1.Cancer -Targeted MR Molecular Imaging.

Magnetic resonance (MR) imaging has been widely used in the clinic because of the benefit of high spatial and temporal resolution, and the excellent anatomical tissue contrast. Cancer-targeted MR molecular imaging comprises 3 major components: a relevant molecular target which is specifically highly expressed on the membrane of the cancer cell; a target specific imaging probe which is composed of superparamagnetic iron oxide nanoparticle coreconjugated target specific ligand such as antibody, peptide, and molecules; MR imaging hardware and software which are sensitive to the imaging probe. Among the various molecular targets, HER2/neu receptor antibody, folic acid, and arginine-glycine-aspartic acid (RGD) are well known targeting ligands. The sensitivity of the cancer-targeted MR imaging is affected by the magnetic susceptibility of the T2 contrast agent, resolution of the image, targeting efficiency of the imaging probe, and image acquisition pulse sequence. Recently, successful cancer-targeted MR imaging with T1 contrast agent and cancer-specific molecular MR imaging using innate contrast of the cancer cell by chemical exchange phenomenon without using the imaging probe has been introduced. Cancer-targeted MR molecuar imaging is a robust diagnostic method to detect cancer at the cellular stage of the cancer development and it would help improve early detection rate of the cancer.

2.PHYSICAL PROPERTIES OF THE REUSED IPS EMPRESS CERAMIC PART III. STUDY ON THE STAIN RESISTANCE.

The purpose of this study was to investigate the possibility on recycling of IPS Empress ceramic for the wide use of esthetically pleased IPS-Empress ceramic in clinical prosthodontic treatment. The specimens were made of first, second, and third pressed IPS Empress ceramic and were immersed in 0.05% methylene blue solution for 24hours. The amount of color change before and after stainning was measured with colorimeter(Model Tc-6Fx, Denshoku Co., Tokyo, Japan). The change of E in first pressed ceramic was 18.33 and those in second and third pressed ceramics were 19.33 and 19.93 respectively. Although there was statistical difference among them, the differences were minute and acceptable. From the results of this study, the possibility of recycling of IPS Empress ceramic could be suggested.

3.Fracture of the Femoral Component in Whiteside Ortholoc Modular Total Knee ArthroplastyL: 2 Cases Reports.

The breakage of component is rare in TKA and usually restricted to hinges and linked design. Occasionally fracture of metal tibial component has been noted, but fracture of the femoral component has been rarely reported and occurred to be a significaot problem in TKA. This study reports 2 cases of femoral component fracture using Whiteside Ortholoc total knee system and analyzes the cause of this phenomenon. One pahent, a 67 years old female who weighed 74kg, was performed 1ke left TKA using Whiteside Ortholoc g system with cement. The size of feraoral component was small plus, and small tibial component, 14mm tibial insert and 28mm pateBa were used, The medial site of femoeal component fractured at 38 months postoperatively. A second female patient, who weighed 68kg and was 64 years old at the time of surgery, had also the left TKA using a medium sized femoral component of Whiteside Ortholoc modular system with cement. The small-Ex large tibial component, 8rrun tibial insert were used, but did not replace the pateBa. Also at 37 mainths postaperatively the medial site of femoral compcnent fractured, Ail cases were revised with using a cemented P,F.C. Modular Total Knee System (Johnson k Johnson). All two cases were kactured at the junction between the posterior bevel md distal surface of the medial femoral condyle. The thickness and length of Whiteside Ortholoc femoral component are thinner and shorter at posterior bevel surface compared with other TKA systems. So it is encouraging that a minor design modification of Whiteside Ortholoe femoral component and also should be edueated for the prohibihon of excessive flexion of the knee joint after TKA.

4.The Length of postoperative antituberculous therapy in patients with pulmonary tuberculosis.

