7.Marginal accuracy and fracture strength of Targis/Vectris Crowns prepared with different preparation designs.

STATEMENT OF PROBLEM: Targis/Vectris restorations provide excellent esthetics. Marginal accuracy is significantly influenced by the preparation design. There were no studies to examine the effect of preparation design on the marginal discrepancy and fracture strength of Targis/Vectris crowns. PURPOSE: This study evaluated the marginal accuracy before and after cementation, and the fracture strength of FRC/Ceromer(Targis/Vectris) crowns according to different preparation design. MATERIAL AND METHOD: Three metal dies with different convergence angles(6degrees, 10degrees, 15degrees) were prepared. Total 30 (10 for each angle) Targis/Vectris crowns were made. The restorations were evaluated for adaptation of the margin before and after cementation, then were compressively loaded to failure. Fracture surfaces of the crowns were examined using a SEM. Results. The mean marginal gap was 49micrometer for 6degrees, 55micrometer for 10degreesand 70micrometer for 15degreesand in clinically acceptable level. The mean marginal gap increased significantly after cementation. The increasing amount during cementation was the largest in the 6degrees group. The crowns on 6o convergence angle had a significantly higher fracture strength than the crowns on 15degrees angle. Mean fracture strength of total crowns regardless of convergence angle was 1390 N, which was higher than all-ceramic crowns. SEM observation showed two-mode fracture pattern. CONCLUSION: From the results of this study, all of the FRC/Ceromer crowns had clinically acceptable marginal accuracy and could withstand the bite force. Moreover, less convergent angle than all-ceramic crown might be recommended for preparation procedure.

8.Short-term Effect of Intravitreal Bevacizumab Injection Preventing Panretinal Photocoagulation-Induced Macular Edema in Proliferative Diabetic Retinopathy.

PURPOSE: To investigate the effect of intravitreal bevacizumab injection in preventing panretinal photocoagulation (PRP)-induced macular edema and visual dysfunction in proliferative diabetic retinopathy. METHODS: We conducted a prospective study of 40 consecutive eyes (20 patients) with proliferative diabetic retinopathy whose visual acuity (logMAR) was 0.2 or less, foveal thickness (microm) was 280 or less, and retinopathy was bilaterally symmetrical. In Group 1 (20 eyes), PRP was performed with intravitreal bevacizumab injection 1 week before the initiation of PRP, but in Group 2 (20 eyes) PRP alone was performed. In all eyes, PRP was completed using the same technique. Best corrected visual acuity (BCVA) and foveal and parafoveal thicknesses were measured before treatment and 1 and 3 months after PRP. RESULTS: Before treatment, the mean BCVA and foveal and parafoveal thicknesses were not statistically significantly different between the two groups (p>0.05). At 1 and 3 months after PRP, the mean BCVA improved and the foveal and parafoveal thicknesses were significantly smaller in Group 1 than in Group 2 (p<0.05). CONCLUSIONS: Intravitreal bevacizumab injection before PRP could help prevent PRP-induced macular edema and visual dysfunction in patients with proliferative diabetic retinopathy and good vision.

10.Blind Sac Approach Using Silastic Block for Cochlear Implantation in Patients with Cholesteatoma.

Cochlear implant (CI) surgery in cholesteatoma is challenging because of the risk of residual or recurrent infection. Although CI could be done with subtotal petrosectomy in single or staged surgery, this surgery needed additional surgical procedures to obliterate the mastoid cavity. This paper describes a new surgical technique for CI surgery in cholesteatoma without external auditory canal closure.
