1.A case of Arnold-Chiari malformation.

The Arnold-Chiari malformation (ACM) is an anomaly of the hindbrain consisting of two components: a variable displacement of a tongue of tissue derived from the inferior cerebellar vermis in the upper cervical canal and a similar caudal dislocation of the medulla and fourth ventricle. Hydrocephalus and meningomyelocele are another abnormalities that associated frequently. We present a case of Arnold-Chiari malformation with hydrocephalus and meningomyeolcele and a brief review of the literatures was added.

Malignant Mixed Mullerian tumors(MMMTs) are unusual neoplasms occumng mostly in the uterus and, ralely, they arise in the ovary. The clinical features of malignant mixed mullerian tumor of the ovary are similiar to other ovarian malignancies. The clinical course is rapidly progressive and fatal. The optimal treatment modalities has remained elusive. The most reliable prognostic criterion is the initial tumor stage and the overall survival was poor. We experienced two cases of malignant mixed miillerian tumor of the ovary, so we report these cases with a brief review of the concerned literatures.

5.Hereditary Optic Atrophy, Neural Deafness, and Peripheral Neuropathy.

The triad of bilateral optic atrophy, hearing deficit, peripheral neuropathy is knoun to be a rare disorder. The authors experienced eight patients in three generations of the same family with this triad of deficits. The disorder developed since their childhood and the course was slowly progressive. Nerve conduction study suggested peripheral neuropathy and sural nerve biopsy was compatible with demyelinating neuropathy, as there were reduction of myelinated nerve fibers in number and focal onion-bulb formation. The mode of inheritance of the family seems to be autosomal dominant, with relatively high penetrance. In many respects, the disorder resembles the cases reported by Sylvester(1958) and Iwashita et al.(1970). But still we do not know the exact etiology of this disorder.

6.A Clinical Study on Chronic Carbon Disulfide(CS2) Intoxication.

Carbon disulfide(CS2), a volatile solvent, has been widely used in the manufacture of the artificial silk because of its highly fat-soluble property. It has been accepted that CSz is mostly absorbed to the human beings by inhalation arld does harms mainly to the central and peripheral nervous systems and the cardiovascular system. But it is conkoversial whether the pathologic changes are due to the direct toxic effects of the CS2 or to the secondary effects of the CS2-induced arteriosclerosis. Recently the authors have observed the six patients of viscose-rayon worker with a longstanding exposure to the CS2 and obtained the results as follows:1. The ages of onset in the 6 subjects (5 males and one female) were ranged from @41 to 53 years(mean;47.1) and the duration of exposure from 11 to 18 years(mean;l4.5). 2. Three of them had no hypertension. 3. Neurologically they showed headache, dizziness, general malaise, gait difficulty, visual and memory disturbances, paresthesia of distal extremities, dementia, parkinsonian features, pseudobubar palsy, spastic gait, dysarthria, hyperreflexia, and reduced or absent ankle jerks. 4. Nerve conduction studies(N=4) revealed that three of them had polyneuropathy of axonal type.

8.Effect of the Isolation Method of Mouse Inner Cell Mass, Types of Feeder Cells and Treatment Time of Mitomycin C on the Formation Rate of ICM Colony.

OBJECTIVE: This study was carried out to evaluate the effect of the isolation methods of inner cell mass from mouse blastocyst, types of feeder cells and treatment time of mitomycin C on the formation rate of ICM colony. METHODS: The inner cells were isolated by conventional immunosurgery, partial trophoblast dissection with syringe needles and whole blastocyst co-culture method. Commercially available STO and primary cultured mouse embryonic fibroblast (pMEF) feeder cells were used, and mitomycin C was treated for 1, 2 or 3 hours, respectively. The formation rate of ICM colony was observed after isolation of ICM and culture of ICM on the feeder cells for 7 days. RESULT: The ICM colony formation rate on STO were significantly higher in partial trophoblast dissection group (58%) than that in immunosurgery (12%) or whole blastocyst culture (16%) group (p<0.05). The formation rate on pMEF feeder layer was higher in partial trophoblast dissection (88%) and whole blastocyst culture (82%) group than that in immunosurgery (16%) group (p<0.05). When mitomycin C treated to pMEF for 2 hours, the formation rate of 88% was significantly higher than those of other conditions. CONCLUSIONS: Above results showed that the efficient isolation method of ICM from blastocyst was the partial trophoblast dissection and the appropriate treatment time of mitomycin C was 2 hours. However, the subculture of ICM colony and characterization of stem cells should be carried out to confirm the efficacy of the partial trophoblast dissection method.

9.The Effect of Quinidine on Digoxin Clearance.

BACKGROUND: Quinidine appeared to increase serum digoxin levels when given with quinidine. Therefore elevated serum digoxin concentrations and clinical toxicity have been reported in patient receiving quinidine. Currently, Bayesian method which estimates the most probable parameters of the drug for each patient from population parameters data is useful approach for adjusting digoxin dosage. To increase the accuracy of Bayesian method, it is desirable to use population parameters of Korean. Therefore we evaluated the effect of quinidine on digoxin clearance in Korea. METHOD: Patient's records from 19 adult cardiac disease without CHF having normal renal and liver function from Seoul National University of Hospital respectively wre evaluated. Digoxin pharmacokinetic parameters, CL and Vd, were obtained from serum concentration of digoxin of single and combined therapy at each steady-state by using bayesian method. RESULTS: This study show that quinidine reduced the total body clearance of digoxin from 2.39+/-0.17 to 1.51+/-0.08ml/min/kg(p<0.05) and reduced the digoxin volume of distribution from 8.57+/-0.29 to 4.98+/-0.19L/kg(p<0.05). This results show that digoxin dosage reduced to 40-50% in Korean, if quinidine therapy is initiated.

10.Chronic-Binge Ethanol Drinking Exacerbates Liver Damage through Oxidative/Nitrosative Stress.

Chronic binge alcohol drinking is a major cause of liver disease including steatosis, chronic hepatitis, and liver cirrhosis. The purpose of this study is to examine the change of oxidative/nitrosative stress in the chronic-binge ethanol model. Using wildtype (WT) mice, we show that chronic binge alcohol drinking develops steatosis and apoptotic cells. Furthermore, the activities of anti-oxidant and mitochondrial enzymes including catalase (CAT), superoxide dismutase 1 (SOD1) and glutathione peroxidase (GPx) were decreased in chronic binge alcohol-treated mice. Protein nitration was also increased in a chronic-binge group compared to pair-fed control counterpart. These findings suggest that chronic binge alcohol drinking plays a critical role in hepatic injury through oxidative/nitrosative stress.
