5.Intratumoral Fat in Neurofibroma and Coexistence of Eccrine Hidrocystoma.

We report a case of intratumoral fat in neurofibroma and coexistance of eccrine hidrocystoma on the occipital area of the scalp for one year duration. There are several diseases showing fatty change histopathologically. Among them, few cases of neurofibroma showing fatty change had been reported. The mechanism of fatty change is unclear, but several hypotheses are proposed. Here we report a case of neurofibroma showing fatty change and coexistence of eccrine hidrocystoma, and discuss the pathomechanism of fatty change and its relationship with disease.

7.Two Csaes of Multiple Eccrine Hidrocystoma.

Eccrine hidrocystoma is a benign eccrine duct-derived cyst which may be solitary or multiple. We report two cases of multiple eccrine hidrocystoma, which have two points of sameness in middle aged women and worsening of the condition in summer and improvement in winter. Recently, the incidence of multiple eccrine hidrocystoma seems to be decreasing due to well-developed cooling devices in summer. In spite of its decreasing incidence, we think that it is still noteworthy to keep in mind the possibility of eccrine hidrocystoma when a patient presented with multiple facial papules in hot season.

8.Eccrine hidrocystoma of the Nasal Tip.

Eccrine hidrocystoma is a cyst origined from the secretory portion of the eccrine sweat gland and tends to occur as a solitary facial lesion. Clinically, an eccrine hidrocystoma presents as a small, translucent and cystic swelling(often having a bluish hue) which measures 1 to 3 mm in diameter. In some patients, the mass is appeared with numerous lesions. The mass has the tendency that the number of cyst increases in warm weather and decreases during winter. The histologic finding is an intradermal cyst with a thin wall composed of two layers of eosinophilic- staining cuboidal cells. Eccrine hidrocystoma has not yet reported in the journal of korean society of plastic and reconstructive surgeons. Thus, we report a case of 65-years-old female patient with eccrine hidrocystoma, which presents 2.1 x 1.6 x 1.6 cm in size and bluish hue color, occurring on the nasal tip.

9.The Utility of HMFG-1 and GCDFP-15 to Discriminate the Differentiation of Eccrine and Apocrine Neoplasms.

The discrimination of eccrine and apocrine neoplasms is still in dispute among researchers. To discriminate the differentiation of eccrine and apocrine neoplasms, immunohistochemical stainings with HMFG-1 and GCDFP-15 were performed on 12 specimens of eccrine and apocrine neoplasms, 3 of normal palmar tissues, and 3 of normal axillary tissues. The 12 cases of eccrine and apocrine neoplasms included 3 cases of syringoma, 2 cases of eccrine poroma, 2 cases of hidradenoma, 2 cases of apocrine hidrocystoma, 2 cases of syringocystadenoma papilliferum, and 1 case of spiradenoma. The 3 specimens of palmar tissue were used as the positive control of eccrine unit, and the 3 specimens of axillary tissue as that of apocrine unit. The results are as follows; HMFG-1 was positive in the cytoplasm and luminal surface of normal apocrine units, and also positive in those of eccrine units. Still more, HMFG-1 was documented as positive even in the eccrine poroma which is originated from eccrine unit. GCDFP-1 was positive in both normal eccrine and apocrine units, and glandular structure was stained stronger than ductal structure. In all of eccrine and apocrine neoplasms, syringoma, eccrine poroma, and spiradenoma which were known to differentiate to ductal structure were negative in GCDFP-15, but hidradenoma, apocrine hidrocystoma, and syringocystadenoma papilliferum which were known to differentiate to glandular structure were positive, partially. All these findings suggest that GCDFP-15 might be a useful marker for glandular differentiation rather than ductal differentiation, and both HMFG-1 and GCDFP-15 might not be useful to discriminate eccrine and apocrine neoplasms. Further intensive studies about normal eccrine and apocrine unit should be performed to determine if HMFG-1 could be used as a significant marker of apocrine unit, and it is necessary to develop new methods for the discrimination of eccrine and apocrine neoplasms.

10.A case of apocrine hidrocystoma.

A 26 year old female presented complaining of a pea-sized nodule on her left lower eyelid for 10 years duration. Histopathological study showed several cysts in the dermis surrounded by secretory cells. The cysts were filled with decapitatory secretions. Positive staining with carcinoembrynic an tigen(CEA) was detected by immunoperoxidase technique. These clinical and histopathological findings suggest apocrine hidrocystoma developed on the left lower eyelid.
