1.EXERCISE DOES NOT INCREASE n-3 POLYUNSATURATED FATTY ACIDS (PUFA)-INDUCED OXIDATIVE STRESS

The objective of this study was to examine whether the combination of n-3 polyunsaturated fatty acids PUFA) supplementation and physical exercise training would decrease oxidative stress in comparison with n-3 PUFA supplementation only in humans. Eighteen women college students were divided into a perilla oil supplemented control group or a perilla oil supplemented physical exercise trained group throughout the experimental period of 4 weeks. After the intervention in both groups, plasma triglyceride levels were decreased. Serum thiobarbituric acid reactive substances (TBARS) level in the control group was increased by n-3 PUFA supplementation. In the trained group, however, TBARS level was unchanged. Plasma vitamin C level in the trained group was significantly decreased by n-3 PUFA supplementation. These results suggest that the combination of n-3 PUFA supplementation and physical exercise training might result in attenuated tissue damage induced by reactive oxygen species, if appropriate daily antioxidants, especially vitamin C, were provided.
2.EFFECT OF N-3 POLYUNSATURATED FATTY ACID SUPPLEMENTATION ON AEROBIC CAPACITY IN YOUNG WOMEN

N-3 polyunsaturated fatty acid supplementation has been recognized to affect the peripheral oxygen delivery system with increasing blood rheology. The purpose of the present study was to investigate whether n-3 polyunsaturated fatty acid supplementation, using purified perilla oil rich in α-linoleic acid, improves aerobic capacity in young women. Eighteen young, sedentary female college students were divided into an n-3 polyunsaturated fatty acid supplemented control group (PUFA-C, n=10) and an n-3 polyunsaturated fatty acid supplemented trained group (PUFA-T, n=8). All subjects took 20g of perilla oil (11g of n-3 polyunsaturated fatty acid) in addition to the usual diet throughout the experimental period of 4 weeks. PUFA-T subjects exercised for 30 min on a bicycle ergometer (intensity, 60% of VO2max) 4 times a week for 4 weeks. Maximal oxygen uptake (VO2max) and oxygen uptake at the ventilatory anaerobic threshold level (VT) significantly (p<0.05) increased after treatment in both groups. However, the endurance time in the exhaustive exercise test significantly (p<0.05) increased in the PUFA-T group only. Increasing rates of VO2max and VT with treatment for the PUFA-C group were lower than those for the PUFA-T group (VO2max, 12.6% vs 14.4%, VT, 9.7% vs 16.9%). After treatment, these values returned to baseline levels within 2 months of the recovery period without n-3 polyunsaturated fatty acid supplementation in both groups. Only for the PUFA-T group, VO2max and VT at 2 months after the treatment period were significantly (p<0.05) higher compared with baseline levels. These results suggest that n-3 polyunsaturated fatty acid supplementation might have a beneficial effect on improving aerobic capacity with increasing peripheral oxygen delivery. However, n-3 polyunsaturated fatty acid supplementation was less effective than aerobic training.
3.Evaluation of hypertensive effect of the acupuncture to the carotid sinus by means of indirect instantaneous sphygmomanometry.

During and after the “Toshi” which is the acupuncture of the carotid sinus, systolic and diastolic blood pressure were measured in the basal pharanx of finger arteries by means of indirect sphygmomanometer called the “volume compensation method”. After 15 minutes resting, 2 minutes acupuncture on both sides of the carotid sinus was carried out in 16 hypertensive subjects. Finger arterial pressure showed a significant decrease by about 14mmHg (mean) in systolic pressure and by 9mmHg (mean) in diastolic pressure during and after the acupuncture (p<0.05). The decrease in blood pressure showed a maximum at 15-20 minutes after the withdrawal of needles. Pulse pressure and heart rate did not show a significant change. These results were almost similar to those obtained in normotensive subjects. These data suggests that hypertensive subjects are also responsible to “Toshi” and that this effect may not be caused by the cardiac inhibition refrex but the change in vascular tonus.
4.Left Subclavian Artery Occlusion: Femoro-Axillary Artery Retrograde Bypass.

The treatment tactics for subclavian artery occlusion include the more commonly used endovascular therapy rather than surgical intervention. We present a case of a 61-year-old woman with dialysis-dependent chronic renal failure who experienced left finger necrosis in the left upper extremity. To salvage the limb, we performed femoro-axillary (fem-ax) artery bypass using an autologous saphenous vein graft. However, 10 months later, she experienced coldness in the left forearm. Angiography revealed chronic total occlusion of the venous bypass. Despite emergent thrombectomy, redo fem-ax artery bypass operation was performed using a prosthetic graft. Upper limb salvage can be achieved by fem-ax artery retrograde bypass.

5.Noninvasive measurement of volume elastic modulus in human finger arteries during and after the LI4 and LI10 acupuncture.

