Malignant fibrous histiocytoma, which is interesting because of its pleomorphic nature, is a rare soft tissue sarcoma possibly arising from histiocyte. The tumors are usually situated in the skin or subcutaneous tissues, especially of the extremities, but they may also involve deeper structures, such as the retroperitoneum or genital organs. Herin we report a case of malignant fibrous histiocytoma in the retroperitoneal space and discuss the pathologic and clinical features.

PURPOSE: To report a case of malignant fibrous histiocytoma of the lateral canthal area that was diagnosed clinically as a benign epidermal inclusion cyst. CASE SUMMARY: A 49 year-old man presented with a cutaneous mass involving the lateral canthal area. A 0.7-cm cystic lesion was freely movable and non-inflamed. After excisional biopsy, the results of immunohistochemical staining led to a final diagnosis of low-grade malignant fibrous histiocytoma. The specimen from excisional biopsy had margins positive for malignancy; the patient underwent a second procedure for complete resection, and that specimen had tumor-free margins. CONCLUSIONS: Fibrous histiocytoma is a rare disease but may occur in the eyelid. Therefore, when an eyelid tumor progresses rapidly, malignant fibrous histiocytoma should be considered as a diagnosis.

Fibrous histiocytomas are complex group of tumors that feature cells resembling fibroblasts and histiocytes and that exhibit a cellular arrangement referred to as a storiform or cartwheel pattern. A 36 year old man was referred to our hospital with 2 months history of protruding mass in his right eye. Mass excision was performed and histopathologic examination including immunohistochemical stainning revealed malignant fibrous histiocytoma and he received radiation therapy.

A malignant fibrous histiocytoma is a rare pleomorphic tumor of the soft tissues. Primary retroperitoneal involvement of a malignant fibrous histiocytoma is very rare and often diagnosed in an advanced stage as it has no specific symptoms. Herein, a case of a malignant fibrous histiocytoma involving the retroperitoneum is reported, which was incidentally found in a 74-year-old male, who complained of pain in the right abdomen and thigh.
