1.Study on Serum Lipoprotein Patterns in Normal Korean and in the Patients with Cardiovascular Diseases.

Serum lipoprotein patterns were studied in 78 normal persons and in the following patients; 79 patients with hypertension, 21 patients with cerebrovascular accident, 10 patients with myocardial infarction, 18 patients with diabetes mellitus, 10 patients with nephrotic syndrome, and 10 patients with chronic renal failure. The serum lipoproteins were separated by electrophoresis on cellulose acetate which gives high resolution of chylomicron, beta-lipoprotein, pre-beta-lipoprtoein and alpha-lipoprotein. Lipoproteins were stained by Oil Red O saturated in 60% ethanol, and each fraction was cut and eluted for spectrophotometric determination. The results obtained are summarized as follows. 1. In normal Korean, serum alpha-lipoprotein showed decrement with aging, while serum beta-lipoprotein increasesd with aging, but the difference was not statistically significant. No significant alteration by sex in serum lipoprotein patterns were observed. Pre-betalipoprotein was observed in 28.6% of normal Korean under the age of 30, and about 50% of normal Korean over the age of 30. 2. Serum beta-lipoprotein was higher significantly in the patients with myocardial infarction than in normal control subjects. 3. Serum beta-lipoprotein in the patients with essential hypertension was significantly higher than in normal control subjects. Serum lipoprotein patterns were, however, not statistically different by both sex and age in essential hypertension, It was observed that increased serum beta-and pre-beta-lipoprotein in essential hypertension was closely related with the intensity of albuminuria. 4. No significant difference in the serum lipoprotein patterns between normal control and the patients with cerebrovascular accident was observed. 5. In patients with diabetes mellitus, marked increase in the serum beta- and pre-beta-lipoprotein was observed. 6. in the patients with nephrotic syndrome, the increase in the serum beta-and pre-beta-lipoprotein was marked, and markedly increased pre-beta-lipoprotein was the case in more than half of the patients as compared to the other groups. 7. No difference was observed in the composition of polar lipids between the patients with various diseases (essential hypertension, diabetes mellitus and cerebrovascular accident) and normal person.

3.Effect of Extracorporeal Shock Wave Lithotripsy of Caliceal Stone according to the Location of the Stone .

PURPOSE: Lower caliceal stones treated with ESWL do fail to pass more frequently than middle or upper caliceal stones. We analyzed the results of ESWL of renal caliceal stones according to the location of calix. MATERIALS AND METHODS: We reviewed the stone-free rate in 119 caliceal stone patients treated with 3rd generation lithotriptor, EDAP LT-02 according to the caliceal location of the stone. Location of the stones were upper calix in 30, middle calix in 30, and lower calix in 59 patients. RESULTS: Stone-free rate was 90.0%(27/30 patients) in middle caliceal stones, 74.6%(44/59 patients) in lower caliceal stones, and 66.7%(20/30 patients) in upper caliceal stones. CONCLUSIONS: Stone-free rate of lower caliceal stones is higher than upper caliceal stones.

4.Clinical Manifestations of Overuse Injury in Musicians.

OBJECTIVE: To know the clinical features of overuse injury in performing musicians and to evaluate related factors that may affect the overuse injury and playing related pain. METHODS: Eighty professional musicians and music college students were examined during 1 year. All were examined by a history taking, physical examination, radiologic exam and if needed electromyography. Nonparametric Sperman's correlation were used for the statistic analysis and 5 interval grading system, visual analog scale for pain, and stress, criteria of joint hypermobility were used for various measurements. RESULTS: Fifty four men were studied in 3 music colleges and a professional orchestra. Frequency of symptom was 46.3%. The string players were most frequently affected (65%), whereas the wind instrument players were least frequently affected (11.5%). The most commonly affected site was the hand and wrist (54.2%) followed by the spine. Patients were divided by the severity into 5 groups and among them the grade 1 was most common (29.6%). No significant correlation was seen between the practice hours per week but a significant correlation was noted between the stress and playing related pain. Joint hypermobility plays an important role in music playing which is dependent on the kind of instruments. CONCLUSION: Incidence of the overuse injury in musicians is high in Korea and the prevention and early treatment are essential for the good outcome of overuse syndrome.

6.A Case of Surgical Arterial Ligation in Combination with Transarterial Embolization in the Management of Recurrent Uterine Arteriovenous Malformation.

Although uterine arterovenous malformation(AVM) is a rare cause of menorrhagia or postmenopausal bleeding, it is important to consider that curettage may precipitate life-threatening hemorrhage and therefore it is contraindicated when AVM is suspected. Special investigations such as hysteroscopy, Doppler flow ultrasound and angiography are important diagnostic tools, and transarterial embolization(TAE) has replaced hysterectomy as the treatment of choice in women who wish to retain their fertility. But due to the high incidence of collateral vessels, recurrence of the vascular malformation after TAE is common. We have experienced two cases of AVM, one of which was managed by TAE in combination with surgical arterial ligation.

7.Study on Jumper's Knee: Symptom and Diagnostic Method by Using MRI and Ultrasonography in Basket Ball Players.

OBJECTIVE: To assess the jumper's knee for the symptoms and diagnostics by MRI and ultrasonography in basket ball players. METHOD: Twenty knees of 10 basketball players with chronic knee pain were assessed by the history taking, physical examination and diagnosis by magnetic resonance imaging and ultrasonography. Their average age was 17.4 years. Stanish classification is used for grading the symptoms of jumper's knee. RESULTS: Nine proximal patellar tendons were diagnosed as jumper's knees (45%) and 11 distal tendons were diagnosed as jumper's knees (55%). In ultrasonographic findings, average proximal patellar tendon thickness was 4.5 mm+/-1.2 mm, and distal patella tendon thickness was 7.1+/-1.1 mm. Sensitivity was 63% (12/19) and specificity was rated as 100% (21/21). By the MRI findings, sensitivity was rated as 32% (6/19) and specificity was rated as 90% (16/21). Study between the patient's clinical severity level by Stanish classification and thickness of patellar tendon showed no significant correlation (<0.5). CONCLUSION: The ultrasonography to be more convenient and easier as a diagnostic method for the jumper's knee than MRI.

10.Angioedema-like facial swelling as a first manifestation of systemic lupus erythematosus.

Angioedma is a group of disorders with multifactorial etiology but a similar clinical expression, is characterized by swelling of subcutaneous or submucosal tissue. Systemic lupus erythematosus(SLE) is a chronic multi-systemtic disease characterized by inflammation and tissue damage resulting from deposition of auto-antibodies and immune complex. We experienced a case of angioedema which was the first manifestation of SLE in 24-year-old female patient. She had suffered from severe facial edema and multiple lymphadenopathy for six months and she also had pleural effusion, positive anti-nuclear and anti-DNA antibody test. Marked decrease of C3 and C4 levels was noted with normal antigenic level, and activity of Cl esterase inhibitor. The angioedema was not improved with anti-hitamine agents and instead disappeared with use of corticosteroid and non-steroidal anti-inflammatory drugs. Complement levels normalized after corticosteroid treament. We report a case of SLE which initial manifestation was angioedema.