BACKGROUND: The length of postoperative drug therapy remains controversial in pulmonary tuberculosis. We analyzed our experiences to determine the postoperative duration of chemotherapy after resection. METHODS: A retrospective review was performed in 66 of 95 patients that underwent pulmonary resection for pulmonary tuberculosis between January 1993 and December 1998. We compared the relapse rates according to the length of postoperative chemotherapy in each group, classified by the results of sputum AFB culture before the surgery, the number of resistant durgs, the number of prior treatment and the division of anti-TB drugs used postoperatively. RESULTS: Fifty three of 66(80.3%) were men and 13(19.7%) were women with a median age of 33.5 years(range, 16 to 63). The mean lengths of the pre-and post-operative chemotherapies were 4.9 months, and 12.9 months respectively. Five of 66 patients(7.6%) relapsed during the mean period of follow up(39.7 months). In the group less than three times of the prior treatment, there were two relapses(20%) in Ed-the highlight above-rephrase 10 patients that were medicated for 6 months or less, and one relapse in 43 patients(2.3%) that took medicine for more than 6 months(p=0.03). In the group using second-line drugs postoperatively, there was one relapse(25%) in four patients that were medicated for 12 months or less. No patient in a total of 17 that received medicine for more than 12 months relapsed(p=0.03). CONCLUSION: We recommend that patients with the prior treatment less than three times should be treated for more than 6 months after resection and patients using the second-line drugs postoperatively should be medicated for more than 12 months.

6.THE SHEAR BOND STRENGTH OF RESIN TEETH TO THE DENTURE BASE RESIN.

The bond failure of resin teeth to denture base resin is one of the failure in prosthodontic treatment. The purpose of this study was to evaluate the bond strength of artificial resin teeth to the denture base resins. Specimens were made with heat curing acrylic resins (Vertex Rs, Lucitone 199) and artificial resin teeth (Tiger, Trubyte biotone, Endura, Orthosit, Trubyte bioform IPN) and the bond strength were measured with testing machine (Zwick, Germany) and the mode of bond failure were observed. The results were as follows; 1. The bond strength of Vertex Rs to artificial resin teeth was the highest in Trubyte biotone, and It was the lowest in Tiger and Trubyte bioform IPN 2. The bond strength of Lucitone 199 to artificial resin teeth were higher in Orthosit and Trubyte bioform IPN than in Tiger and Trubyte biotone. 3. The bond strength of Trubyte biotone to Vertex Rs was higher than to Lucitone 199 and that of the Trubyte bioform IPN to Lucitone 199 was higher than to Vertex Rs. 4. Nearly all bond failures of specimens occured cohesively within the resin teeth.

7.A Case of Cutaneous Sarcoidosis.

A case of cutaneous sarcoidosis in the 66 year-old Korean female patient is presented. She shows typical skin manifestations of plaque type in the absence of other common objective evidence of systemic involvement. Diagnosis was confirmed by charcteristic histological findings and positive Kveim test with the support of other compatible clinical and biochemical findings. She had been followed up for 3 years since the first visiting. Recently chest X-ray, slit lamp examination and skin biopsies were taken again, but failed to reveal any significant interval changes.

10.Quantitation of C-reactive Protein Levels and Erythrocyte Sedimentation Rate after Spinal Surgery.

OBJECTIVES: In order to standardize the CRP, ESR and WBC count after three types of spinal surgery and get clinical usefulness from these laboratory dadas after spinal surgery and clincal usefulness of laboratory datas. MATERIAL AND METHODS: Three groups of patients(microdisectomy, anterior and posterior fusion) were chosen for the study. CRP, ESR and WBC levels were prospectively determined before surgery and at days 1-5, 10, 14,21 and 42 after surgery. RESULTS: In all patients, preoperative normal CRP level(<10mg/L) increased, reaching peak levels on the second day after anterior fusion(84.6mg/L), and at the third day after microdiscectomy(54.5mg/L) and posterolateral intercorporal fusion(152.2mg/L), with normalization in 5-10 days. Preoperative normal ESR level increased to peak level on the forth day after microdiscectomy(33.0mm/hr), and increased double peak levels after anterior(postoperative days 2&5) and posterolateral intercorporal fusion(postoperative days 3&5). ESR followed by a slow and irregular decrease, and at 42 days after surgery often remained elevated. WBC increased to variable peak level within normal limit in all patients. CONCLUSIONS: The rapid decline in CRP will probably be interrupted by a second rise or persisting elevation if infection occurs. CRP is presumably a better test than ESR for early detection of postoperative infection.
