1.A Case of Kimura's Disease with an Unusual Location and Clinical Manifestation.

Kimura's disease is a chronic inflammatory condition of unknown cause. It usually appears as a painless nodule in the head and neck region often associated with regional lymphadenopathy, but there have rarely been reports of penile involvement or clinical presentation as an ulcerative plaque. We present a case of Kimura's disease occurring on the penile shaft as an ulcerative plaque in a 44-year-old man. A vesicular eruption had occurred on his penile shaft followed by an ulcerative plaque for 2 months. He had suffered from recurrent regional lymphadenopathy for 15 years and he had also exhibited intermittent eosinophilia. Histopathologic findings revealed diffuse eosinophilic infiltration in the dermis of the skin and eosmophilic abscesses in the lymphoid follicles of the mguinal lymph node. After short-term systemic steroid therapy and local injection of triamcinolone acetonide, the ulcerative plaque was healed.

2.Characteristics and osteogenic effect of zirconia porous scaffold coated with beta-TCP/HA.

PURPOSE: The purpose of this study was to evaluate the properties of a porous zirconia scaffold coated with bioactive materials and compare the in vitro cellular behavior of MC3T3-E1 preosteoblastic cells to titanium and zirconia disks and porous zirconia scaffolds. MATERIALS AND METHODS: Titanium and zirconia disks were prepared. A porous zirconia scaffold was fabricated with an open cell polyurethane disk foam template. The porous zirconia scaffolds were coated with beta-TCP, HA and a compound of beta-TCP and HA (BCP). The characteristics of the specimens were evaluated using scanning electron microscopy (SEM), energy dispersive x-ray spectrometer (EDX), and x-ray diffractometry (XRD). The dissolution tests were analyzed by an inductively coupled plasma spectrometer (ICP). The osteogenic effect of MC3T3-E1 cells was assessed via cell counting and reverse transcriptase-polymerase chain reaction (RT-PCR). RESULTS: The EDX profiles showed the substrate of zirconia, which was surrounded by the Ca-P layer. In the dissolution test, dissolved Ca2+ ions were observed in the following decreasing order; beta-TCP > BCP > HA (P<.05). In the cellular experiments, the cell proliferation on titanium disks appeared significantly lower in comparison to the other groups after 5 days (P<.05). The zirconia scaffolds had greater values than the zirconia disks (P<.05). The mRNA level of osteocalcin was highest on the non-coated zirconia scaffolds after 7 days. CONCLUSION: Zirconia had greater osteoblast cell activity than titanium. The interconnecting pores of the zirconia scaffolds showed enhanced proliferation and cell differentiation. The activity of osteoblast was more affected by microstructure than by coating materials.

3.Prognostic Factors in Patients with Hypertensive Basal Ganglionic - Thalamic Intracerebral Hemorrhage.

The authors carried out various treatment modalities in 74 consecutive patients with hypertensive basal ganglionic-thalamic intracerebral hemorrhage and were admitted to the Department of Neurosurgery. Konkuk University Hospital, from Jan. 1991 to Dec. 1993. A variety of prognostic factors that influence mortality were observed. The locaton of hematoma was at the basal ganglia in 47 cases and at the thalamus in 27 cases. The prognosis gets poorer as the hematoma extended wider and deeper. The prognosis was unfavorable when the hematoma was over 30cc(P<0.001). The mortality rate was higher in cases with IVH than in cases without IVH(P<0.005). In cases with IVH, 19 cases(26%) showed dilated 4th ventricular hemorrage and higher mortality rate(P<0.001). Cases in which the GCS were less than 9 on admission showed higher mortality rate(P<0.0001). The mortality rate was also higher if the midline shift was more than 10mm on the initial brain CT scan(P<0.005). THe group where the unilateral or bilateral pupillary light reflex was unreactive(35cases) showed poorer prognosis than the group where the bilateral pupillary light reflex was reactive(P<0.0001). The ventriculocranial ratio(VCR), hydrocephalus, surrounding edema edema around the heamtoam, and treatment modality were not related to the prognosis. The significant prognostic factors in patients with hypertensive basal ganglionic-thalamic intracerebral hemorrhage were location and type of hematoma, whether or not the volume of hematoma is more than 30cc, IVH, dilated 4th ventricular hemorrhage, Graeb's score of more than 7, GCS of less than 9, midline shift of more than 10mm, and reactivity of pupillary light reflex.

4.A Case of Thoracic Meningomyelocele Containing Accessory Cord.

The authors report a case of thoracic meningomyelocele. The patient had not have any presenting symptoms until adult age. We confirmed it with radiological(postmyelography CT and MRI) and pathological diagnosis. Pre-, post- and intraoperative SSEP(somatosensory evoked potential) monitoring were performed. Characteristically, the mass contained accessory cord which attached to inner cyst wall with multiple fibrous bands. Meningomyelocele was completely removed without any neurologic sequelae.

5.A Case of Clear Cell Acanthoma.

We report a case of clear cell acanthoma occurred in a 61 year old man. He developed a brownish bean sized nodule on the left chest 3 years ago. The skin lesion has slowly increased in size without any subjective symptoms. Histopathologic examination revealed proliferation of clear cells within a sharply demarcated area of the epidermis. The presence of numerous neutrophils throughout the epidermis were also noted.

