2.The Effect of Relaxation Technique on Postoperatative Discomfort of the Patients with Lumbosacral back pain.

One of the important activities nursing is the alleviating of discomfort including pain as providing comfort to patient. Postoperativediscom has been underestimated because it is only a part of postoperative physiological process and disappear in time. The study was designed to investigate the effect of relaxation technique on postoperative discomfort of patients with lumbosacral back pain. A total of 40 patients were selected as a subject from August 12th to October 10th 1995 who had been hospitalized at NS and OS nursing units in K medical center. Of them twenty were in the experimental group and the remaining twenty were in the control group in convenience sampling. The tools for study were Relaxation technique, designed to use postoperative setting adequately by Park, and Postoperative Discomfort Scale. The data were analyzed by use of t-test an x2 test. The study was concluded as follows : The main hypothesis, that "the experimental group who used relaxation technique will express a lesser score of postoperative discomfort than the control group who did not use relaxation technique" is divided into seven sub-hypotheses. Hypothesis 1, that "the experimental group will express less postoperative pain than the control group"was supported(t=-7.52, p=0.000). Hypothesis 2, that "the experimental group will express less postoperative feeling of unpleasantness than the control group"was supported(t=-7.04, p=0.000). Hypothesis 3, that "the experimental group will express less postoperative immobilization discomfort than the control group"was supported(t=-6.66, p=0.000). Hypothesis 4, that "the experimental group will show fewer nonverbal expressions of postoperative distress than the control group"was supported(t=-3.75, p=0.01). Hypothesis 5, that "the experimental group will use analgesic medication less frequently than the control group" was supported(t=-4.15, p=0.000). Hypothesis 6, that "the experimental group will have less change in vital sings between the pre and postoperative periods than control group"was supported only for respiratory rate(t=-4.06, p=0.000). Hypothesis 7. that "the experimental group will express less postoperative voiding difficulty than the control group"was not supported(t=0.06, p=0.951). As a result, this study showed that relaxation technique has the effect to postoperative discomfort of the patients with lumbosacral back pain, so the researcher thought it is enable for nurses to help patients undergoing postoperative discomfort.

7.Growth outcome in congenital hypothyroidism.

Congenital hypothyroidism is one of the most common endocrine disease in childhood and it causes not only mental retardation but also growth retardation. There were many papers about evaluation of developmental outcome in congenital hypothyroidism. The aim of this study was to evaluate growth outcome in congenital hypothyroidism. We evaluated 65 patients with congenital hypothyroidism diagnosed at Yonsei University College of Medicine. The results were summerized as follows; 1) The Male to female ratio was 1:1.4 2) Among the 65 patients, under 1 year of age with 16 cases (24.6%), 1~4 years with 19 cases (29.2%), 5~10 years with 22 cases (33.8%), above 10 years with 8 cases (12.3%). 3) Among the 65 patients, 30 cases (46%) had ectopic thyroid, 18 cases (28%) had dyshormonogeneses, 10 cases (15%) had hypoplasia and 7 cases (11%) had aplasia. 4) At the initial diagnosis, all the patients showed decreased T3, T4 and increased TSH level. 5) Before treatment, bone age and height age were delayed but they were normalized after treatment. 6) There was correlation between age of initial treatment and current height percentile. 7) There was significant correlation between initial T3 level and height age delay. 8) In the 4 cases who were diagnosed and treated before the age of 4, final adult height would be achieved within normal range. In conclusion, delayed growth due to congenital hypothyroidism would be prevented by early diagnosis and treatment. Therefore, the recognition of the importance of early diagnosis and treatment is emphasized.

9.Evaluation of the Contaminant Bacteria Isolated from Humidifiers Used in the Intensive Care Unit.

BACKGROUND: This study aims to obtain basic data regarding the proper maintenance and potential role of humidifiers in the development of nosocomial pneumonia. We investigated into a spectrum of bacteria that grew after disinfection in humidifiers that were used in intensive care units (ICUs). The bacteria were isolated at different time points after disinfection. METHODS: We randomly selected 6 humidifiers that were used in an ICU and conducted a series of bacterial culture studies at 24 hr, 48 hr, 72 hr and 168 hr after disinfecting the six humidifiers. The fluid from the humidifider was cultured in the same manner as the dialysate. RESULTS: No bacteria were isolated in most of the 6 humidifiers 24 hr after the disinfection of the instruments. Gram-negative bacilli such as Acinetobacter baumannii, Burkholderia cepacia, and Stenotrophomonas maltophilia were cultured at 48 hr after disinfection. CONCLUSION: Regular and thorough disinfection of humidifiers is necessary to prevent possible spread of infection via humidifiers in the ICU.
