3.PREVENTION OF POSTOPERATIVE SWELLING AND PAIN BY DEXAMETHASONE AFTER OPERATIVE REMVOVAL OF IMPACTED THIRD MOLARS WITH BONE REDUCTION.

The purpose of the present placebo-controlled double-blind study was to investigate the preventive effect of 5mg dexamethasone on swelling, trismus, and pain after removal of impacted mandibular third molars with bone reduction. The participants were consisted of 20 healthy individuals(9 male, 11 female) with bilaterally symmetrical impacted mandibular third molars. Each patient served as his own control. Immediately after removal of impacted teeth, either 5mg dexamethasone or normal saline was injected intramuscularly. On the first day, 2nd day, and 7th day after operation, facial swelling, trismus, and pain were evaluated. On the first day, the difference in the volumetric change of facial swelling was 45% (p<0.05) as measured by Holland's nine pointers facebow. The trismus was reduced by 28% (p<0.01) in test froup. Pain assessed with Visualanaloguescale(VAS) was reduced by 33% (p<0.001) by dexamethasone. It can be concluded that sterokd treatment is recommended in third molar surgery when pronounced postoperative swelling, trismus, and pain are expected.

5.Studies on Serum Lipids Level and Lipoprotein Patterns in Normal Korean Adults and Hypertension.

The author measured the value of serum triglyceride, total cholesterol and phospholipid in normal Korean adults and patients with complicated hypertension. Sera obtained from 295 cases of normal Korean male and female, and also 109 cases of patients with hypertension were analysed for triglyceride, cholesterol and phospholipid. The patterns of serum lipoprotein fraction in 171 normal Korean and 109 patients with complicated hypertension including cerebrovascular diseases and ischemic heart disease were studied by agarose gelelectrophoresis. The results are summarized as follows; 1. The mean value of serum triglyceride, cholesterol and phospholipid in normal Korean were 85.4+/-20.5mg%, 187.9+/-16.7mg%, and 193.4+/-34.6mg% respectively. 2. Sex and age difference showed no statistical significance in the mean values of serum triglyceride, cholesterol and phospholipid in hypertensive patients. 3. The mean values of serum triglyceride in the patients with complicated hypertension such as cerebral thrombosis gave higher value than in the patients with other cerebral complications. 4. The mean value of alpha-lipoprotein, pre-beta-lipoprotein and beta-lipoprotein in normal Korean were 29.6+/-5.0% (142.6+/-30.1mg%), 13.0+/-4.3% (62.7+/-24.8mg%), and 57.4+/-30.1% (276.1+/-56.6mg%) respectively. 5. The alpha-lipoprotein fraction in normal Korean demonstrated the tendency of decreasing as age is progressing in both sex, but pre-beta and beta-lipoprotein fraction gave a tendency of increasing by progressing age in both sex, but was not confirmatory in nature. 6. The alpha-lipoprotein fraction in patients with hypertension gave lower value than in the normal subjects, but the value of pre-beta and beta-poprotein were elevated significantly among patients with complicated hypertension than in normal person. 7. The incidence of hyperlipidemia in normal Korean and patients with hypertension were 31.9% and 66.5% respectively. 8. The relative incidences of hyperlipidemic probable phenotypes in normal Korean were as follows: 7% of normal person had Type II (IIa 3.3%, IIb 3.7%), 3.5% had Type III, and 21.4% had Type IV.

7.Bone graft procedure with endosseous implants : A review of the literature.

Recent development of dental implantology has taken an interest in the bone graft procedure. This is a review of literature, published from 1994 to November 1999. This study is provided by MEDLINE search. In this study, 718 patients received 829 graft with placing 2,677 endosseous implants. In mandible, nonvascularized or vascularized block bone grafts provided better results (success rate 95.2%) than particulate grafts(83.6%). But in maxilla, particulate grafts provided better results(93.7%, 86.2%) and more cases especially in sinus elevation. There were many cases using autogenous bone graft and revealed good results, but allogenic or alloplastic bone graft materials also were used by many surgeons.

8.The Effect of Topical Perfluorophenanthrene on Corneal Neovascularization in the Rats.

The effect of topical perfluorophenanthrene on corneal neovascularization was studied at chemically induced corneal burn on the rats. A central corneal burn was produced with 10% NaOH. Right eye was treated with perfluorophenanthrene and gentamicin eye drops four times a day for five days, the left with balanced salt solution and gentamicin eye drops as control. After five days treatment the rats were sacrified, and the tissue slide were prepared for microscopIc examination. Using slide projector the tissue slide were projected and magnified on the screen. The number of corneal new vessels, the distance of new vessel from the limbus, and the degree of increased corneal thickness were calculated in both groups. As the result there were no statistical difference between two groups. Analysis using ANOVA program showed the fact that statistically significant correlationship between the degree of corneal thickness and the number of the new vessels and the distance from the limbus were found. The result could confirm the mechanism of corneal neovascularization that disarrangement of the compact corneal tissue could allow the formation of the new vessels.

9.Transcatheter Instillation of Urokinase into Loculated Pleural Effusion: Analysis of Treatment Effect.

PURPOSE: To evaluate the indication for intracavitary Urokinase(UK) in the treatment of Ioculated pleural effusion. MATERIALS AND METHODS: We analyzed CT and US in 31 patients who were treated with intracavitary UK in Ioculated pleural effusion. In each patient, a single chest catheter (10-12F) was insected under imaging guidance. When the amount of drainage was less than 100ml/day, UK was instilled through the catheter until less than 50ml/day was drained. On follow-up chest radiographs of more than 1 month, we classified the results of treatment into 3 groups:(1) completely effective (lung expansion, over 80%);(2) partially effective (20-80%); (3) ineffective (below 20%) group. Sonographic pattern of pleural fluid was classified into anechoic, septated, and honeycomb appearances and the thickness of parietal pleura was measured on CT. RESULTS: Sixteen patients were completely effective, nine were partially effective, and six were ineffective. patients with completely or partially effective outcome had anechoic and linear septated appearance on had less than 4mm of parietal pleural thickness on CT. Of six ineffective patients, US showed linear septated in one patient and honeycomb appearance in five patients and the thickness of parietal pleura on CT was 3 mm in one patient, 4ram in two patients, 5mm in one patient, and 6mm in two patients. CONCLUSION: UK instillation through percutaneous catheter was an effective method in the treatment of Ioculated pleural effusion. However, we found near complete reaccumulation of pleural fluid when honeycomb appearance of pleural fluid on US or more than 5mm parietal pleural thickness on CT was observed, which might suggest that we should consider the other kinds of treatment method in those patients.
