1.A study to the workers exposed to organic solvents by neurobehavioral tests.

In order to evaluate the confounding factors of neurobehavioral tests and the neurobehavioral effects in the workers exposed to organic solvents, NCTB was carried out on 100 workers. 46 workers had never been exposed to neurotoxic substances, and the others were being exposed to the solvents, mainly toluene. Simple reaction time, digit symbol, Santa Ana dexterity test and persuit aiming were different with age in non exposure group. Simple reaction time was carried out well in males, and digit symbol and persuit aiming were in females. There was no difference at educational level when the subject was educated over 12 years. Santa Ana dexterity and Benton visual test differed according to exposure level to toluene, however simple reaction time didn't. The acute neurotoxic effect was not excluded in this study. But, NCTB could be used to evaluate and prevent neurobehavioral changes in workers exposed to neurotoxic solvents in Korea.

2.A histomorphometric study on the effect of surface treatment on the osseointegration.

STATEMENT OF PROBLEM: Many studies have been conducted to improve the primary stability of implants by providing bioactive surfaces via surface treatments. Increase of surface roughness may increase osteoblast activity and promote stronger bonding between bone and implant surface and it has been reported that bioactive surface or titanium can be obtained through alkali and heat treatment. PURPOSE: The purpose of this study was to evaluate the stability of alkali and heat treated implants via histomorphometric analysis. MATERIAL AND METHODS: Specimens were divided into three groups; group 1 was the control group with machined surface, the other groups were treated for 24 hours in 5 M NaOH solution and heat treated for 1 hour at 600degrees C in the atmosphere (group 2) and vacuum (group 3) conditions respectively. Surface characteristics were analyzed and fixtures were implanted into rabbits. The specimens were histologically and histomorphometrically compared according to healing periods and change in bone composition were analyzed with EPMA (Electron Probe Micro Analyzer). RESULTS: 1. Groups treated with alkali and heat showed increase of oxidization layer and Na ions. Groups 2 which was heat treated in atmosphere showed significant increase of surface roughness (P<.05). 2. Histomorphometric analysis showed significant increase in BIC (bone to implant contact) according to increase in healing period and there was significant increases in groups 2 and 3 (P<.05). 3. BA(bone area) ratio showed similar results as contact ratio, but according to statistical analysis there was significant increase according to increase in healing period in group 2 only (P<.05). 4. EPMA analysis revealed no difference in gradation of bone composition of K, P, Ca, Ti in surrounding bone of implants according to healing periods but groups 2 and 3 showed increase of Ca and P in the initial stages. CONCLUSION: From the results above, it can be considered that alkali and heat treated implants in the atmosphere have advantages in osseointegration in early stages and may decrease the time interval between implantation and functional adaptation.

3.The Incidence of Scoliosis in Korea

Authors examined 30, 000 minifilms(boys and girls each 15, 000) which were taken for survey of pulmonary tuberculosis for the students of the first grade, middle school in Seoul(average 13 years old), to determine the ineidence rates of scoliosis. It was considered lateral curvature of spine more than 10 degrees as non-postural scoliosis, which were analyzed for the presence of bony abnormality, sex incidence, degree of curvature, distribution of size of curvature, patterns of curvature, and direction of curvature. Following results were obtained. 1. The overall incidence of scoliosis was 1. 46%; female 1. 86% and male 1.07%. 2. Scoliosis due to bony abnormality was found in 4 cases. 3. 4.Tissue Pressure Changes following Tibia Fracture

Delayed diagnosis of the compartmental syndrome and subsequent delay in performing the fasciotomy can result in needless loss of function and possible amputation of the involved extremity. Unfortunately early evidence of this syndrome is difficult to assess. A direct measurement of the tissue pressure within a closed compartment has been developed which provides physicians with reliable information for determining the need for fasciotomy. In the 27 cases of the tibia fracture, tissue pressure was measured directly by method of the needle manometer in the Department of Orthopedic Surgery, Han Kang Sung Sim Hospital from March, 1979 to August, 1979. The results were as follows: 1. The highest mean tissue pressure per hour was 28 mmHg in anterior compartment at 24 hours after trauma, and 28.4 mmHg in deep post compartment at the same hours. 2. The highest pressure measured in all cases was 38 mmHg in anterior compartment at 24 hours and 39 mmHg in deep posterior compartment at 48 hours after trauma. 3. The return of increased tissue pressure to less than 10 mmHg took 122.6 hours in anterior and 124.4 hours in deep posterior compartment. 4. Tissue pressre was higher when there was associated fibula fracture. 5. Tissue pressure was higher in displaced fractures than in undisplaced fractures. 6. Tissue pressure was higher when fracture was in its upper one-third. 7. Tissue pressure was higher in comminuted fracture than simple fracture.

6.Nonsurgical Percutaneous Retrieval of Catheter Emboli from the Heart.

The increasing use of intravenous polyethylene catheters has led to a growing incidence of accidental catheter breakage and migration of fragments into the central vascular system usually in the inferior vena cave, the right atrium, of the pulmonary artery. The most common complications were formation of a thrombus in the area of the foreign body, infection with endocarditis, and perforation of the heart. And so obviously the polyethylene catheter must be removed. In 1967, Massumi and Ross et al have been successful in removing a catheter fragment from the right atrum percutaneously with a snare device. With some modification as suggested by Curry, method of retrieving fragments of polyethylene catheters from the heart without chest surgery utilize wire snares or endoscopic forceps. This report describe the technique and its use in successfully managing these complications in four consecutive cases.

