1.The Prognostic Significance of p53 Protein and PCNA in Advanced Gastric Carcinoma.

The 5 year-survival rates were examined to evaluate the prognositic significance of the expression of the p53 protein and the positivity of the PCNA in 108 cases of advanced gastric carcinoma. The p53 protein and PCNA were stained by immunohistochemistry in the tissue of the gastrectomized specimen. The results were as follows. 1) The overall 5 year-survival rate of advanced gastric carcinoma was 42.3 % and the significant prognostic factors were a pathologic stage and p53 protein(p<0.005). 2) The expanding or infiltrating type by Ming's classification and the intestinal or difftise type by Lauren's classification had similar prognosis. 514_ @@l %R-t 3) The 5 year-survival rate of the p53-positive group was 25.1% and that of p53-negative group was 56.1%(p<0.005). 4) The 5 year-survival rate of the PCNA low-grade tumors by PCNA stain(<50%)was 48.7% and that of the high-grade tumor(>=50%)was 29.9%(p>0.1). 5) There was a tendency to have a good prognosis in the p53-negative group and low grade tumors in the stage 11, III, and IV. There was a significant difference between p53 protein positive and negative groups in the stage III(p<0.005), but no significant differences were found in the other groups. The above results indicate that the p53 protein is an another useful tool for prediction of the prognosis in advanced gastric carcinoma.

2.An Immunohistochemical Study of Alpha-1-antichymotrypsin and Lysozyme in the Hofbauer Cells of Human Placentas.

The present study has shown that immunohistochemical staining of the human placentas (5 first trimester and 20 full term placentas) for confirmation of the monocytic linease of its macrophage function. We used two macrophage associated glycoprotein; alpha-1-antichymotrypsin (A1Ac) and lysozyme (LSZ). The results from presence or absence of A1Ac & LSZ by immunohistochemical methods can be helpful to deceide the degree of differentiation of macrophage. In al the placentas examined a strong cytoplasmic reaction for A1Ac was seen in the Hofbauer cells, and the same cells of serial sections didn't stain for LSZ. The strong cytoplasmic reaction for A1Ac supports that Hofbauer cells are macrophage, but they didn't stain for LSZ, a bactericidal enzyme, we propose that these cells are not fully differentiated macrophage. The lack of LSZ may have some relevance to the pathogenesis of certain placental infections.

3.FRACTURE STRENGTH AND MARGINAL FIT OF IN-CERAM, COPY-MILLED IN-CERAM, AND IPS EMPRESS 2 ALL-CERAMIC BRIDGES.

All-ceramic restorations have become an attractive alternative to porcelain-fused-to-metal crowns. In-Ceram, and more recently IPS Empress 2 were introduced as a new all-ceramic system for single crowns and 3-unit fixed partial dentures. But their strength and marginal fit are still an important issue. This study evaluated the fracture resistance and marginal fit of three systems of 3 unit all-ceramic bridge fabricated on prepared maxillary anterior resin teeth in vitro. The 3 all-ceramic bridge systems were: (1) a glass-infiltrated, sintered alumina system (In-Ceram) fabricated conventionally, (2) the same system with copy-milled alumina cores (copy-milled In-Ceram), (3) a heat pressed, lithium disilicate reinforced glass-ceramic system (IPS Empress 2). Ten bridges of each system with standardized design of framework were fabricated. All specimens of each system were compressed at 55degree at the palatal surface of pontic until catastrophic fracture occurred. Another seven bridges of each system were fabricated with standard method. All of the bridge-die complexes were embedded in epoxy resin and sectioned buccolingually and mesiodistally. The absolute marginal discrepancy was measured with stereomicroscope at x50 power. The following results were obtained: 1. There was no significant difference in the fracture strength among the 3 systems studied. 2. The Weibull modulus of copy-milled In-Ceram was higher than that of In-Ceram and IPS Empress 2 bridges. 3. Copy-milled In-Ceram (112micrometer) exhibited significantly greater marginal discrepancy than In-Ceram (97micrometer), and IPS Empress 2 (94micrometer at P=0.05. 4. The lingual surfaces of the ceramic crowns showed smaller marginal discrepancies than mesial and distal points. There was no significant difference between teeth (incisor, canine) at P=0.05. 5. All-ceramic bridges of three systems appeared to exhibit sufficient initial strength and acceptable marginal fit values to allow clinical application.

4.11 Cases of Aspergillosis.

Genus of aspergilli are ubiquitous saprophytic molds in nature, which are recognized about 700 species, at least eight are pathologic for human. Pulmonary aspergillosis is rare disease, most frequent presenting as secondary invasion, and aspergillosis of the nasal cavity is the most common sites of the extrapulmonary aspergillosis. Predisposing factors for pulmonary aspergillosis are chronic lung disease, tuberculosis, antibiotics, cytotoxins and neoplasia, hematologic disorders, and histoplasmosis etc. Authors have experienced 10 cases of pulmonary aspergillosis and a case of aspergillosis of nasal cavity in Dept. of Pathol. Chosun univ. hospital and christian hospital from Feb. 1981 to July 1984. Patient of pulmonary aspergillosis are received surgical resection and survived well without complications.

5.A Study of Vascular Changes in the Lymph Nodes.

