3.Percutaneous transluminal angioplsty in arteriosclerosis

Percutaneous transluminal angioplasty(PTA) is a non-operative therapeutic procedure to the dilation ofstenosis or to the recanalization of occlusion in artherosclesrotic atherosclerotic arteries using of dilatationcatheters. PTA was performed 37 times in 34 patients with arteriosclerosis for 1 year and 4 months from March.,1982 to June, 1983 at department of radiology, Klinikum Barmen/west Germany. The results were as follows; 1. Themale to female ratio was 2:1 and peak age range was from 61 to 70. 2. The most common indication was clinicalstage II b with 19 cases(51.3%) 3. PTA was performed most commonly in superficial femoral artery in 25cases(67.5%). 4. Acute major complication occurred in 2 cases(5.4%). PTA is a alternative or complementarytherapeutic procedure to vascular surgery.

4.Influence of Endothelin-1 on Cultured Vascular Smooth Muscle Cell Proliferation.

BACKGROUND: Proliferation of vascular smooth muscle cells(VSMC) is a critical event in the development of atherosclerosis. Endothelin-1(ET-1), a vasoconstrictor peptide produced by endothelial cells and VSMC, might play a role in vascular remodeling. To investigate the proposed 'mitogenic' potential of ET-1, we examined the effects of ET-1 on the proliferation if cultured porcine aortic VSMC and on the potential synergism with platelet-derived growth factor(PDGF). MATERIALS AND METHODS: VSMC were obtained from porcine aorta and cultured in Dulbecco's modified Eagle medium supplmented with 10% fetal bovine serum(FBS). VSMC grown subconfluently in 12-well plate were stimulated by ET-1, PDGF, and ET-1 & PDGF and DNA synthesis was determined as the uptake of 3H-thymidine into cell cultures. We also examined the effects of BQ123, a selective ETA receptor antagonist, and NG-methyl-L-arginine(NMLA), a nitric oxide synthase (NOS) inhibitor. RESULTS: ET-1 elicited a 2.5-fold increase of cultured VSMC DNA synthesis, comparing with basal medium, and PDGF elicited a 4.8-fold increase, whereas ET-1 and PDGF elicited a 8.8-fold increase, showing synergistic effect. Proliferative activity of ET-1 on VSMC was blocked(39%) by BQ123, however, the synergistic effect of ET-1 and PDGF was not blocked by BQ123. The synergistic effect of ET-1 and PDGF was increased when co-stimulated with NMLA. CONCLUSION: ET-1 is a co-mitogen for VSMC from porcine aorta, whose proliferative activity requires serum or other growth factors such as PDGF for its maximal activity. The proliferative activity of ET-1 is considered to be transduced partly by selective activation of the ETA receptor, however, the synergistic effect of of ET-1 and PDGF is to be stimulated by non-ETA receptor.

5.A Study of the noise level in hospital and the Count-Measure against the noise.

In this study noise source in a ward at four general hospitals in Seoul area has investigated and analysed. The degree of reaction against noise by 171 randomly has also been examined. The results of the study have shown that the source of noise is the speakers of wired broadcasting or from visiting guests in two hospitals located in residential area. The patients at the two other hospitals located at commercial site have been suffered more from traffic noise, However, because of their separated living at hospital from their ordinary houselife, sixty on percent of the inpatients have wished a music sound of around 60 dB (A). After having considered the results of the investigation and wishes of the inpatients, following suggestions have been made: 1. Reduce the number of guests or their length of stay. 2. Wired bodadcasting system should be substitued by sireless one, or if it's unavoidable, it should be used in office rooms only. 3. Since the stops and starts of vehicles induce much noise, Seoul City Government be requested to prepare an appropriate administrative measure for the vehicles around hospital area and ti should prevent the establishment of new hospitals along high way site. 4. By using earphone, inpatients can choose a wireless channel according to each individual's taste. This through the masking effect, would cover up the noise source. 5. Rooms along the streets should be utilized as offices, otherwise double windows should be set up for inpatient's wards.

6.Rapid Detection Test for Streptococcus pyogenes in Normal Carriers of Group A Beta-Hemolytic Streptococci.

