1.A Clinical Study of Hand Tumors

Hand tumorsare many and varied, although benign tumors are common. The pathology and histology of these tumors are not unlike these characteristics of the tumor when it occurs elsewhere. Early diagnosis and proper treatment of all hand tumors have been emphasized since the growth of tumors and pain can cause disturbance of intrinsic function of the hand. For the period of 10 years from 1977 to 1986, 105 cases of hand tumor were treated surgically and the results of clinical observation were as follows ; l. Among 105 cases, benign tumor was 101 cases(96.2%) Bone origin tumor was 20 cases, in which enchondroma and enchondromatosis were 17 cases(70%). Soft tissue origin tumor was 85 cases, in which ganglion was 54 cases(63.5%). 2. Among 105 cases, 42 cases were male and 63 cases were female. In age distribution, it showed even distribution. 3. In the duration of wymptom, 51 cases were below 1 year(48.6%). 4. The tumor occured 85 cases in soft tissue, 14 in phalanx, 7 in metacarpal and 1 in carpal bone. 5. The tumors were treated by excision or curettage and bone graft with good improvement, but postoperatively recurred in 2 cases of Hemangioma. Ampuation was performed in 3 cases of malignant tumors without recurrence.

2.Treatment of Trochanteric Fracture of the Femur with Flexible Intramedullary Nailing

The purpose of early reduction and internal fixation of the trochanteric fracture is early mobilization and reducing the complication rate. The morbidity and mortality rates remain high in elderly patients with this type of injury. This is due to reduced tolerance to prolonged bed rest, lenghy operation, and massive soft tissue dissection and blood loss. We feel the Ender method has the advantages of ease performance, decreased operating time, and immediate weight bearing in most cases. Total 23 cases of trochanteric fracture of the femur was treated at the Department of Orthopaedic Surgery of National Medical Center during the period from June, 1985 to December, 1987. The results were as follows : l. Out of 23 patients, 16 patients were female and the mean age was 71 year old. 2. The most common cause was the injury from slip down. 3. By the Tronzo classification, the most common type was type III. (14 cases) 4. The average duration from accident to operation was 4.5 days and average blood loss was 210 ml during operation. 5. The average bony union was obtained in 16.4 wks. 6. For prevention of distal pin migration, we performed circumferential wiring through the eye of distal pin in 6 cases and resulted in no migration until complete bony union.

3.The Changes of Serologic Markers in Pneumoconiosis of Coal Workers.

BACKGROUND: Pneumoconiosis is a parenchymal lung disease that results from the accumulation of coal dust in the lungs and the consequent tissue reaction. To evaluate the role of various personal factors in pneumoconosis and the significance of some serologic markers for assessing the disease activity related to pneumoconiosis, the Rheumatoid Factor(RF), α1-AT, C-Reactive Protein(CRT), ceruloplasmin and fibrinogen levels were measured. METHOD: All the patients were males, 45-76 years old, and the mean duration of coal dust exposure was 23.2 years. 51 patients were classified as having Simple Pneumoconiosis (SP), 59 had Progressive Massive Fibrosis (PMF). Fifty eight men with ages ranging from 26-70 years wer used as normal controls. The serum RF and CRT were titrated using an Autochemistry analyzer (HITACHI 7150 : Japan) and the α1-AT and ceruloplasmin levels were measured using a Nephelometer (Behring Nephelometer : Germany) and the fibrinogen levels were estimated by using and Autoanalyzer for hematologic coagulation. RESULT: There was a higher RF level in the SP, and PMF groups than in the control groups but there was no statistical difference. The CRP, α1-AT, and ceruloplasmin levels were also higher in the SP, and PMF groups. However, the fibrinogen concentration was within the normal ranges in both the SP and PMF groups. CONCLUSION: The CWP (Ed note : Define CWP) patients had significantly higher CRP, α1-AT, and ceruloplasmin levels compared to the control group. It is believed that these serologic changes could be used as a marker of the disease activity.