During and after the LI4 and LI10 Acupuncture, the volume elastic modulus Ev in the basal phalanx of finger arteries were measured. Ev values were noninvasively obtained using a new type of plethysmograph called the “electric impedance-cuff” which can detect indirect arterial pressure and volume change in the artery. After 15 minutes of rest, 2 minutes acupuncture on LI4 and LI10 were carried out in 16 healthy subjects, respectively. Volume elastic modulus showed a significant decrease (p<0.01) during and after the LI10 Acupuncture. Its decrease were maintained at least 20 minutes after the withdrawal of the needle. While the Ev values did not show the typical change during and after the LI4 acupuncture. These date suggest that the elasticity of the peripheral wall was lagely affected by the LI10 acupuncture, and the effect was caused not only by peripheral reflex but by other factors.
6.The status of malaria before and after distribution of ITNs from 1999 to 2006 in two districts of Khammouanne Province, Lao P.D.R

In 1999, an insecticide-treated net (ITN) distribution project was started in several malaria-endemic villages in Lao P.D.R., namely Vientiane, Bolikhamxay and Khammouanne Provinces. After the completion of the project, it was found that the ITNs were effective for malaria control based on the analysis of the slide positivity rate for malaria parasites between 1999 and 2000 [6]. We conducted malariometric, entomological and KAP surveys in 2005-06 to confirm the status of malaria and to determine the effectiveness of ITNs in three different socioepidemiological areas (near city, rural and remote), represented by the Xebangfay and Boualapha districts, Khammouanne Province.
A marked decrease in the annual malaria incidence and slide positivity rate was noted from 1999 to 2006 in the two districts. The malaria prevalence was significantly reduced in near city areas (5.6%-10.7% in 1999 to 0% in 2005-06) and rural areas (21.4%-50.9% in 1999 to 0%-1% in 2005-06). Twelve positive cases were recorded in remote areas (3.4 - 7.7% in 2006).
The illiteracy rate was significantly higher among the respondents in the remote (83.9%) than among those in the near city (32.7%) and rural (54%) areas. In all areas, more than 50% of the villagers indicated that they were not aware of malaria. Similarly, about 60% of the respondents were found to have incorrect knowledge concerning malaria transmission. In remote, 40.9% of the respondents were aware of the methods to prevent malaria infection and 49.5% used mosquito nets throughout the year, rates significantly lower than the corresponding rates in near city and rural. The density of persons per net in remote (3.1-5.9) was considerably higher than that in near city (1.8-2.1) and rural (1.2-2.7). Malaria vector mosquitoes, such as An. minimus and An. nivipes were collected by human-baited adult collection, and cow-baited and CDC light traps. The results of the analysis suggested that the lower the number of persons per net the more effective the reduction of malaria morbidity in these areas. It is imperative that ITN distribution and health education regarding malaria be strengthened, especially in remote, but also in other areas.
7.Age-Related Prevalence of Periodontoid Calcification and Its Associations with Acute Cervical Pain

STUDY DESIGN: Prospective study. PURPOSE: To assess the prevalence of periodontoid calcification and its associations with acute cervical pain. OVERVIEW OF LITERATURE: Calcium pyrophosphate dihydrate (CPPD) deposition disease is a common rheumatological disorder that occurs especially in elderly patients. Although CPPD crystals induce acute arthritis, these crystals are not usually symptomatic. Calcification surrounding the odontoid process (periodontoid calcification) has been reported to induce inflammation, resulting in acute neck pain. This disease is called crowned dens syndrome. Whether calcification induces inflammation or whether the crystals are symptomatic remains unclear. METHODS: The prevalence of periodontoid calcification at the atlas transverse ligament was examined by computed tomography of the upper cervical spine in patients suspected of brain disease but no cervical pain (control group, n=296), patients with pseudogout of the peripheral joints but no cervical pain (arthritis group, n=41), and patients with acute neck pain (neck pain group, n=22). Next, the correlation between the prevalence of periodontoid calcification and symptoms was analyzed. RESULTS: In the control group, 40 patients (13.5%) showed periodontoid calcification with no significant difference in the prevalence with gender. The prevalence of calcification increased significantly with age (p=0.002). In the arthritis group, 26 patients (63.4%) reported periodontoid calcification. In the neck pain group, 14 patients (63.6%) reported periodontoid calcification. Multiple logistic regression analysis by age and group revealed that higher age, inclusion in the arthritis group, and inclusion in the neck pain group significantly affected the prevalence of calcification. CONCLUSIONS: Our results cumulatively suggest that periodontoid calcification is an aging-related reaction and that calcification per se does not always cause neck pain. Periodontoid calcification was observed more frequently in patients with pseudogout of the peripheral joints and in those with acute neck pain than in asymptomatic control patients.

8.An update of oncologic and obstetric outcomes after abdominal trachelectomy using the FIGO 2018 staging system for cervical cancer: a single-institution retrospective analysis

Objective:
To apply the International Federation of Gynecology and Obstetrics (FIGO) 2018 staging system to all patients who underwent trachelectomy in our previous study and to update the oncologic and obstetric results.
Methods:
We retrospectively reviewed the medical records of patients in whom abdominal trachelectomy was attempted between June 2005 and September 2021. The FIGO 2018 staging system for cervical cancer was applied to all patients.
Results:
Abdominal trachelectomy was attempted for 265 patients. Trachelectomy was converted to hysterectomy in 35 patients, and trachelectomy was completed successfully in 230 (conversion rate: 13%). Applying the FIGO 2018 staging system, 40% of the patients who underwent radical trachelectomy had stage IA tumors. Among 71 patients who had tumors measuring ≥2 cm, 8 patients were classified as stage IA1 and 14 as stage IA2. Overall recurrence and mortality rates were 2.2% and 1.3%, respectively. One hundred twelve patients attempted to conceive after trachelectomy; 69 pregnancies were achieved in 46 patients (pregnancy rate: 41%). Twenty-three pregnancies ended in first-trimester miscarriage, and 41 infants were delivered between gestational weeks 23 and 37; 16 were deliveries at term (39%) and 25 were premature deliveries (61%).
Conclusion
This study suggested that patients judged to be ineligible for trachelectomy and patients receiving overtreatment will continue to appear using the current standard eligibility criteria. With the revisions to the FIGO 2018 staging system, the preoperative eligibility criteria for trachelectomy, which were based on the FIGO 2009 staging system and tumor size, should be changed.
