2.A case of proliferating tumor unusually occurring on the lnguinal area.

Proliferating trichilemmal tumor is a relatively uncommon tumor which develops preferentially as a solitary tumor on the scalp of elderly women. Histologically, the tumor i epresents a proliferation of the outer root sheath epithelium in a characteristic pattern, but the presense of cellular atypia, dyskera.totic cells and mitotic figures may lead to a false impression of squamous cell carcinoma. We report a case of proliferating trichliemmal tumor in a 70-yeiir-old male who presented with a slowly growing tumor on his right inguinal area for 2 years. This case is unusual in that the tumor occurred on the inguinal are anoher than the scalp in a male.

3.Influence of weight gain to cardiovascular risk factors.

BACKGROUND: The obese patients increased recently in Korea due to excessive energy intake and decreased physical activity. We are well known that obesity is associted with an increased risk of cardiovascular disease, angina pectoris, hypertension, but not known to influence of weight gain to change in cardio-vascular risk factors. Once, the study of obesity are many but the study of influence of weight gain are small in Korea. This study investigated to influence of weight gain in cardio-vascular risk factors. METHODS: We gatherd case and control group among persons who has taken twice medical examination except disease person from October 1990 to October 1995 at SungAe General hospital. In our study, total 167 case, compare weight gain group(>3kg) with control group to change in cardiovascular risk factor. RESULTS: Sex, age, smoking history, obesity index were not statistically significant difference between two group in the first medical examination. systolic blood pressure, diastolic blood pressure, fasting blood sugar, HDL, total cholesterol/HDL were not statistically significant difference between two group for weight gain. Total cholesterol and triglyceride, LDL were stastistically significant increased. CONCLUSIONS: Weight gain was increased of total cholestrol and triglyceride, LDL, so weight control was important.

4.The Comparison of Effect of Epidural Morphine and of Morphine-Fentany1-Bupivacaine Mixtere for Analgesia Afrer Cesarean Section.

The purpose of this study is to demonstrate a new postoperative analgesic which minimize the physiological changes of patients. Recently, it has become popular to administer opioids to epidural space rather than intravenously or by intramuscular administration. However, the side effects of opioids have limited the usefulness of this procedure. We tried to find out a way of providing postoperative analgesia while minimizing the side effects of opioids. This study has tested the effects of administering a mixture of small doses of morphine, fentanyl and bupivacaine and epidural morphine alone. The data of this study has taken from 20 paturients who have scheduled for cesarean section. We have classified into two groups which consist of 10 paturients each. We have administered epidural morphine alone to the first group and a small doses of morphine-fentanyl-bupivacaine mixture to second group. We have evaluated analgesic effects by comparing both groups' pain scores of the operative day and of the first postoperative day and evaluated such side effects as the degree of itching, nausea, vomiting, dizziness and respiratory depression by asking to paturients. This study has found the followings: 1) Pain scores of the first group in the operative day and the first postoperative day are 1.2+/-0.4 and 0.4+/-0.5. The second group's pain scores are 0.2+/-0.4 and 0, and the second group's pain scores have significantly reduced (p<0.05). 2) Frequencies of such side effects as itching, nausea and vomiting are higher in the first group (8) than in the second group(4). In conclusion, small doses of morphine-fentanyl-bupivacaine mixture has demonstrated a more effective postoperative analgesia with minimum side effects in pain controlling after cesarean section than epidural morphine alone.

7.Immunohistochemical obervation of EGF,TGF - alpha and EGF-R in psoriatic epidermis.