6.The Supine and Prone Position for Computed Tomographic Myelography(CTM) of the Lumbar Spine: Change of Gantry Angle.

For evaluating the lumbar disc space, the fact that the postmyelography computed tomography was more accurate than computed tomography or myelography is well-known. But the technical difficulty is limitation of gantry angle. The prone position scanning may help to lessen this trouble. A prospective computed tomographic scanning was performed in both the prone and supine position on 43 patients, who had low back pain. The angle between the vertebral end plate and vertical line was measured in both position at L3-4, L4-5 and L5-S1 interspaces. The advantages of prone scanning in spinal aligment was noted at L5-S1 disc space(79.1%) and also more desirable if computed tomography was performed with metrizamide. With above advantages, the evaluation of bulging annulus was became more easier.

7.The marginal fidelity of Procera(R) AllCeram alumina copings and crowns of patients.

STATEMENT OF PROBLEM: Use of all-ceramic prostheses fabricated with CAD/CAM systems is increasing in the dentistry. Marginal fidelity in production of all-ceramic restoration has important clinical implications and is a key consideration issue in CAD/CAM production as well. PURPOSE: The objective of this study was to analyse marginal fidelities of Procera(R) Allceram Crown. MATERIAL AND METHODS: On 56 patients treated with Procera(R) system Allceram Crown at Dankook Dental Hospital, marginal discrepancies of 101 abutments were measured by stereomicroscope at coping and final restoration stages. Paired t-test and one-way analysis of variance on marginal discrepancy data were conducted to determine the presence of significant differences between measurement and measuring point stages. RESULTS: Marginal discrepancies of final restoration (45.82 +/- 30.84 micrometer) were lower than alumina coping (53.84 +/- 38.83 micrometer). Furthermore, the differences were found to be statistically significant at 95% confidence level. Anterior marginal discrepancies were lower than posterior marginal discrepancies, but they were not statistically significant. Lingual marginal discrepancies were higher than other measurement sites, and the differences were found to be statistically significant at 95% confidence level. CONCLUSION: Within the conditions of this study, marginal fidelities of Procera(R) Allceram Crown were acceptable, and after porcelain build-up, marginal fitness improved over alumina coping. More careful scanning is needed for better results.

8.Oculomoter Nerve Palsy Due to Arachnoid Cyst in the Newborn Infant.

A 22 days old newborn infant with left total ophthalmoplegia involving manifestations of palpebral ptosis was examined and treated. An arachnoid cyst located in the temporo-parietal lobe as the very rare incidence of etiological lesion was detected on brain C/T. Following the successful cystoperitoneal shunt, the ophthalmoplegia improved clinically.

9.A study on the effective cleaning of healing abutment using healing abutment case

Purpose:
The present study aimed to investigate the effective cleaning of healing abutment (HA) using Healing abutment case (HA case) by observing oral microorganisms with phase contrast microscope.
Materials and methods:
32 patients with two or more implants placed in the same jaw, a total of 64 HAs (experimental group 32, control group 32) were selected and the control was cleaned with an alcohol swab. At the first and second visits, each group was observed before cleaning, and the experimental group was additionally observed after cleaning at the first visit. A 400× phase contrast microscope was used for the observation of oral microorganisms for its amounts.
Results:
There was no significant difference in the amount of oral microorganisms was found between the groups at the first visit, no significant difference according to gender, maxilla or mandible, and buccal or lingual surface. There was a statistically significant difference in the amount of oral microorganisms according to supra-gingival and sub-gingival (P<.05), There was also a significant difference in the comparison before and after cleaning in the experimental group (P<.05). There was a significant difference in the amount of oral microorganisms in each group at second visit (P<.05).
Conclusion
Healing abutment cleaning using healing abutment case solution is more effective than simple cleaning with alcohol swab.
10.Bleeding Volume after Surgery for Trochanteric Fractures of the Femur in Patients Treated with Antiplatelet Agents: Comparison according to Surgical Timing.

PURPOSE: We evaluated the bleeding volume after surgery for trochanteric fractures of the femur in patients treated with antiplatelet agents according to surgical timing. MATERIALS AND METHODS: We selected 20 patients who had trochanteric fractures of the femur treated with antiplatelet agents from January 2009 to June 2010. Group I included 9 patients who discontinued antiplatelet medication and had delayed operations at an average of 6.5 days and Group II included 11 patients who underwent early operations within 24 hours. Group I included 2 males and 7 females; their average age was 77.8 years (range 59~86). Group II included 4 males and 7 females, with an average age of 73.5 years (range 61~84). We compared the two groups' volume of intraoperative bleeding, the preoperative and postoperative hemoglobin levels and the volume of postoperative transfusion. The Mann-Whitney U test was used for statistical analysis. RESULTS: The volume of intraoperative bleeding was 88 ml in group I and 106 ml in group II (p>0.01). The difference in the hemoglobin was a decrease of 2.4 mg% in group I and a decrease of 2.2 mg% in group II (p>0.01). The volume of postoperative transfusion was 0.6 pints in group I and 1 pint in group II (p>0.01). CONCLUSION: We found a similar bleeding volume regardless of operative timing after surgery for trochanteric fractures of the femur in patients treated with antiplatelet agents.
