1.Association of Sexual Disorders with Peripheral Neuropathy in Alcohol Dependence.

The alcoholic neuropathies developed in approximately 34% of chronic alcoholics and the sexual dysfunction had been experienced in 8-54% of male alcoholics(Schiavi 1990). The aims of this study were to identify the prevalence of subclinical polyneuropathies and sexual disorders in alcohol dependence, and to evaluate the association between them. The nerve conduction velocity(NCY), electromyography(EMG), and pudendal somatosensory evoked potentials(SEPs were tested for the male alcoholics(N=34) and controls(N=17 for NCV & EMG, N=25 for pudendal SEPs). The pudendal SEPs were measured by the following procedures, in which we simulated the dorsal nerve of penis attached by the ring electrode(stimulus intensity, three times of threshold ; stimulus rate, 1-4.7Hz: stimulus duration, 0.1 or 0.2msec), and recorded at the scalp(active electrode, 2cm behind Cz ; reference electrode, Fz. The NCV and EMG detected signs of peripheral neuropathies in 79.4% of alcoholics. Among the alcoholics, 64.7% were abnormal on the pudendal SEPs. Among the alcoholics who revealed abnormality on EMG and NCV, 81.4% were abnormal on the pudendal SEPs, in which 51.9% were not responded. The P1 latencies between peripheral neuropathies and sexual disorders in the alcoholic. The prevalence of subclinical neuropathies and sexual disorders seemed to be much higher in alcohol dependence that expectation, and these two problems were relatively correlated, and our results suggested that the peripheral polyneuropathies were one of the prerequisites of sexual disorders.

2.Two cases of Cheilitis Granulomatosa.

We report herein two cases of cheilitis granulcmatosa which were beneficial effects with intralesional triamcinolone acetonide. Casc 1, 27-year-old, showed both lip swelling with moderate scrotal tongue for 7 years and Case 2, 50-year-old, -had lower lip swelling for 1. 4 years. Histopathologic findings show lymphohistiocytic infiltration in the dermis with focal tuberculoid granuloma in case 1 and ill-defined granuloma in case 2.

3.Effectiveness of the intraoperative chain cystourethrogram in "No-incision" bladder neck suspension.

"No-incision" bladder neck suspension, Gittes` operation, is a simple & effective technique among the various surgical technique. But occasionally patients was complained of long - term urinary retention and voiding difficulty after operation because or excessive traction of the nylon suture. We prevented them using chain cystourethrogram intraoperatively. This operation was performed under the spinal anesthesia which the patients can do the Valsalva maneuver during operation. Posterior urethrovesical angle was not significantly different between preoperative & intraoperative chain cystourethrogram. We corrected wide posterior urethrovesical angle to about 100 (mean angle of the normal range). Normal voiding after catheter removal & duration of admission was signiricantly decreased in intraoperative chain cystourethroram group(p<0.05). During 11 months follow-up, overall cure rate was 84.6% without long-term postoperatively voiding difficulty. We concluded that intraoperative chain cystourethrogram was useful methods for good results.

6.A Case of Dyschromatosis Symmetrica Hereditaria.

We described a case of dyschromatosis symmetrica hereditaria in a 20-year old male patient who had spotty hyperpigmentation mingled with patches of hypopigmentation on the dorsal surfaces of both hands, feet., elbows and knees. In his family. the mother and elder brother were also affected by this disease. The histologic finding of the pigmented macule showed mild hyperkeratosis and marked increase of pigmentation at the basal cell layer, and that of the depigmented macule showed a decreased amount of melanin.

Pigmented neurofibroma, a variant of neurofibroma, has rarely been observed in patients with or without neurofibromatosis. Pigmented neurofibroma is characterized histologically by the coexistence of scattered melanin-laden cells and benign spindle cells with neural differentiation. We report a case of pigmented neurofibroma in a 22-year-old female patient.

8.A Study on the Organ Transplantation According to the Law of Organs Transplantation.