Streptococcus pyogenes has been regarded very importantly because of its severe sequelae, namely rheumatic fever and acute glomerulonephropathy. Rapid tests to identify this organism have been studied widely in developed countries in 1980s. We proceeded the rapid test (A Strept ADTM, Denka Seiken Co, Japan) of S. pyogenes on 141 children who were Known to be carriers of this organism to evaluate whether the rapid test could be used at the pediatric office base. Throat cultures were taken simultaneously to evaluate the efficacy of this rapid test. The test shoewed very low sensitivity of 17% (13/14) and high specificity of 99% (65/66). The positive predictive value was.93% (13/14) and negative predictive value 51% (65/127) respectively. Most of the positive results of rapid test have been agreed with the actual number of colonies on the throat culture. The reasons for the low sensitivity and negative predictive value of the rapid test should be discovered and supplemented if it could be applicable to the pediatric office and accurate evaluation of the rapid test in patients with acute pharyngitis is needed.

7.Clinical study of Traumatic Fracture-Dislocation of the Hip

Traumatic fracture-dislocation of the hip is a mutilating injury which often comprises multiplicity of complications and requires well planned therapeutic procedures. In this paper 36 cases of fracture-dislocation of the hips treated at the Department of Orthopedic Surgery, The National Medical Center, during the years from 1959 to 1971 is reported. 1. The incidence of the injury is remarkably higher in the male than in the female as 29 versus 7, and it was higher in the age group of 20-30 years. It is of interest the incidence is equally distributed in age group of 10 to 60 years in this series. 2. The mode of the injury was considerably higher in falling type of trauma as 16 cases out of 36 cases, and the incidence of posterior dislocation was higher than other type of disIoatons, 22 cases out of 36 cases. Grade III type of the injury had higher incidence than other grades of the injuries, 17 cases out of 36 cases. 3. 14 cases out of 36 cases had complicated injuries in other part of the bodies, and among them 18 was complicated fractures. Sciatic nerve paralysis and head injuries were complicted in four respectively. 4. Seven out of eight cases treated by conservative treatment had good results, and seven out of 12 cases treated by primary acetabuIosynthesis had good results. As a whole the conservative treatment had better result than that of the surgicaI treatment, this seems to be derived from the fact that the surgically treated group had higher proportion of higher grade of injuries than in the group of the conservative treatment. 5. The higher grade of the injury the poorer the results of the treatment. The post-operative and post-therapeutic complications mostly occurred among the cases, who had delayed treatment for more than two monsths after the injury had been sustained. Aseptic necrosis in the femoral head occured in four cases and reccurrent dislocation occurred in three cases in this series.

8.Pattern of Distant Lymph Node Metastasis in Colorectal Carcinoma and its Correlation with Distant Organ Metastasis: CT Evaluation.

PURPOSE: To evaluate the pattern of distant lymph node metastasis in colorectal carcinoma and its correlation with distant organ metastasis. MATERIALS AND METHODS: We retrospectively reviewed abdominal CT scans of 46 patients with pathologically proven colorectal carcinoma. RESULTS: The incidence of distant lymphadenopathy in colorectal carcinoma was 30.4%(14/46). The most commonly involved distant lymph node was the left paraortic lymph node below the renal hilum(9/25). The most common type of distant lymphadenopathy was solitary type(7/14) and all of these lymphadenopathies were noted in the left paraortic lymph node below the renal hilum. Six cases of left sided colorectal carcinoma showed left paraortic lymphadenopathy with solitary type. The incidence of distant organ metastasis was 17. 4%(8/46) and markedly increased if distant lymphadenopathy was multiple and confluent, or confluent type(5/7). CONCLUSION: The incidence of distant lymphadenopathy in colorectal carcinoma was not high and the most common lymphadenopathy was the left paraaortic lymph node with solitary type below the renal hilum. The possibility of distant organ metastasis was high if distant lymphadenopathy was multiple and confluent, or confluent type.

10.Accessory Hepatic Vein: MR Imaging.

PURPOSE: To evaluate the MR appearance of the accessory hepatic veins. MATERIALS AND METHODS: The study included 87 consecutive patients for whom abdominal MR images were obtained. The subjects who had liver lesion or hepatic vascular abnormalities were excluded. Couinaud classified accessory hepatic veins into inferior and middle right hepatic veins. Our major interests were evaluation of the incidence, morphology, and location of the accessory hepatic vein. RESULTS: Inferior right hepatic vein was demonstrated in 43 out of 87 patients(49%). The morphology was linear in 35 patients(80.5%), and V-shaped in 8 patients(19.5%). In 40 patients(93%), the inferior right hepatic vein was located in the posteroinferior aspect of the right lobe. Middle right hepatic vein was demonstrated in 7 out of 87 patients(8%). All were single linear in morphology, combined with the inferior right hepatic vein, and located between the right hepatic vein and inferior right hepatic vein. CONCLUSION: The accessory hepatic vein was demonstrated in 49% among the Korean adult population, and was located in posteroinferior portion of the liver, in 93%.
