1.Late Mortality after Reconstructive Surgical Treatment of Atherosclerotic Occlusive Disease.

We reviewed the clinical course of 127 patients who underwent treatment for atherosclerotic disease between June 1993 and January 2001. There were 108 men and 19 women. The ages ranged from 49 to 88 years with a median age of 71.2 at the time of the first operation. Major risk factors included ischemic heart disease (21%) and diabetes mellitus (20%). Ninety-five percent of the patients were followed successfully and the follow-up period ranged from 0 to 90 months with a mean of 33 months. Two patients died perioperatively due to myocardial infarction. There were 29 late deaths. The overall actuarial survival rate was 69.7% at 5 years. The 5-year actuarial survival rate and the mean survival time for men and women were 71.6%, 66.1 months and 62.3%, 58.9 months. The 5-year late survival rate and the mean survival time for patients with and without ischemic heart disease were 57.0%, 57.4 months and 74.2%, 68.5 months. The differences were not statistically significant. The 5-year late survival rate and the mean survival time for patients with and without diabetes mellitus were 65.5%, 59.1 months and 70.9%, 67.4 months. The differences were not statistically significant. Amputation was performed in 7 patients, the actuarial survival rate at 1 year and the mean survival time were 42.9%, 7.1 months for patients with amputation, and 93.0%, 69.5 months without amputation (p<0.01).
2.Feasibility of metronomic chemotherapy with tegafur-uracil, cisplatin, and dexamethasone for docetaxel-refractory prostate cancer

Objectives: To evaluate the efficacy of tegafur–uracil (UFT), a prodrug of 5-fluorouracil, plus cisplatin and dexamethasone in patients with docetaxel-refractory prostate cancers.
Methods: Twenty-five patients with docetaxel-refractory prostate cancer were administered oral UFT plus intravenous cisplatin (UFT-P therapy) and dexamethasone. Treatment responses were assessed monthly via prostate-specific antigen (PSA) level measurements. Treatment-related adverse events and overall survival were also assessed.
Results: UFT-P therapy resulted in decreased PSA levels in 14 (56%) patients and increased PSA levels in 11 (44%). In patients with increased PSA levels, 7 (64%) of the 11 patients displayed decreased PSA doubling times. The UFT-P therapy response rate was 84% (21/25 patients). Imaging studies revealed that tumor shrinkage during UFT-P therapy occurred in 1 patient in whom bilateral hydronephrosis caused by lymph node metastasis improved. The median survival time from docetaxel initiation was 36 months. In UFT-P-treated patients, the median PSA progression and overall survival times were 6 and 14 months, respectively. UFT-P treatment-related adverse events were mild diarrhea, general fatigue, and anorexia. Treatment was not discontinued for any of the patients. UFT-P therapy did not cause serious hepatic or renal dysfunction or pancytopenia.
Conclusions: UFT-P therapy is a safe and effective treatment for patients with docetaxel-refractory prostate cancer, although large-scale, multicenter, prospective studies are needed to validate these findings.
3.Change of Gastric Emptying With Chewing Gum: Evaluation Using a Continuous Real-Time 13C Breath Test (BreathID System).

BACKGROUND/AIMS: There are few reports on the correlation between chewing gum and the gastrointestinal functions. But previous report showed use of chewing gum to be an effective method for controlling gastrointestinal symptoms. The aim of this study was to determine the correlation between chewing gum and gastric emptying using the continuous real time 13C breath test (BreathID system). METHODS: Ten healthy male volunteers participated in this randomized, 2-way crossover study. The subjects fasted overnight and were randomly assigned to chewing gum (Xylish, 2-3/1 tablet) for an hour following intake of a test meal (200 kcal/200 mL) or intake of the test meal alone. Gastric emptying was monitored for 4 hours after administration of the test meal by the 13C-acetic acid breath test performed continually using the BreathID system. RESULTS: No significant differences in the calculated parameters, namely, T1/2 (median, 111.82 vs 109.26 minutes; P = 0.575), Tlag (median, 53.28 vs 56.53 minutes; P = 0.333), gastric emptying coefficient (median, 3.58 vs 3.65; P = 0.285), regression-estimated constant beta (median, 1.85 vs 1.80; P = 0.575) and regression-estimated constant kappa (median, 0.61 vs 0.62; P = 0.959) were observed between the test meal alone group and the test meal and chewing gum group. CONCLUSIONS: This study showed that chewing gum had no effect on the rate of gastric emptying. Therefore, since chewing gum did not enhance the speed of gastric emptying, it may ameliorate gastrointestinal symptoms through other mechanisms, such as saliva and autonomic nervous system.

