1.A clinical study on wheezing of mycoplasma pneumoniae pneumonia in children.

The relationship between respiratory infections and exacerbations of wheezing in patients with wheezy bronchitis or asthma has been described. Most respiratory infections were viral or Mycoplasma pneumoniae. This study was performed to evaluate the effect of Mycoplasma pneumoniae infection on bronchial reactivity and atopic background. 106 patients with Mycoplasma pneumoniae pneumonia who were admitted to the department of pediatrics, Fatima Hospital, Taegu during the period of two years from January 1989 to December 1990 were involved in this study. The results were as follows 1) Out of 106 cases, 69 were male,37 were female and 34.9% of the cases were between 5~6 years of age. 35 cases(33.0%) had wheezing and the incidence of wheezing were higher in male than in female. 2) The incidence of wheezing was high in the patients with serum IgE level higher than 200IU/ml, patients with past and family history of allergy and the patients fed with formula feeding in infancy. 3) Eosinophils and the associated maxillary sinusitis had no effect on the incidence of wheezing. 4) The recurrence of wheezing was higher in Mycoplasma pneumoniae pneumonia with wheezing(26.8%) than in patients without wheezing(3.8%).

2.Recurrent Vestibulopathy: Clinical Characteristics and Efficacy of Combination Therapy .

BACKGROUND AND OBJECTIVES: Recurrent vestibulopathy is defined a disease characterized by more than a single episode of vertigo of duration characteristic of endolymphatic hydrops but without auditory or clinical neurological symptoms or signs. To investigate the clinical characteristics and the efficacy of combination therapy, we analyzed the clinical records of the patients diagnosed as recurrent vestibulopathy. MATERIALS AND METHOD: Clinical records of sixty four patients diagnosed as recurrent vestibulopathy were retrospectively reviewed. The data on age, sex distribution, natural history, family history of recurrent vestibulopathy, concurrent headache, caloric response was analyzed. The efficacy of combination therapy for vertigo control in the patients with a minimum 24-month follow-up was also evaluated. RESULTS: Mean onset age of recurrent vestibulopathy was 43 years and there was a female preponderance. Concurrent headache and elevated SP/AP ratio in electrocochleogram was frequently observed in these patients. After the combination medical therapy, patients with severe recurrent vestibulopathy showed significant decrease in the number of vertigo spells with 37.5% of complete control of vertigo. CONCLUSION: As a distinctive clinical disorder with unknown cause, recurrent vestibulopathy should be always considered to the patients complaining recurrent episodic vertigo. Combination therapy individualized to the symptoms and signs of the patients with recurrent vestibulopathy might be effective in reducing the frequency of vertigo attacks. Further case-control studies with large population should be necessary.

3.Change of Platelet Count and Mean Platelet Volume after Intravenous Immunoglobulin Injection in Acute Idiopathic Thrombocytopenic Purpura.

PURPOSE: The platelet synthesis is extremely variable after intravenous immunoglogulin injection (IVIG) in acute idiopathic thrombocytopenic purpura (ITP). To investigate the size variation of platelet according to the time sequence of ITP, the relationship between platelet number and mean platelet volume (MPV) was analyzed. METHODS: Twenty acute ITP patients who showed abrupt increase of platelets within 48 hours after IVIG were selected. We checked the platelet number and MPV, thereafter analyzed the relationship. RESULTS: At the early phase of ITP before IVIG, MPV was normal or slightly decreased. However, as the number of platelet increased after IVIG, MPV increased together until platelet count reached 100, 000/mm3. The MPV decreased afterward, therefore the platelet mass was preserved. CONCLUSION: At the early phase of ITP before the increase of platelet, MPV decreased in spite of low number of platelet. After IVIG, there was an abrupt increase of MPV with platelet number. There might be some contributing factors for these, particularly IL-6, IL-11 and thrombopoietin. Now, we need more experimental data to explain these findings.

