1.Cervical Prevertebral Hematoma - a Rare Complication of Acupuncture Therapy: A Case Report.

A 36-year old man with neck pain developed an acute cervical prevertebral hematoma after acupuncture therapy at an oriental medicine hospital. MR imaging demonstrated a fluid collection, and this suggested a diagnosis of retropharyngeal hematoma, and the patient was managed conservatively. We report here on a case of a retropharyngeal hematoma following acupuncture therapy and we review the relevant literature.

2.Studies on Total Exchangeable Sodium and Plasma Volume in Hypertension.

Total exchangeable sodium and plasma volume were measured to evaluate role of sodium in hypertension by radioisotope 24 Na and Evans blue in 25 patients with essential hypertension, 10 with malignant hypertension, 5 with renal hypertension and 15 normal controls. The results obtained are as follows. 1. The total exchangeable sodium content was 35.8+/-3.5mEq/kg in normal controls, 35.6+/-2.7mEq/kg in essential hypertension and 36.2+/-3.5mEq/kg in renal hypertension revealing no statistical significance in difference. In malignant hypertension, it was 44.9+/-2.0mEq/kg and it was markedly elevated than in normal controls. 2. Plasma volume in normal controls was 44.0+/-4.7ml/kg. In essential hypertension there was two groups, one (group II, 35.2+/-5.6ml/kg) was slightly lower than the other (group I, 43.2+/-4.8ml/kg). In malignant hypertension, it was 56.9+/-7.5ml/kg revealing significant increase than in normal controls. 3. Changes of the total exchangeable sodium content was observed after the administration of hydrochlorothiazide 50mg in normal controls and essential hypertension. In normal controls there was no statistical significance in reduction. In essential hypertension, initially low salt group (group II) showed no significant reduction and the other group (group I) showed significant reduction with depression on blood pressure. 4. Long term therapy with hydrochlorothiazide 50mg for 2~3 months in the group of hypertension who responded well with short term therapy (group I), the contents of exchangeable sodium reduced significantly than normal controls and the antihypertensive effect was sustained. 5. The reduction of plasma volume with hydrochlorothiazide in essential hypertension are similar as changes of total exchangeable sodium. 6. In cases of low salt diet 4gm of daily salt intake, there was similar results as diuretic therapy in exchangeable sodium. 7. In patients who responded well to antihypertensive effect of hydrochlorothiazide or low salt diet (group I), salt loading of 10gm daily showed an increase of total exchangeable sodium and an elevation of blood pressure with statistical significance.

4.Clinical research of Shoulder Injuries Developed in Volleybal Players Spking at a Posterior Angle

There is no sport in which the shoulder does not play an integral role. There are many patients with shoulder joint problems in volleyball, baseball, tennis and golf. In volleyball, a quick strong spike is necessary. Therefor, the treatment of shoulder injuries as well as the protection of the shoulder is a great concern of the player. The authors have done and analysis of the playing history and the clinical studies of the volleyball players who have been recorded as patients of the department of orthopedic surgery, Korea National Police hospital, from January 1977 to December 1980. Results of the study are as follows: l. Of the ages between 17 and 25, the average age was 19.9. 2. Out of 228 case studies 96 complained of shoulder pain or 42.1%. 3. The 84, or 87.6% of the players used the posterior angle spike. The anterior angle spikers and middle angle spikers were 6 players or 6.2% each. 4. Of 60 posterior angle spikers attempted to change spiking angle, 46 or 76% were able to alieviable the shoulder pain. 5. In the posterior angle spikers the lever arm is at its shortest extent so the cases of pain were analyzed as due to repeated impingement of supraspinatus tendon.

6.The Use of Holmium: Yag laser in Partial Menisectomy

Previous applications of laser bone and cartilage ablation have focused largely on the CO2 and the Nd:Yag lasers, using both the continuous wave and rapid superpulsed mode, which revealed severe thermal damage such as tissue necrosis and carbonization of the remaining tissue. In contrast, Excimer lases have provided better histologic results with minimal or no thermal damage, but the ablation rate and cutting efficiency have remained unsatisfactory. Though arthroscopic partial menisectomy has become the accepted technique for dealing with tears in menisci, division of the meniscus is sometimes difficult in the confined joint space of the knee and is associated with iatrogenic injury to the articular cartilage. The ability to quickly and safely divide or remove meniscal tissue would be a distinct advantage. The Holmium:Yag laser has many potential advantages over the CO2 laster, the Nd:Yag laser and the Excimer laser. Its principal advantages include minimal mechanical trauma to the articular cartilage, greater access to tight or restricted area of the knee joint, and its ability to function in a saline medium and to resect meniscus with minimal tissue necrosis. We have evaluated the effectiveness of the Holmium:Yag laser 74 partial menisectomies of 57 patients. Among 74 meniscal tears, there were 35 medial and 39 lateral meniscal tears. The average operation time was 33 minutes in the menisectomy and hospital stay was average 3 days. There was no significant carbonization on the surrounding tissue. Menisectomy of the posterior horn was safe and easy and there was minimal iatrogenic articular cartilage damage.