6.An Experimental Study on Bioeffects of Hyperbaric Ambient Air and Oxygen in Rats.

It Is known that free radical is produced by many environmental factors and leads to a variety of pathophysiological lesions often resulting in metabolic Impairment and cell death. Many researches have been performed to limit production of the tree radical. The superoxide dismutase (SOD) has been known to cure of disease relevant to oxygen poisoning. The bis-carboxylethylgemanium sesquioxide (Ge-132) has been suggested to show scavenging activity to precent oxygen damage from lesions. In this study, bioeffects of SOD and Ge-132 for dismutation and scavenging ol the harmful free radical were investigated in Sprague-Dawely rats exposed to the hyperbaric oxygen (HBO 100% oxygen) and hyperbaric ambient air (HAA) under 3.0 atmospheres absolute (ATA) for 180 minutes. When rats were treated with SOD alone anti SOD plus Ge-132 after exposed to HBO, concentrations of potassium and sodium tended to significantly decrease to those in control at levels of p<0.05 and p<0.01, respectively. In contrast, when rats were treated with the SOD only and the SOD only Ge-132 after HBO exposure, creatine phosphokinase significantly increased to the normal level in control. To see whether the SOD anti SOD plus Ge-132 could heal lesions due 4o the tree radicals, rats were exposed to the HBO environment and treated with the SOD alone and SOD plus Ge-132 and then those lung tissues were histopathologically observed using transmission electron microscope (TEM). Lung In exposure to the HAA environment was congested and its alveolar sac was filled with several erythrocytes. Lung to the HBO environment was filled with more erythrocytes than in the HAA. In addition, there were severe lesions of cellular necrosis, hemorrhage, infiltration of inflammatory leucocytes. When rats in exposure to HBO were treated with SOD alone or SOD plus Ge-132, no pathological changes in the lung were observed in comparison with control group. These data indicated that the Ge-132 may synergistically Influence the SOD to remove the tree radicals harmful to tissue.

7.Petrous Apex Cholesterol Granuloma Presenting as Endolymphatic Hydrops: A Case Report.

A petrous apex cholesterol granuloma (PACG) is the most common lesion of the petrous apex mass. Affected patients present with various symptoms such as hearing loss, vertigo, headache, tinnitus, facial spasms, and diplopia. We report the case of a 32-yr-old man with a PACG, who was first misdiagnosed with Meniere's disease. He was placed on a low-salt diet, and prescribed medication from another hospital, for several months, but the symptoms persisted and worsened. The patient presented to the emergency room complaining of left facial twitching and numbness. To rule out a central neurological lesion, temporal bone magnetic resonance imaging was carried out and a 2.5 cm mass with high signal intensity on T1- and T2-weighted imaging, without gadolinium enhancement, was found. Because of the hearing and facial problems, we drained cholesterol-bearing material via an infralabyrinthine approach using a computer aided image-guided surgical device, the BrainLAB(R). After the operation, the vertigo and hearing loss were no longer present. It is likely that the patent's Meniere's disease-like symptoms were due to the compression of the endolymphatic sac by a PACG.

