We investigated the training system of a young cardiovascular surgeon in Japan. We presented the result of surveillance at the 49th Annual Meeting of the Japanese Society for Cardiovascular Surgery 2019, and report here the summary.

The work style of doctors gets attention within the Work Style Reforms that have been fully implemented since 2019. Now, we conducted a questionnaire survey at 10 institutions in Chugoku and Shikoku region and reviewed the latest work style of cardiovascular surgeons in comparison with other departments.
3.Need for peri-operative weight loss among obese colorectal cancer patients

Colorectal cancer patients account for the largest proportion of total cancer patients in Japan. With an increase in its surgical cases and relatively higher five-year survival rate, the number of cancer survivors is expected to be increasing. Therefore, primary prevention, surgical therapy and recurrence prevention for colorectal cancer are public health priorities in Japan. General and abdominal obesity (Not only abdominal but also general obesity) are risk factors for colorectal cancer. In addition, accumulated abdominal fat can extend time spent in surgery by hindering operative procedures and surgical field expansion. These factors raise the risk of postoperative complications such as bleeding and surgical site infection, which eventually leads to increased medical expenses. Physical activity is one of protective factors for colorectal cancer. Recent studies showed that exercise intervention in cancer survivors reduces the cancer recurrence risk. Furthermore, it has been suggested that exercise interventions are effective in reducing fatigue, improving immune function, and maintaining physical function. However, the effectiveness of preoperative exercise intervention aimed at reducing weight in colorectal cancer patients remains unknown. In this review article, we discuss necessity for peri-operative weight loss among obese colorectal cancer patients.
4.Applications of Orento Based on“Upper Heat and Middle Cold”

Orento is generally used for upper gastrointestinal symptoms. We have used orento for various diseases based on “upper heat and middle cold”. As a result of the evaluation of 30 responders to orento, the most frequent sign of “upper heat” was “yellow tongue coating” (29/30 responders), while the most frequent sign of “middle cold” was “preference for warm foods and beverages” (27/30 responders). Compared with the non-responder group, the responder group showed significantly higher frequencies of “gastrointestinal cold”, which is the sum of “preference for warm foods and beverages” and “aggravation of gastrointestinal symptoms by cold foods and beverages” (29/30 responders, p = 0.047), and “upper/lower gastrointestinal symptoms” (29/30 responders, p = 0.014). These results suggest that “yellow tongue coating” is the most important sign for “upper heat” while “gastrointestinal cold” is the most important sign for “middle cold”. As Kampo patterns for orento, gastrointestinal symptoms often occur, but there is a possibility of upper or lower gastrointestinal symptoms, skin disorders, and sleep disorders.
