1.Study on allergy to medications in staff of Bach Mai Hospital

Allergic reactions to medications are increasing not only in patients but also in medical practitioner. Objectives: 1) Finding out prevalence of drug allergy in Bach Mai hospital staffs and listed medications caused allergic reactions. 2) Comparison some aspects of drug allergy between the health staffs and other groups. 3, Study on clinical symptoms of drug allergy and bring forward views. Subjects: 215 physicians, pharmacists, nurses, techniciants of Bach Mai Hospital and 200 staffs with other jobs. Method: Cross-sectional comparative surveys. Results: The prevalence of allergic reaction in health staffs is higher and these ones had more likely to have drug allergy by 2.5 times compared with other occupational groups. Almost all medications used in clinical practice can cause allergic reactions to the health staffs. Antibiotics were major cause and allergic reactions to familiar beta-lactams happened in the health staffs via contact. Main clinical symptoms of drug allergy were urticaria, prurigo, vomitting, nause and erythema with 3 main syndromes: urticaria, quincke edema and allergic dermatitis. Allergic reactions in health staffs of hospital are needed to pay more attention. Conclusion: the prevalence of drug allergy in Bach Mai Hospital staffs is high and these subjects should be protected from this condition.

2.Application of microplate almar blue assay for rapid detection of drug resistance of M. tuberculosis strains

Background: Recently, microplate almar blue assay has been used commonly in detecting gen related to tuberculosis drug resistance, which provides results after 5-7 days with lower cost compared to traditional methods. Objective: To evaluate the application of microplate almar blue assay for rapid detection of drug resistance of M. tuberculosis strains. Subject and Method: A microplate-based assay which uses Alamar blue reagent - an oxidation reduction dye (MABA), was used for the determination of the anti-tuberculosis drug (isoniazid-INH, rifampicine-RIF, streptomycine-STR and ethabuton=EMB) resistance of 96 M. tuberculosis strains isolated from Vietnamese patients in comparison to those obtained by conversional method. Result: MABA showed to have high sensitivity and specificity in testing the sensitivity to individual anti-tuberculosis drugs (from 82.4% for STR to 93.3% for - INH and from 82.5% for EMB to 98.4% for STR; respectively), as well as for the multi-drug resistant M.tuberculosis (86.4% of sensitivity), highly correlated with the result determined by proportion method. Conclusion: MABA reveals the advantage in shortening test time, in simple performance and lower cost compared with the conversional culture based methods.

3.Anti-Inflammatory and PPAR Transactivational Effects of Oleanane-Type Triterpenoid Saponins from the Roots of Pulsatilla koreana.

In this study, 23 oleanane-type triterpenoid saponins were isolated from a methanol extract of the roots of Pulsatilla koreana. The NF-kappaB inhibitory activity of the isolated compounds was measured in TNFalpha-treated HepG2 cells using a luciferase reporter system. Compounds 19-23 inhibited TNFalpha-stimulated NF-kappaB activation in a dose-dependent manner, with IC50 values ranging from 0.75-8.30 microM. Compounds 19 and 20 also inhibited the TNFalpha-induced expression of iNOS and ICAM-1 mRNA. Moreover, effect of the isolated compounds on PPARs transcriptional activity was assessed. Compounds 7-11 and 19-23 activated PPARs the transcriptional activity significantly in a dose-dependent manner, with EC50 values ranging from 0.9-10.8 microM. These results suggest the presence of potent anti-inflammatory components in P. koreana, and will facilitate the development of novel anti-inflammatory agents.

Extracellular beta amyloid (Aβ) plaques are the neuropathological hallmarks of Alzheimer’s disease (AD). Accordingly, reducing Aβ levels is considered a promising strategy for AD prevention. 3’-O-acetyl-24-epi-7,8-didehydrocimigenol-3-O-β-D-xylopryranoside significantly decreased the Aβ production and this effect was accompanied with reduced sAPPβ production known as a soluble ectodomain APP fragment through β-secretases in HeLa cells overexpressing amyloid precursor proteins (APPs). This compound also increased the level of sAPPα, which is a proteolytic fragment of APP by α-secretases. In addition, 3’-O-acetyl-24-epi-7,8-didehydrocimigenol-3-O-β-D-xylopryranoside decreased the protein level of β-secretases, but the protein levels ofA disintegrin and metalloproteinase (ADAM) family, especially ADAM10 and ADAM17, are increased. Thus, 3’-O-acetyl-24-epi-7,8-didehydrocimigenol-3-O-β-D-xylopryranoside could be useful in the development of AD treatment in the aspect of amyloid pathology.

Methods:
A cross-sectional study was conducted in which 24 Vietnamese adults with bronchiectasis were enrolled from June 2023 to August 2023. All participants underwent bronchoscopy to collect bronchial lavage fluid (BLF) from two bronchial locations: one in the region with the greatest bronchial dilatation and one in the normal bronchi or in patients with all lobes affected, the least abnormal lobe (abnormal BLF [ABLF] and normal BLF [NBLF], respectively). Spontaneously expectorated sputum was also collected.
Results:
Out of 24 cases, the prevalence of mild, moderate and severe bronchiectasis was 14/24 (58.4%), 5/24 (20.8%), and 5/24 (20.8%), respectively. NE concentration and activity were significantly higher in sputum and ABLF than in NBLF (p<0.001). Sputum and ABLF were highly correlated (r=0.841, p<0.001) with no significant difference in NE activity between sputum and ABLF. Higher levels of NE activity were seen in more severe bronchiectasis than in mild bronchiectasis in all samples but were only statistically significant for NE activity in sputum (r=0.418, p=0.042).
Conclusion
NE activity and concentration are elevated in areas of the lung most affected by bronchiectasis. Sputum is a valid surrogate of pulmonary NE levels, as they correlate strongly with ABLF and confirm in a Vietnamese population the relationship between NE activity and disease severity.