7.Effect of Preoperative Fasting on Blood Sugar and Blood Gas Analysis .

Blood sugar and blood gas analysis changes caused by a 10 hour fast and a 15 hour fast were studied in sixtythree patients. In the 10 hour fast group, mean blood sugar level was 79.58+/-8.77mg/dl compared with the 15 hour fast group was 77.22+/-14.93 mg/dl. There were no remarkable changes on blood gas analysis for both the 10 hour fast group and 15 hour fast group. Neither age nor sex had any significant influence on blood sugar level or blood gas analysis. Thus, in order to make preoperative recommendations as well as to give appropriate intravenous fluids during and following operation, the anesthesiologist must be acquainted with the physiology of fasting.

8.A Case of Bleomycin Induced Bronchiolitis Obliterans Orgnizing Pneumonia.

There are numerous agents with potential toxic effects on the lung. In particular, cytotoxic drugs constitute the largest and most imprtant group of agents associated with lung toxicity. Bleomycin is commonly used, either alone or in combination with other chemotherapeutic agents, in the treatment of squamous cell carcinoma(head and neck, esophagus, and genitourinary tract), lymphoma, and germ cell tumor. One of the therapeutic advantages of bleomycin is its minimal bone marrow toxicity. However, pulmonary toxicity is one of the most serous adverse side effect. Classically, pulmonary toxicity manifests as a diffuse interstitial process or less commonly as a hypersensitivity reaction. This pulmonary toxicity is generally considered to be dose related and can progress to a fatal fibrosis. It is also possible that bronchiolitis obliterans organizing pneumonia(BOOP) is another manifestation of bleomycin induced toxicity. Bleomycin induced BOOP is less common and has a favorable response to steriod therapy. Here we present a case that demonstrates a BOOP, secondary to a relatively small cumulative dose of bleomycin(225mg/??, may be reversible.

9.Combination Therapy with Bacillus Calmette-Guerin (BCG) and Interferon-alpha in Superficial Bladder Cancer Patients: Immune Effector Cell Function and Toxicity.

The aim of this study was to evaluate immune effector cell function and toxicity of adjuvant intravesical bacillus Calmette-Guerin(BCG) plus systemic interferon- alpha administration in 12 superficial bladder cancer patients received transurethral resection. 80mg Pasteur strain BCG was instilled into the bladder at 2 weeks after transurethral resection and carried out once a week for 6 weeks. 3 million units interferon-alpha 2b was injected subcutaneously at 2 weeks after transurethral resection and carried out 3 times a week during the period of BCG therapy. Natural killer cell activity, Helper T cell/Suppressor T cell (CD4/CD8) ratio and phagocytic activity were evaluated by flowcytometry and toxicity was graded according to WHO guidelines. The results were as follows: 1. Natural killer cell activity of interferon-alpha or BCG in combination with interferon-alpha treated group was significantly increased compared to control or BCG treated group, also the activity of those two groups was significantly correlated between pre and post treatment state. 2. CD4/CD8 ratio of interferon-alpha or BCG in combination with interferon-alpha treated group was significantly increased compared to control or BCG treated group, also the activities of those two groups were significantly correlated between pre and post treatment state. 3. No significant differences in phagocytic activity were seen between all experimental groups or pre and post treatment state. 4. The toxicity of BCG in combination with interferon-alpha treated group showed minor to moderate and self-limiting in majority of the patients. By the results of this study, it is supposed that intravesical BCG instillation in combination with subcutaneous Interferon-alpha injection increase systemic immune response without significant toxicity and may be effective in prophylaxis of tumor recurrence in the patients with superficial bladder cancer.

10.Efficacy of Ferritin - folate - cyanocobalamin Supplementation during Pregnancy for Prevention of Anemia.

OBJECTIVE: The purpose of this study is to evaluate the efficacy of ferritin-folate-cyanocobalamin supplementation for prevention of anemia during pregnancy. METHODS: The authors conducted a clinical investigation on 50 pregnant women from 20th to 36th gestational weeks. The cobination of cyanocobalamin coenzyme 500mg, folic coenzyme 800mcg, and ferritin 20mg constituted the supplementation. The parameters examined in first trimester as baseline, before treatment(at 20th weeks), and after treatment(at 36th weeks) were : hemoglobin, hematocrit, ferritin, mean corpuscular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC), mean corpuscular volume(MCV), red blood cell count(RDW), folic acid, and vit. BPaired sample t-test was used for comparison. RESULTS: The results indicated a significant increase in the value of hemoglobin(p<0.05) and hematocrit(p<0.01) in comparison to before and after the treatment. The values of serum ferritin, folic acid, vitamin Bwere increased after the treatment compared to those of before the treatment, though there was no statistical significance. The results of MCV, MCH, MCHC, and RDW showed no statistically significant in comparison to before and after the treatment. CONCLUSIONS: These data indicate that supplementing ferritin 20mg-folate 800mcg-cyanocobalamine 500mcg per day from 20th to 36th weeks' gestation can increase values of hemoglobin, hematocrit, and ferritin concentration and can be cosidered as an appropriate method to prevent iron deficient anemia. It also might increase the value of folic acid and vitamin B12, concentration, but further study is stiU needed to determine whether the supplementation of folate and cyanocobalamine in combination with iron can have better eflicacy than iron alone in prevention of iron deficiency anemia.