4.An Adult Surgical Case of Anomalous Origin of the Right Coronary Artery from the Pulmonary Artery

Anomalous origin of the coronary artery from the pulmonary artery (ACAPA) is a rare congenital heart disease. A woman in her 60s was diagnosed as ARCAPA during examination for angina. A magnetic resonance image showed myocardial ischemia in a small area of the right coronary artery (RCA) and left anterior descending artery. Reimplantation of the RCA to the ascending aorta and patch plasty of the pulmonary artery were performed. Repositioning of the RCA orientation was necessary for relief of the kinking. The postoperative course was uneventful. She is now free from angina 15 months after surgery.
5.Five Cases of Breast Cancer-Related Therapy Complications Successfully Treated with Kampo Medicine

Five cases of various diseases associated with breast cancer treatment including arthralgia, menopausal symptoms, chemotherapy-associated symptoms (nausea, vomiting, numbness, pain, general fatigue), and anxiety neuroses during follow-up were treated with Kampo medicine and a favorable course was obtained. Combined use of Kampo treatment can reduce the suffering caused by the side effects of breast cancer treatment, alleviate mental symptoms, prevent recurrence, and improve the patients' health. Kampo medicine plays a role in cancer supportive care for breast cancer.
6.Cigarette Beetles (Lasioderma Serricorne) Occurring in Plantago Seed

In the pharmaceutical department of Iizuka Hospital, larvae of the cigarette beetle (Lasioderma serricorne) were found in Plantago seed for the first time since the Department of Japanese Kampo Medicine started in 1992. The larvae grew into pupae and then adults. The plastic-packaged Plantago seed was left for about 3 months, during which several larvae and adult cigarette beetles appeared. Cigarette beetles are very common insects in summer and it is important that we store crude drugs in hard closed containers in our refrigerators. The larvae can bite strongly and puncture plastic packaging, however they cannot live under 15 ℃. Education of crude drug storage for patients is very important.
7.Ligation of Lumbar Arteries and Stent Graft-Conserving Aneurysmorrhaphy for Type II Endoleak

Background : Persistent endoleak is a major cause of aneurysmal enlargement or rupture after endovascular aneurysm repair (EVAR). Although several reports have described ligation of lumbar arteries and stent graft-conserving aneurysmorrhaphy as useful strategies, treatment for type II endoleak after EVAR is controversial. Objectives : We investigated the early results in 5 patients who underwent ligation of lumbar arteries and stent graft-conserving aneurysmorrhaphy for type II endoleak. Methods : A>10 mm increase in aneurysm diameter after primary EVAR or a maximum diameter>65 mm serve as indications for intervention for type II endoleak. Under general anesthesia, following transperitoneal exposure of the abdominal aorta, the infrarenal aorta was banded using a tape at the proximal landing zone. After the aorta was opened without clamping, the lumbar arteries were ligated, and a stent graft-conserving aneurysmorrhaphy was performed. Results : The mean interval from the primary EVAR was 47±17 months. The mean operation time was 215±76 min. Blood transfusion was necessary in 4 patients (estimated blood loss 1,260±710 ml). No in-hospital deaths were observed, and the mean postoperative hospital stay was 26±20 days. One patient developed aspiration pneumonia and 1 developed surgical site infection post-surgery. The diameter of the aneurysm changed from 68±8 to 47±5 mm during hospitalization and decreased further to 36±7 mm at the last follow-up. Conclusions : The early results of ligation of lumbar arteries and stent graft-conserving aneurysmorrhaphy were favorable. Although this strategy could be useful for aneurysmal dilatation secondary to persistent type II endoleak after EVAR, the indications for this approach should be determined following careful evaluation of the patient's status considering the invasiveness of the procedure.