PURPOSE: 2003, Advances in medical-surgical technologies, histo-compatibility and immunosuppressive drugs, such as FK-506, MMF, have caused a dramatic increase in the rates of successful organ transplantation (TP). As the waiting list of patients requiring TP grows, there is a subtle donation. What is worse, its increased issues of ethics, for instance, definition of brain death, consent of living donation, paid organ donation, decreased altruism, living will, justice of sharing organ. Therefore, This study was done to develop on legal, ethical and philosophical for whole procedure of organ transplantation which is a life-saving treatment improvement the quality of life. METHODS: A survey questionnaire was used and received responses from TP doctors(59) and nursing coordinators(46) at 53 hospitals where TP were being done for this study. The basic data in TP were collected, 1) to identity the staffs in charge of TP and their jobs in the hospital, 2) committee of hospital ethics (CHE), 3) committee of brain death confirmation definition (CBDCD), 4) to research what consideration are appropriate donors and recipients, 5) the facility of hospital for TP, 6) to analyzed awaiting solution the problems for TP. The data were analyzed using SAS-program by descriptive statistics, t-test, ANOVA and Pearson correlation coefficient. RESULTS: The jobs of TP nursing coordinators were not only to give nursing information but also to take care of donors and recipients (62.8~84.4%). The job of TP has not been identified between CHE and CBDCD (62.8%). The mean score of CHE's members was 6.33 persons. The mean scores of the problems regarding the law of TP was 2.88 (full score:4). The high items of the problems regarding the law of TP were "the decreased TP, donation and brain death because of KONOS (Korea Network for Orgen Sharing) which operated TP". The mean score of the improvements regarding the law of TP was 2.97 (full score:4). The highest item of the improvements regarding the law of TP was "Compensation of loss should be paid by the KONOS. The procedure of confirm is a loose legal. Therefore that is concern about commercialization of human organ. Definition of brain death is rigid regulation. Sharing organs are a fair dealing. The problems were significantly correlated with the improvements (P<0.0001). CONCLUSION: The finding of the study suggested that a TP should manage donors and recipients in line with legal, ethical, philosophical, humanistic aspects. Various viewpoints, the CHE should be enacted from the principle of human dignity, conducted by CBDCD of professional members and the nursing coordinators played an important role. It is suggested that TP might meet the request of the time so that KONOS should be changed with the other organization of newly methods and responsibility.

10.The Cardioprotective Effect of Ischemic Preconditioning : Role of Adenosine and Protein Kinase C.

BACKGROUND: Brief episodes of coronary blood flow interruption, ischemic preconditioning (IP), following a prolonged ischemia induces myocardial tolerance to ischemia and improves myocardial function during reperfusion by undefined mechanism. Recently, it has been suggested that the signal transduction pathway of the cardiomyocyte itself may involve in this protection. The aims of the present study were : (1) to examine the effect of adenosine in early phase of IP, (2) to define the relationship between the adenosine and protein kinase C(PKC) METHOD AND RESULTS: Heart isolated from New Zealand White rabbit (1.2 - 1.5kg body weight, n=78) were perfused with Tyrode solution by non-recirculating Langendorff technique. After stabilization of baseline hemodynamics, the hearts were subjected to receiving 45min global ischemia (I) and 120min reperfusion (R) with or without IP. IP was induced by a single dose of 5min I and 10min R. A part of the IP hearts, calphostin C (200nmol/L), a PKC inhibitor, was administered 5min before IP and sustained during IP regimen. Left ventricular function and coronary flow were monitored. Infarct size was determined by staining with 1% triphenyltetrazolium chloride solution and computerized planimetry. Adenosine concentration in the coronary flow was determined by HPLC. Myocardial cytosolic and membrane PKC activities were measured by (32)P-r-ATP incorporation into PKC specific peptide. Expression of PKC-e and PKC-o was determined by SDS-PAGE and Western blot. IP enhanced improvement of functional recovery (p<0.05, in the left ventricular developed and end-diastolic pressure ; p<0.01, in the coronary flow) during 120min R after 45min I. Preconditioned hearts showed reduction in the infarct size compared with the non-preconditioned hearts (p<0.05) ; however, IP-induced protection was lost by calphostin C. Adenosine release from the cardiomyocytes abruptly increased to 10-20 folds baseline just after IP manipulation and decreased rapidly on reperfusion. Cytosolic PKC activity significantly decreased in the preconidtioned hearts which received 45min I(p<0.05) and 45min I and 120min R(p<0.01), while the membrane fraction increased in the former(p<0.05) and the latter(p<0.01) groups. There was no significant difference in the PKC-o activity among all experimental groups in cytosolic and membrane fraction, however, the membrane PKC-e isoenzyme activity was increased in the preconditioned hearts which received 45min I. CONCLUSION: These results indicate that (1) a single dose of brief ischemia has an infarctlimiting effect and can improve post-ischemic contractile dysfunction after 45min subsequent sustained I ; and (2) increase of adenosine release in the earlier period of IP regimen and translocation of PKC from the cytosol to myocyte membrane may be important processes signal transduction for protection. These results suggest that cardioprotective mechanism responsible for IP in isolated rabbit heart may be initiated by adenosine and PKC.