Epidermal growth factor(KGF) and transforming growth factor arpha(TGF-a) are polypeptides of 53 amd 50 amino acid residuies. Both bind to epidermal growth factor receptor (EGR-R) leading to phosphorylation of the receptor, enhancement of its tyrosine-specific kinase activity and ultimately to stimulation of cell growth. To study he role of EGF, TGF-a, and EGF-R in differentiation and hyperproliferation of cell, we se lected psoriasis vulgaris, because the affected keratinocyte may house both an abnormally increased proliferative capacity and an abnormally differentiated state. The biopsy specimens were taken from involved and uninvolved skin of 20 patients with psoriasis and immunoperoxidase studies with formalin-fixed paraffin-embedded tissues were performed with EGF, TGF-a and EGF-R useing the Vectastain ABC irnmunoperoxidase stain system. The antibodies were used at a concentration of 6 ug/ml In involved psoriatic skin, distributions of TGF-a and EGF-R were increased in all layers of epidermis as compared to normal, uninvolved psoriatic skin, in which chev were showed to the basal and parabasal layers. However, distribution of EGF was weekly positive in the basal layers of epidermis in both involved and uninvolved skin with no difference between toem. These results suggest that increased distribution of TGF-a and EGF-R may be involved in hypoproliferative state of epidermal keratinocytes in psoriatic lesion.

8.Crotamiton Resistant Scabies.

A case of crotamiton resistant scabies is described in a 21-year-old male patient The scrapings using mineral oil from lesions on the wrist and finger webs showed mites and eggs, typical of scabies. Ten percent crotamitn cream was applied 10 times in one month without any effect. We could not find any source of reinfection in the patient. After above treatment the scrapings still showed mites. and eggs. Retreatment with 1% gamma benzene hexachloride cream was effective in clearing of the skin rashes and itching sensation.

9.Osteogenic Sarcoma following Irradiation for Retinoblastoma.

A case of Osteogenic sarcoma arised from orbit 4 years after the radiation therapy for retinoblastoma in a 9 years old girl is presented. 4 Years ago, under the diagnosis of retinoblastoma, the left orbital exenteration was performed and was treated with x-ray radiation giving 4.500 gamma tumor dose in 4, weeks. On admission, the left orbital cavity was filled with hard mass and orbital bone revealed extensive destruction with osteoplastic changes on x-ray, and diffuse sarcomatous growth of anaplastic cells with abundant osteogenesis and osteoclastic activity microscopically. It is of authors' opinion that osteogenic sarcoma of orbital bone in this presented case might developed by irradition on retinoblastoma.

10.Morphological and biochemical alteration by ultraviolet B irradiation in hairless mouse.

BACKGROUND: It is well known that ultraviolet light(UVL) may cause skin cancer, decrease immune function and promote skin ageing. It is also known that an increase of chlorofluorocabons as air pollution, which csuses the depletion of ozone of the earth in ihe atmosphere, enables harmful ultraviolet-B(UVB) to reaeh the surface of the earth more easily: The purpose of this study is to determine the harmful effect of UVB on the skin by observing coicurrently the morphologieal snd biochemical changes in the UVB irradiated skin. OBJECTIVE: The animal used was the hairless mouse(Hr+/Kud) which are considered to be the most suitable for a UVB irradiation experiment. The Fluorescent sunlamp(Toshiba FL, 20SE, Toshiba electrie Co., Tokyo, Japan) was used as a source of UVB. METHOD: The skin of the back and ear was irradiated by an increasing doses of UVB. In morphological changes, the slteration in keratinocytes and Langerhans cells in cell number and morphology were observed. In biochemical changes, activities of tte superoxide dismutase and eatalsse, which scsvanges reactive oxygen species(O and H) producec in the skin by UVB irradistion were assayed. RESULT: Sunburn cells appeared st 60mJ/cm of UVB and increased in number in proportion to the UVB irradiation with dose dependent pattern. The Langerhans cell decreased in number in proportion to UVB irrsdiation in dose dependent manner(half maximum dose was 60mJ/cm), and was not found at 1J/cm. The morphological changes of the Langerhans cells, such as a loss of dendrites and granulation, were noticed at 60mJ/cm of UVB irradiation. The enzyme activity of catalase decreased in proportion to UVB irradiation. The enzyme activity of SOD was not changed by UVB irradiation, however, it significantly increased at 1000mJ/cm UVB irradiation. CONCLUSION: This study suggest that UVB irradiation to the skin causes a reduction in the immune funetion and alters the normal biochemical function of the skin.