7.A Case of Multiple Intraosseous Lipomas

Fatty tumours are,the most common soft tissue tumours whereas they are among the rarest tumours in bone. Dahlin quoted an incidence of 1 in 1, 000 of all bone tumours, and fatty tumours in bone are almost single intraosseous lipoma and of four types; I. Soft tissue lipomata or liposarcomata, secondarily affecting bone. 2. Parosteal lipomata. 3. Intraosseous lipomata. 4. Liposarcoma of bone. Here we report a case of multiple intraosseous lipomas in a 23 year old woman.

8.Clinical Study of the Fracture of the Carpal Scaphoid

Ten cases of fracture of the carpal scaphoid analysed clinically and treated by admission at Department of Orthopedic Surgery, National Police Hospital during the period of 1975 to 1980. The results were summerized as follows: 1. The mode of the injury were consist of 3 in football game, 2 in judo, 2 in army training and 1 in fallen from height, 1 in slipping down, 1 in direct blow and almost falling down by outstretched hand. 2. Of the 10 cases, there were 9 male and 1 female patients. This result showed marked predominence of male over female patients and most commonly occured between 20 and 29 of age. 3. In 8 cases, the fracture was found on the waist of the carpal scaphoid. 4. Nonunion of the carpal scaphoid were noticed in 8 cases. 5. 2 cases of the recent fracture, there were appeared by rechecking X.-ray after 2 weeks. 6. The good results were obtained by bone graft according to method of Russe.

9.Calcaneus Fractures: Clinical Study of Axial Fixation

There is a great difference of opinion regarding the treatment of intraarticular fractures of the calcaneus. Many authors suggested that there are essentially four basic methods of treatment, but the choice of treatment depends on the surgeons preference, the type of fracture, and the age of the patient. We obtained the following results in 11 cases of calcaneal fractures involving subtalar joint which were treated by axial fixation at our hospital from 1977 to 1981. 1. The favourable results were obtained in 5 cases of 6 feet of tongue type fractures and in 3 cases of 5 feet of joint depression fractures. 2. Even if the joint depression type fracture which is not severely comminuted, the treatment with axial fixation is thought to be good method. 3. Axial fixation must be tried in three faces of reduction which are reestablishment of Bohler angle and subtalar joint and reduction of lateral bulging of calcaneus. The results were more excellent in which reduction is achieved near anatomical position.

10.The Asian-Pacfic Gamma Nail for Intertrochanteric Fracture of the Femur.

PURPOSE: To evaluate the treatment results of geriatric intertrochanteric fractures by using Asian-Pacific Gamma nails. MATERIALS AND METHODS: From June 1994 to June 1997, 58 cases of the intertrochanteric fractures were treated with Asian-Pacific Gamma nail, particularly compression hip screw fixation complications such as communited fracture involving lesser trochanter, transverse or reverse oblique intertrochanteric fracture. We evaluated the bone union time, neck-shaft angle, lag screw sliding by follow up radiographs, and studied complications and intraoperative cautions. RESULTS: The average age was 74.4 years old, the mean duration of follow-up was 17 months, the mean duration of bone union was 14.2 weeks, the average neck-shaft angle was postop. 133+/-2.78 degree and last follow-up was 129.5+/-3.37 degree, and the average lag screw sliding was 4.7+/-3.16 mm. Intraoperative complications were difficulty in distal drilling (posterior drilling) in 4 cases, fracture displacement by nail insertion in 3 cases, and postoperative complications were coxa vara in 13 cases, superior cutting out of lag screw with nonunion in 1 case and superficial infection in 1 case. The entry portal must be formed by awling. The fracture displacement when inserting nail could be prevented by proper reaming after inserting the C-shaped guide pin, and posterior drilling could be avoided by confirming the locking between target device and nail. CONCLUSIONS: We conclude that the Asian-Pacific Gamma nail could appropriately treat the osteoporotic, communited intertrochanteric fracture in elderly patients and we obtained satisfactory results.
