1.Three Cases of Neutrophilic Eccrine Hidradenitis.

Neutrophilic eccrine hidradenitis(NEH) was originally described in 1982 by Harrist et al. in a patient with myelogenous leukemia receiving chemotherapy. Clinically NEH represents various cutaneous manifestations with or without tenderness and pruritus. Histologic examination demonstrates a neutrophilic infiltrate within and around the eccrine gland and degeneration of the eccrine gland structures. Although the pathogenesis and possible cause of NEH remain unknown, it is probably an unusual cutaneous reaction to chemotherapeutic agents. A few cases of infection associated eccrine hidradenitis are found in the literature. We report three cases of neutrophilic eccrine hidradenitis. Two cases were associated with hematologic malignancy. The third case was associated with an infection of Vibrio vulnificus.

2.A case of chlorpromazine induced priapism.

Priapism is a well recognized complication of some oral medications. Those most commonly cited are: antipsychotic drugs, especially the phenolhiazine: trazodone and chlorpromazine :and the antihyperlensive drugs hydralazine, guanethidine and prazocin. We report a case or priapism associated with the use of chlorpromazine who experienced 2 episodes or priapism and never experience priapism after cessation of the drug.

3.Effect of Adenosine on Penile Erection in Rats.

We designed this study to investigate the effects of adenosine on penile erection in Sprague- Dawley rats, in vivo. This was performed by simultaneous monitoring intracavernosal pressure( ICP) and systemic blood pressure with UROCOMPACT 6000 plus( WIEST, Germany) after intracavernosal injection of adenosine, papaverine and adenosine plus papaverine. Increment of maximal ICP was observed in the adenosine treated-group (0.015 to 0.15mg, n=30) in dose dependent manner, i.e., 15.8+/-0.95cmH2O with 0.015mg adenosine to 44.6+/-2.72cmH2O with 0.15mg adenosine, in mean ICP(n=l0, P<0.05). A similar finding was observed in the papaverine treated-group (0.015 to 0.15mg, n =30) in dose dependent manner. i.e., 28.1+/-2.88cmH2O with 0.015mg papaverine to 62.3+/-4.93cmH2O with 0.15mg papaverine, in mean ICP (n=10, P<0.05). The additive effect of adenosine plus papaverine treated-group( 0.015 + 0.015 to 0.15 +0.l5mg, n=30) was to enhance the results obtained by adenosine or papaverine alone in dose dependent manner. i.e., 50.7+/-1.71cmH2O with 0.015mg + 0.015mg papaverine to 79.5+/-5.63cmH2O with 0.15mg adenosine + 0.15mg papaverine. in mean ICP(n= 10, P<0.05). These results suggest that adenosine plays an important role on penile erection in rats. Adenosine may be useful for the future management of impotence, although the clinical effect remains to be investigated.

4.Role of Redistribution and 24 Hour Reinjection Images to assess Myocardial Viability in Patients with Acute Myocardial Infarction.

PURPOSE: We evaluated the importance of redistribution and 24 hour reinjection images in Tl-201 SPECT assessment of myocardial viability after acute myocardial infarction (AMI). MATERIALS AND METHODS: We performed dipyridamole stress-4 hour redistribution-24 hour reinjection Tl-201 SPECT in 43 patients with recent AMI (4-16 days). The myocardium was divided into 16 segments and perfusion grade was measured visually with 4 point score from 0 to 3 (absent uptake to normal uptake). A perfusion defect with stress score 2 was considered moderate. A defect was considered severe if the stress score was 0 or l (absent uptake or severe perfusion decrease). Moderate defect on stress image were considered viable and segments with severe defect were considered viable if they showed improvement of 1 score or more on redistribution or reinjection images. We compared the results of viability assess-ment in stress-redistribution and stress-reinjection images. RESULTS: On visual analysis, 344 of 688 segments (50%) had abnormal perfusion. Fifty two (15%) had moderate perfusion defects and 292 (85%) had severe perfusion defects on stress image. Of 292 severe stress defects, 53 were irreversible on redistribution and reversible on reinjection images, and 15 were reversible on redistribution and irreversible on reinjection images. Two hundred twenty four of 292 segments (76.7%) showed concordant results on stress-redistribution and stress-reinjection images. Therefore 24 hour reinjection image changed viability status from necrotic to viable in 53 segments of 292 severe stress defect (18%). However, myocardial viability was underestimated in only 5% (15/292) of severe defects by 24 hour reinjection. CONCLUSION: The 24 hour reinjection imaging is useful in the assessment of rnyocardial viability. It is more sensitive than 4 hour redistribution imaging. However, both redistribution and reinjection images are needed since they complement each other.

5.Long-Term Follow-Up of Refractive Accommodative Esotropia: Decompensation and Cessation of Spectacle Use.

