1% BA in petrolatum using Finn chamber on Scanpor tape on the right arm before medication and next day on the left arm after medication of each day. Mefenamic acid did not show any significant differences before and after ingestion of drug. Naproxene reduced reaction about half of patients. Ibuprofen reduced reaction in almost all patients and blocked reaction completely in 9 of 13 patients. This results suggested that there was no correlation between blocking effect to BA induced contact urticaria and so called anti-inflammatory potencies of naproxene and ibuprofen, and that NICU by BA is partly mediated by prostaglandins(PG) or mediated by other mediators, which were potentiated by PG, except histamin.

4.Study on the Effects of Epidermal Growth Factor on Epidermal and Hair Growth in the Mouse.

Epiclerrnal growth factor(EGF), a low-molecular-weight polypeptide chain consisting of 53 amino acids, stimulates the growth and keratinization of epidermal cells. Recently, some authors reported that hair growth rate and follicular bulb growth were inhibited by EGF in neonatal mice. Since the hair and follicular cells are of epidermal origin, the effect of EGF would appear to be inconsistent with its role promoting the proliferation and differentiation of epidermal cells in vivo and in vitro. We have investigated the effects of EGF on the hair and epidermis in adult male mouse. The results are summarized as follows: l. 1'he hair length and maximum diameter were significantly reduced in EGF- treated regions compared with control group, and inhibition of hair growth was correlated with EGF dose. Thickness of the epidermis was significantly increased in EGF-treated regions compared with control group, and proliferation of the epidermis was correlated with EGF dose. 3 There were no significant differences in the hair length, maximum diameter, and egidermal thickness between untreated contralateral right mid-side regions of EGF-treated groups and control group.

5.Studies on Serum Lipids Level in Normal Korean Adults and Vegetarians.

The serum lipids level of healthy Koreans, including 306 normal adults, 15 overweight persons, 25 faculty members and 40 vegetarians (monks) were studied in 1973. The following results were obtained. 1. The mean serum liplids values of normal Korean adults were 188.7+/-33.7mg% for total cholesterol, 176.9+/-47.0mg% for phospholipid and 78.0+/-28.9mg% for triglyceride. 2. It was observed that the serum level of normal Koreans were influenced only by age and were not influenced statistically by sex, weight and socioeconomic state. 3. The mean serum lipids level of Korean vegetarians (monks) were 188.0+/-44.0mg% for total cholesterol, 158.5+/-35.6mg% for phospholipid and 64.8+/-21.4mg% for triglyceride and not influenced by sex and duration of altered eating habit. 4. The serum lipids level of Korean vegetarian (monks) were significantly low in phospholipid and triglyceride but not in cholesterol. 5. No significant difference in serum total cholesterol level between normal and vegetarian were observed all the role of diet of general population were discussed.

7.Clinical Study on Congestive Heart Failure.

Our experience with 150 patients with congestive heart failure has been reviewed with particular reference to physical and laboratory findings on admission, precipitating factors and diagnostic clues. Congestive heart failure has been defined as that heart failure due to myocardial failure (primary or secondary). (1) Congestive heart failure was found in 3.65% of total medical admissions and in 24.0% of total cardiovascular patients. There was no difference between two sexes and incidence was peak in the 6th decade. (2) Rheumatic heart disease was the most common cause of congestive heart failure (41.0%). Hypertensive and/or atherosclerotic heart disease (36.5%), cor pulmonary (6.6%), postpartum heart failure (5.3%), pericardial disease (3.6%), thyrotoxic heart disease (2.0%) and congenital heart disease (0.6%) were the etiologies of the remainder. In male hypertensive and/or atherosclerotic heart disease was most common cause of congestive heart failure. (3) The precipitating factors of congestive heart failure were found in 31% and infection, pregnancy, discontinuation of digitalis and physical exertion were major causes. (4) Dyspnea, cervical vein distension, pulmonary rale, hepatomegaly and cardiac murmur were important symptoms and signs on admission. (5) Peripheral venous pressure was elevated in 883.2% and circulation time was prolonged in 90.9%. (6) Leucocytosis (39.8%) albuminuria (60.2%), elevated GOT (serum glntamin oxaloacetic transaminase) (12.0%) and azotemia (50%) were observed. (7) Chest X-ray on admission revealed increased vascularity including pulmonary edema (88.8%), cardiomegaly (83.3%) and hydrothorax (27.1%). (8) Electrocardiographic findings was abnormal in 99.4% and left ventricular hypertnophy (45.7%), atrial fibrillation (27.3%), Right ventricular hypertrophy and low voltage were frequently observed.

9.A Clinical Observation on Total Hip Arthroplasty

Clinical observation was performed on the 84 hips of 72 patients which were replaced by total hip, who were treated at Department of Orthopedic Surgery, College of Medicine, Busan National University, from March 1978 to March 1981. The follow-up period of each case varied from 1 to 4 years. The results obtained were as follows: 1. The peak age incidence ranged from 51 to 60 years, which comprised 37.1% (23 patients) of total cases. 2. The causes of the hip diseases were; 25 cases of post-traumatic avscular necrosis of the head, which comprised highest incidence, 19 osteoarthritis, 14 primary avscular necrosis of the head, 4 ankylosing spondylitis, and 3 rheumatoirl arthritis. 3. The most commonly used prosthesis was Muller type in 64 cases (88.9%); Trapezoidal-28 in 3 cases; Ceramic-metal type in 4 cases; and modified Muller type by Japanese in 1 case. 4. Anterolateral approach was used in all cases except for the 3 cases in whom anterior approach was made. In most cases, greater trochanter osteotomy was not performed except for the 9 cases in whom operative field were poorly exposed without osteotomy. 5. Several complications developed; perforation of femur shaft and acetabulum during the operation in 1 case each, Post-operative deep seated infection in 2 cases, thrombophlebitis in 1 case, wire breakage in 2 cases, stem failure in 1 case, and femoral shaft fracture in 1 case. 6. Postoperative functional evaluation by the Merle d'Aubigne and Postel method was done; pain relief in 92.4%, mobility recovery in 71.6% and functional inprovement in 70.1% belonged to grade 5 and 6. Relief of pain is the most prominent effect of the total hip rep1acement surgery in this series.

10.A Clinical Observation of Girdlestone Operation

We clinically reviewed and analysed 19 patients treated as Girdlestone operation with or without Batchelor osteotomy at the Department of Orthopedic Surgery, Busan National University Hospital from April 1979 to June 1981. The results were as follows: 1. The underlying disease were infected pinning and nailing of 8 cases, pyogenic hip of 4 cases, failed total hip arthroplasty of 4 cases, tuberculous hip of 2 cases and femur neck fracture of 1 case. 2. Satisfactory results were obtained in 7 or 8 cases treated as secondary Girdlestone operation over 50 years of age. 3. Postoperative shortening ogleg length was 3.2cm in average (from 1.7cm to 5.0cm), which was average 2.5cm at 12 weeks traction and average 3.7cm at 6 weeks traction. 4. Postoperative average range of motion was; flexion to 80 degrees and abduction to 30 degrees. There was statistical significancy in activity of daily living of hip function at 12 weeks traction in postoperative score. 5. 6 cases out of 11 cases without pain in postoperative condition showed severe bony contacts and ir- regularities of proximal portion of the operated femur radiologically.