BACKGROUND AND OBJECTIVES: We evaluated the two-year clinical outcomes in patients with angiographically intermediate lesions according to the plaque burden and treatment strategy. SUBJECTS AND METHODS: We prospectively enrolled patients with angiographically intermediate lesions (diameter stenosis 30-70%) with an intravascular ultrasound (IVUS) minimum lumen area (MLA) <4 mm2 with 50-70% plaque burden of 16 Korean percutaneous coronary intervention centers. Patients were divided into medical therapy group (n=85) and zotarolimus-eluting stent group (ZES; Resolute) group (n=74). We evaluated the incidences of two-year major adverse cardiovascular events (MACE). RESULTS: A two-year clinical follow-up was completed in 143 patients and MACE occurred in 12 patients. There were no significant differences in the incidences of death (1.3% vs. 3.0%, p=0.471), target vessel-related non-fatal myocardial infarction (0.0% vs. 0.0%, p=1.000) and target vessel revascularizations (7.8% vs. 4.5%, p=0.425) between medical and ZES groups. Independent predictors of two-year MACE included acute myocardial infarction {odds ratio (OR)=2.87; 95% confidence interval (CI) 1.43-6.12, p=0.014}, diabetes mellitus (OR=2.46; 95% CI 1.24-5.56, p=0.028) and non-statin therapy (OR=2.32; 95% CI 1.18-5.24, p=0.034). CONCLUSION: Medical therapy shows comparable results with ZES, and myocardial infarction, diabetes mellitus and non-statin therapy were associated with the occurrence of two-year MACE in patients with intermediate lesion with IVUS MLA <4 mm2 with 50-70% of plaque burden.

9.Comparison of MMPI Characteristics in Ulcerative Colitis, Hemodialysis patients & Normal Control Group.

OBJECTIVES: For many years, inflammatory bowel disease has been thought to have a strong psychosomatic component, but recent prospective studies show no evidence of correlation between various psychologic factors and the disease activity. However, chronic renal disease still places unique stresses on the hemodialysis patients, with consequent emotional reactions. The aims of this study are to investigate the psychologic factors of ulcerative colitis and hemodialysis patients using MMPI scales, which are objective psychological test, thus give aid to psychological understanding of these patients. MOTHODS: We investigated 23 ulcerative colitis patients in remission followed at outpatient department of Hanyang University Hospital and 25 patients receiving hemodialysis between June, 1994 and August, 1995. Twenty students were selected for normal control group. We utilized T-score of each MMPI scale for analyzing characteristics of each group and regard T-score over 70 or below 40 as abnormal. RESULTS: The average T-scores of each MMPI scale in ulcerative colitis, hemodialysis patients & normal control group were within normal range between 40 and 70, except for T-score of Es scale in hemodialysis patient group(34.5). The results of comparison between each MMPI scale of two patients & normal control group were as follows. 1) L scale was significantly elevated in ulcerative colitis & hemodialysis patients group compared with that of normal control group. F scale was significantly elevated in hemodialysis patients compared with that of ulcerative colitis patients & normal control group (p<0.01). 2) Hs(p<0.01), D, Pt(p<0.05) and Sc(p<0.01) scales were significantly elevated in hemodialysis patients compared with those of ulcerative colitis patients & normal control group. 3) Es scale was significantly lower in hemodialysis patients compared with that of ulcerative colitis patients & normal control group(p<0.01). CONCLUSION: The result presented in this study reflects emotional disturbances, functional impairment, stressful life condition, excessive hypochondriacal thinking, depressed mood and egocentric tendency of hemodialysis patients compared with ulcerative colitis patients & normal control group. This result also reflects the projective tendency, hostility, difficulty in rapport formation, psychologic disturbance, obssesive and rigid personality, anxiety and retiredness of hemodialysis patients compared with ulcerative colitis patients & normal control group. On the other hand, we can't find any statistically significant differences between ulcerative colitis and normal control group.

10.Association of Blood Mercury Level and Neurobehavioral Performance in Korean Elementary School Students.

OBJECTIVES: This study was conducted to examine the effect of blood mercury on neurobehavioral performance in Korean elementary school students. METHODS: We measured the blood mercury concentration in 1,077 elementary school students from Seoul, Busan, Daegu and Gwangju, then the students underwent computerized neurobehavioral test. RESULTS: The mean concentration of blood mercury of study subjects was 2.10 microg/L. An increased blood mercury was associated with an increased reaction time in digit addition and symbol digit. CONCLUSIONS: The blood mercury concentration of study subjects was very high compared to findings in a foreign country. We found several associations between blood mercury level and neurobehavioral performance.