PURPOSE: To analyze clinical characteristics of refractive accommodative esotropia related with the occurrence of decompensation and cessation of spectacle use. METHODS: The records of 60 patients with refractive accommodative esotropia were reviewed. Patients were divided into a control or decompensation group. Twenty-nine patients in the control group who were followed up for long-term were divided into two groups based on spectacle use. RESULTS: In the decompensation group, the amounts of near and distant esodeviation without correction at the first visit and the frequency of failure to achieve central fusion were significantly greater than the control group (p < 0.05). Ages at the onset of esodeviation and first visit, the amount of hyperopia at the first visit, the amounts of controlled esodeviation, amblyopia, and anisometropia were not significantly different between the two groups (p > 0.05). In the group that no longer required glasses, the age at the first visit was greater and the amount of hyperopia and the near and distant esodeviations without correction at the first visit were significantly greater than the glasses-wearing group (p < 0.05). Age at the onset of esodeviation, the amount of controlled esodeviation, refractive change per year, stereopsis, amblyopia, and anisometropia were not significantly different between the two groups (p > 0.05). CONCLUSIONS: In refractive accommodative esotropia, a larger amount of esodeviation at the first visit and failure to achieve central fusion are risk factors for developing decompensation. The older age at diagnosis and smaller amounts of hyperopia and esodeviation at the first visit are predictive factors for the cessation of spectacle use.

8.The Influence of Apolipoprotein E epsilon4 and Lipid Profile on Cognitive Function in Community-Dwelling Elders with Mild Cognitive Impairment.

OBJECTIVE: The aim of this study is to examine the relationship between lipid profiles including total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-cholesterol), and low-density lipoprotein cholesterol (LDL-cholesterol) and cognitive function by the occurrence of the apolipoprotein E (APOE) epsilon4 in the community-dwelling elderly individuals with mild cognitive impairment (MCI). METHODS: The total number of subjects was 203 (77 men and 126 women) who were diagnosed with MCI from a Korean project of "Early Detection of Dementia". Aged 65-85 years were included in this analysis. The eight neuropsychological domain from the Korean version of Consortium to Establish a Registry of Alzheimer's Disease neuropsychological test battery (CERAD-K, NP) were conducted to test subjects. The lipid profiles of all subjects were measured including TC, TG, HDL-cholesterol, and LDL-cholesterol levels and the correlation between the lipid levels and the neuropsychological test scores was analyzed by the occurrence of the APOE epsilon4. RESULTS: There was significant correlation between HDL-C and Word List Recall Test (WLRT) of the neuropsychological test score in CERAD-K (NP) in the presence of APOE epsilon4 (WLRT/HDL-C: r=0.163, p<0.05). CONCLUSION: The HDL-C level might be correlated with verbal episodic memory domain in CERAD-K (NP) test in the presence of APOE epsilon4 in community-dwelling elders diagnosed with MCI.

9.Comparison of One Stage Operation with Staged Operation in Malignant Obstruction of the Left Colon and Rectum.

BACKGROUND: The optimal management of malignant obstruction of the left colon and rectum is controversial. METHODS: A retrospective study was performed of 33 patients who underwent one-stage operation(n=11), staged operation(n=20), and palliative colostomy(n=2) at Kwangju Christian Hospital between January 1992 and December 1996. RESULTS: Eleven patients underwent one-stage operations(7 cases with anterior resection, 3 cases with left hemicolectomy, 1 case with Miles' operation). In this group, postoperative morbidity was 36.3% including 1 wound infection(9.0%), 1 anastomotic leakage (9.0%), 1 postoperative intestinal obstruction and 1 pulmonary complication, but there was no postoperative death. The average of hospital stay in this group was 18.1 days and the first day of normal diet was 6.0 days. Twenty patients underwent staged operations including 12 cases of Hartmann's procedure. In this group, postoperative morbidity was 35.0% including 3 wound infections(15.0%), 2 pulmonary complications(10.0%), 1 parastomal hernia and 1 fistula. There were two postoperative deaths as a result of sepsis. The average of hospital stay in this group was 34.9 days and the first day of normal diet was 6.8 days. CONCLUSION: We believe that one-stage operation was of value in management of malignant obstruction of the left colon and rectum in selected patients.

10.A Case of Ecthyma Gangrenosum Associated with Liver Abscess.

Ecthyma Gangrenosum is a rapidly progressing skin infection charact,erized by edema, hemorragic bullae and necrosis. Ecthyma Gangrenosum occurs almost exclusively in severely imrnunocompromised patients during the course of pseudomanas aeruginosa septisemia. A 5 month-old boy was transferred to our department because of multiple well defined central necrotic black-colored large erythematous bullae. In the bacterial cultures of skin, stool and liver aspiration fluid, Pseudomonas aeruginosa was grown. In the abdominal ultrasonogram and computed tomogram, the multiple liver abscess, hepatomegaly and ascites were shown on both hepatic lobes. Herein we report a case of ecthyma gangrenosum associated with liver abscess.