A retrospective morphological study was conducted on 1677 lymph nodes which were removed surgically under the diagnosis of gastric carcinoma, breast carcinoma, colonic carcinoma, chronic gastric ulcer, and superficial lymph nodes nuder the diagnosis of reactive follicular hyperplasia at Chosun University Hospital during a period of 5 years from 1980 to 1984. The hematoxylin and eosin stained sections were examined to evaluate for any vascular changes. In selected cases further levels were prepared from the paraffin blocks for Gomori's reticulin stain. The vascular abnormalities were divided into 3 groups, such as hemangiomatoid, pan-nodal vasodilatation and miscellaneous types. The miscellaneous type included the proliferation of smooth muscle which encircled numerous small hilar arteries and veins. The results obtained were as follows: 1) The hemangiomatoid lesions were localized lesions which appeared to be associated with local malignancy, but infrequent in incidence. 2) A more frequent vascular abnormality was pan-nodal vasodilatation, which tend to affect many nodes in a single group and is relatively associated with chronic gastric ulcer. 3) Miscellaneous lesion was nonspecific and appeared both local malignancy and chronic inflammation. 4) All three vascular reactions were somewhat more frequent in female than male.

6.Clinical Characteristics and Growth Responses to GH Therapy in Children with Noonan Syndrome.

PURPOSE:Noonan syndrome(NS) is characterized by short stature, congenital heart disease, and typical facies. Recombinant human growth hormone(GH) has been reported to improve growth rate in a similar fashion to that seen in Turner syndrome. We investigated the clinical characteristics and growth reponses to GH therapy in children with NS. METHODS:The cases of sixty seven patients with NS were reviewed retrospectively. Ten of the 65 patients were assessed height, weight and pubertal stage every 3 months during GH therapy. RESULTS:Webbed neck(70%), delayed development(59.7%), low set posterior hairline(56.7%), eye abnormalities(56.7%) and mental retardation(55.2%) were the leading clinical characteristics. Short stature below the 3rd percentile was presented in 73.8 %. Growth patterns in NS children were variable and the evaluation of their growth must be individualized. The increments of height SDS were significant in children with GH therapy(height SDS:from -2.8+/-.6 to -2.3+/-.9, growth velocity:from 4.4+/-.8 cm to 9.2+/-.9 cm during first year, and 6.1+/-.1 cm during second year) (P<0.05). CONCLUSION: This study characterized the clinical profiles in Korean children with NS, which should be further extended with more children with NS. Additionally, the significant increase in final adult height after GH therapy in children with NS should be observed.

8.Metamerism in composite resins under five standard illuminants - D65, A, C, FCW and TL84.

This study was done to present a criterion in selection of the most proper light sources and materials by measuring metamerism index(MI) of the light curing composite resins with spectrocolorimeter. Metamerism is defined when two objects appear to be the same color in one illuminant but different in another. This is due to the fact that they have different spectral curves that fail to match under the second illuminant. In this study, A1 & A3 shade of five light curing composite resins (Esthet-X, Filteck Z250, Filteck A110, Charisma, Vitalescence) were chosen based on Vita shade. Five samples were made for shade of each product with Teflon mold (diameter: 15mm, thickness: 2mm). Metamerism index of each samples on a Barium sulfate plate (L*=96.54, a*=0.19, b*=0.01) prepared for sample fixation were measured with spectrocolorimeter(Miniscan XE plus, Model 4000s, Hunter Lab, USA) by applying standard light source D65, C, Fcw, TL84 and A. Standardization was done with reference standard (X=80.8, Y=85.7, Z=90.8) and light trap. The results were as follows. 1. Different resins with same Vita shade showed recognizable color difference(DeltaE*>2). 2. All composites had MI below accepted value 0.5 between standard illuminant(D65, C, & A) and below 1.5 under fluorescent condition (Fcw & TL84). 3. MI value between D65 and A showed higher value than MI value between other source of light(p<0.01). 4. All resins except Z250 showed MI value that A3 is higher than A1 between D65 and A(p<0.05).

10.Clinical Outcome after Pancreatectomy in Patients with Persistent Hyperinsulinemic Hypoglycemia of Infancy.

PURPOSE: The purpose of this study was to describe the clinical outcome after pancreatcetmy and its relationship with pathological appearances and clinical features in patients with persistent hyperinsulinemic hypoglycemia of infancy(PHHI). METHODS: Medical records of 10 patients(9 males and 1 female, mean age:40.4+/-1.5 months) who were diagnosed as PHHI and underwent pancreatectomy from 1988 to 2000 were reviewed. Clincal and biochemical data were recorded. Subjects were classified arbitrarily into early-onset or late-onset group according to age of onset. Pathologic appearance of pancreas was divided into 2 forms:diffuse or focal. The former had a focal pancreatic adenomatous hyperplasia and the latter was characterized by increased number of betacells with similar distribution seen in normal neonates. RESULTS: One patient had focal, and nine had diffuse lesions. After near-total pancreatectomy, 4 patients(40.0%) showed complete response, 4(40.0%) had persistent hypoglycemia, and 2(20.0%) developed diabetes mellitus. As neurological sequelae, 6 patients(60.0%) had persistent seizures, and 6(60.0%) had delayed motor and speech development. No clinical or biochemical factors related to postoperative outcome were found. CONCLUSION: This data indicate that early diagnosis of patients who present with hypoglycemic symptoms in infancy, especially early in life, and development of more effective therapy are warranted, because there is no clinical or biochemical factor predicting final outcome after near-total pancreatectomy and only 40% of patients with PHHI remained euglycemic after surgery with possible severe neurological sequelae.