5.Temporomandibular Joint Segmentation Using Deep Learning for Automated Three-Dimensional Reconstruction

Purpose:
Cone beam computed tomography (CBCT) is widely used to evaluate the temporomandibular joint (TMJ). For the three-dimensional (3D) assessment of the TMJ, segmentation of the mandibular condyle and articular fossa is essential. This study aimed to perform deep learning-based 3D segmentation of the mandibular condyle on CBCT images and evaluate the performance of the segmentation.
Methods:
CBCT scan data from 99 patients (mean age: 53.3±19.2 years) diagnosed with TMJ disorders were analyzed. From the CBCT images, sagittal, coronal, and axial planes showing the mandibular condyle were selected and combined to form two-dimensional (2D) images. The U-Net deep learning model was used to exclusively segment the mandibular condyle area from the 2D images. From these results, 3D images of the mandibular condyle were reconstructed. Accuracy, precision, recall, and the Dice coefficient were calculated to appraise segmentation performance in each plane.
Results:
The average Dice coefficient was 0.92 for the coronal and axial planes and 0.82 for the sagittal plane. The CBCT image-based segmentation performance of the mandibular condyle in the coronal and axial planes exceeded that in the sagittal plane. The sharpness and uniformity of the 2D images affected segmentation performance, with segmentation errors more likely occurring in non-uniform images. Certain segmentation errors were corrected through software processing. Finally, the segmented mandibular condyle images were applied to the CBCT data to reconstruct a 3D TMJ model.
Conclusions
Mandibular condyle 3D segmentation on CBCT images using U-Net may help evaluate and diagnose TMJ disorders. The proposed segmentation method may assist clinicians in efficiently analyzing CBCT images, particularly in cases involving anatomical abnormalities.

We expressed anti-HBsAg human antibody fragment (B7 Fd) using pRSET-A vector and BL21(DE3)pLysS strain of E. coli. B7 Fd is composed only of variable domain (VH) and CH1 constant domain of IgG heavy chain molecule. This Fd molecule was solubilized using guanidine salt and then expressed in the form of inclusion body and successfully refolded into functional antibody molecule by rapid dilution in refolding buffer. B7 Fd reacted with d epitope of hepatitis B virus surface antigen (HBsAg). Its affinity was determined by competition enzyme-linked immunosorbent assay (competition ELISA). The K value of B7 Fd was 3.3 * 10.

7.The Use of Total Intravenous Anesthesia Without Muscle Relaxation in Amyotrophic Lateral Sclerosis Patient.

66-year-old woman with amyotrophic lateral sclerosis was scheduled for closed reduction and external fixation of left tibio-fibula. Total intravenous anesthesia using 2% propofol and remifentanil was done without muscle relaxant to avoid possible postoperative respiratory complication and pulmonary aspiration. The surgery had done without difficulty and she recovered from anesthesia promptly without respiratory complication and pulmonary aspitation. She discharged satisfactorily 15 days after surgery.

8.A Case Report of Neurologic Sequelae Following Spinal Anesthesia.

Neurologic deficits following spinal anesthesia are extremely rare since the use of sterile spinal anesthesia kit and the relative small doses of the local anesthetics employed. We have experienced 29 years old healthy female patient developed left S root radiculopathy with dysesthesia and motor weakness of left foot after spinal anesthesia for appendectomy. And the patient recovered spontaneously after 6 months without specific treatment except physical and symptomatic therapy.

9.A Case of Left Brachiocephalic Vein Total Occlusion Due to Acute Thrombosis Soon after Permanent Pacemaker Insertion.

Symptomatic pacing lead-associated thrombosis is very uncommon occurring in 0.5-3.5% of pacemaker implants. Especially thrombisis-induced total occlusion occures almost in late stage over several months to years but acute thrombosis occurring several days after venous pacing has not been reported. In this case, We performed upper limb venography in the patient who presented edema and pain of neck, left upper limb and headache as well as intermittent cough occurring in bending forward. A venogram confirmed acute thrombus completely occluding the left brachiocephalic vein and the patient received intravenous heparin and was maintained on warfarin. Repeated veno- graphy after treatment for 30 days revealed persistent thrombus with total occlusion which not be improved signi- ficantly copmpared to previous venogram and collateral veins diverting the blood to the contralateral side and into the superior vena cava was developed. The patient's symptoms resolved almost and that is likely to be due to the development of collateral venous channels.

10.Aberrant Internal Carotid Artery in the Middle Ear: A Case Report.

Aberrant internal carotid artery in the middle ear is a rare disease which, if unrecognized on radiological studies, can lead to serious complications during tissue biopsy. We report the imaging features of a case with aberrant internal carotid artery in the middle ear. A 60-year-old woman visited our hospital because of hearing difficulty on the right side. Temporal bone CT showed a well-defined mass of the right middle ear and lateral bony defect in the carotid canal adjacent to the mass. After arterial phase temporal bone CT with spiral CT andangiography, the mass could be diagnosed as aberrant internal carotid artery in the middle ear.
