A clinical analysis of the septic hip was made on 15 patients, who had been undergone operation in Chung Nam National University Hospital from April, 1977 to Dec., 1978. The following results were obtained. 1. The ratio of male and female was 2.8:1. 2. The most common age group was 7~12 years (5 cases, 33.3%), and next common was 13–20 years (4 cases, 26.7%). 3. The most common causative microorganism was staphylococcus aureus(66.7%). 4. The most common duration of symptoms were less than 5 days (6 cases, 40%), and next common was 6~10 days (4 cases, 27%). 5. The patients, more than 10 days duration of symptoms, had late sequela in 80% but the patients, less than 10 days, had good prognosis in 80%. 6. In authors experience, the most effective treatment for septic hip was early diagnosis and hospitalization, followed by surgical decompression with specific antibiotics.

Articular cartilage have dual functions of shock absorber and bearing surface in a moving joint. Articular cartilage is avascular, so the chondrocytes in mature adult must receive their nourishment solely from synovial-fluid perfusion, and it is known that alteration of synovial-fluid perfusion causes degeneration of articular cartilage. Now, in orthopedic fields, the corrective cast immobilization and traction is in popular use. In order to study the cartilage changes induced by this long-term cast immobilization and traction, compression-immobilization and condylar resection of knee joint in rabbits were performed, and weekly histoiogic examinations of articular cartilage up to 5 weeks were followed. The observations were as follows: 1. Degenerative changes of articular cartilage were observed respectively with compression-immobilization and condylar resection, and the severity of the histologic changes appeared to be proportional to the duration of compression-immobilization and condylar resection. 2. Earlier degenerative changes of articular cartilage in condylar resection group than in compression-immobilization group were noted, and this earlier degenerative changes in condylar resection group suggests that hemarthrosis may play a role in cartilage degeneration. 3. Histologically unchanged calcific cartilage zone and mildly hypertrophied subchondral bone were noted in compression-immobilization group, but vascular invasion to calcific cartilage zone and subchondral osteoporosis were noted in condylar resection group.

On general principle, spondylolisthesis does not need surgical treatment but special features may arise in any such case to change the clinical picture and to indicate the necessity for operative intervention. There have been described a number of techniques and their combinations in the treatment of spondylolisthesis. Despitc continuing efforts to develop better techniques of lumber fusion, failure has been relatively common. In 1953, Watkins dcscribed a lateral approach to the transverse processes. Rombold recently described a lateral mass fusion of a single level for spondylolisthesis using extensive autogenous iliac grafts. He reported 96% roentgenographic fusion. It is the purpose of this paper to review a series of adults with symptomatic spondylolisthesis (16 cases) who were surgically treated by posterolateral fusion. 1. The site incidence of spondylolisthesis was 68.8%(11 cases) in L-5, 25%(4 cases) in L-4 and 6.2% (1 case) in L-4 & L-5. 2. Satisfactory resutts according to Gills criteria was 94% (15 cases). 3. In follow-up study, the younger the adult, the better the clinical results. 4. The fusion rate of intervertebral joint in roentgenographic finding was 100%. 5. There were no significant differences in duration and result of solid fusion between cases applied with hip spica cast and lumbosacral back brace or Norton-Brown brace.

The appendiceal tuberculosis is a rare from of skeletal tuberculosis in child and adults, arising from a focus or foci in posterior compartment of spine. This from may be unilateral and may be isolated or present at multiple levels, characterieed by slight change of the disk space and deformity, but large psravertebral abscess are invariable. Extradural extension may lead to the development of paraplegia. We had experienced one case of appendiceal tuberculosis at L1 and treated with antituberculosis medication and radical curettage, followed with body jacket cast.

The intraosseous ganglion is a rare cystic lesion of bone most frequently located in the subchondral epiphysis of long bones with or without direct communication with a joint cavity. This case is reported to call attention to this lesion which deserves consideration in the differential diagnosis of solitary cystic lesions of bone. We were reported a 59-aged woman without direct communication with the knee joint and osteoarthritis which was treated with the curettage and chip-bone graft.

Paget's disease is a disorder of middle or older aged persons in which abnormal osteoclastic and osteoblastic activity spontaneously occurs within the affected portion of any particular bone or bones. This causes a gradual gross structural alteration through continuous remodeling. The occurance of Pagets disease is extremely rare in China, Japan, Middle East, and Africa. We have experienced one case of Paget's disease of bone affecting the spine and femur, which is reported in this paper with brief review of literature.

The head injuried patient with skeletal trauma is a challege to the orthopedic surgeon. The orthopedist initially may face the difficult management of fracture and dislocation in the patient associated with head injury. The management of the injuries to the head takes precedence over extremity injuries initially, but the orthopedist must be managed aggresively with assumption that full neurologic recovery will occur. This report analysed the treatment and results in 85 cases fracture and dislocation with head injury among the 59 patients at the Chungnam National University Hospital during the period ranging from March, 1980 to March, 1983. End result obtained are as follows: 1. The sex ratio between the male and female was 3.9: 1 and the prevalent age distribution was from 21 to 30 years of age. 2. The motorcycle accident was most common cause of the trauma and the next most frequent one was the automobile accident. 3. Type of head injury were contusion or concussion, intracranial hemorrhage, linear skull fracture, depressed skull fracture and open skull fracture. 4. Better result were obtained by operative treatment in femur fracture, especially spastic group. 5. The incidence of delayed diagnosis was 8 cases and the delay in diagnosis ranged from 15 to 95 days with an average of 38 days. 6. We, consulting with neurosurgeon, must assume that full neurologic recovery will take place and so open reduction and internal fixation should be carried out as quickly as possible.
