1.Studies on Urinary Tract Infections.

An increasing attention has been called to such the problems as on drug resistance of bacteria, cross infection, or hospital infection in the recent antibiotic era. Urine from 131 patients supposed to have urinary tract infectionswere used for bacteriologic-culture and sensitivity determination for isolated icroorganisms in this study. Quantitative urine cultures in 96 cases were also performed and the following results were obtained; 1. Of a total of 171 bacterialstrains isolated by routine cultures of the urine, one hundred and thirty-three were gram-negative bacilli and remaining 41 (23.6%) were gram-positive cocci. Isolated microorganisms include Staphylococci, Klebsiella aerogenes, Pseudomonas aeruginosa, E. coli and Proteus species, in order of frequency. 2. The gram-negative bacilli showed considerable susceptibility to Kanamycin in a range of 58.1 to 90.4% and were less sensitive to Chloramphenicol and Tetracycline. Pseudomonas strains were susceptible only to Polymyxin B. and resistant to all the other antibiotics, Proteus and Klebsiella aeruginosa were resistant to all the antibiotics used in this experiment. 3. 78.3% of the bacterial strains isolatedwere resistant to more than four kinds of antibiotics. 4. Of 96 cases examined by serial quantitative urine cultures, 56 (58.3%) should more than 10 organisms per milliliter of urine and most of them were indwelling catheter.

2.A Case Report of Giant Hydronephosis.

3.Complications of Transurethral Resection of Bladder Tumor.

A clinical observation was made on the 236 patients who had undergone transurethral resection of bladder tumor(TUR) between Jan. 1990. and Feb. 1996. in the Department of Urology, Korea Cancer Center focusing on complications. The mean operating time was 59.0 minutes(n=400). The most common immediate complication was bleeding(1.8%), and the remainder was nonurologic fever(0.8%). The delayed complication was urethral stricture(1.0%) requiting visual urethrotomy. The immediate postoperative morbidity was 2.5% and risk factors for immediate morbidity after TUR were operating time longer than 60 minutes and tumor, invading the muscle or more deeply(p<0.05, by Chi-square test). The delayed morbidity was 1. 0% and the only risk factor for delayed morbidity after TUR was the number of ~IJR more than 2(p<0.05, by Chi-square test). Careful attention to surgical details and indications is needed to reduce the amount and significance of the postoperative morbidity.

5.The Role of Computed Tomography of the Bladder Cancer with Perivesical Fat Infiltration.

Total cystectomy was performed to the 40 patients with urinary bladder tumor who were found perivesical fat infiltration by computerized tomography. 25 patients were previously untreated, and 15 patients had undergone transurethral resection or biopsy of the tumor before computerized tomography. The histopathological stage was compared to the preoperative computerized tomography stage. In pathologic specimen, perivesical fat infiltration was found in 62.5% of the cases, whereas perivesical fat infiltration was not found in 37.5% of the cases. In untouched tumors or after transurethral resection computerized tomography accuracy demonstrated no significant difference.

6.A Case of Retroperitoneal Castleman`s Disease.

We report a case of Castleman`s disease of the hyaline-vascular type which was located in retroperitoneum. A 20-year-old girl was admitted because of a history of postprandial epigastric discomfort. Computed tomography of the abdomen demonstrated a solid 7 x 7 cm mass between the abdominal aorta and the left kidney in the retroperitoneum. At surgery, a solid 9 x 8 x 6 cm tumor was resected. Pathologic diagnosis was Castleman`s disease of the hyaline-vascular type.

7.Diagnostic Values of Abdominal Ultrasonograpy in Patients with Fever and Abdominal Symptoms.

PURPOSE: Acute febrile illness in children frequently accompanies with abdominal symptoms such as vomiting, diarrhea, and abdominal pain, even if its etiology is not occured from the gastrointestinal tract. If the etiology of fever was unknown and the fever was accompanied with abdominal symptoms, we should be concerned about whether the etiology of fever was originated from the gastrointestinal tract or interpretated from the abnormality in the gastrointestinal tract. This study was performed to evaluate the diagnostic value of abdominal ultrasonography in patients with fever and abdominal symptoms. METHODS: We reviewed retrospectively the medical records of abdominal ultrasonographic (US) findings of 60 cases of acute febrile illness with abdominal symptoms at department of Pediatrics, Ulsan Dongang General RESULTS: (100%), splenomegaly in 4 cases (66.7%), ileocecitis in 1 case (16.7%), enlarged mesenteric lymph nodes and splenomegaly in 4 cases (66.7%), enlarged mesenteric lymph nodes, splenomegaly and ileocecitis in 1 cases (16.7%). 6) The enlarged mesenteric lymph nodes and the abnormalities around the appendix were seen in 7 cases (11.7%), which were confirmed as appendicitis all. 7) The thickening of wall in urinary bladder was seen in 2 cases (3.3%) of acute cystitis and acute hemorrhagic cystitis. 8) The subtle thickening of wall in colon was seen in 1 cases (1.7%) of shigellosis. CONCLUSIONS: The enlarged mesenteric lymph nodes, splenomegaly and ileocolitis on the abdominal ultrasonography in patients with fever and abdominal symptoms aree suggestive findings of typhoid fever. The enlarged mesenteric lymph nodes and the abnormalities around the appendix on abdominal ultrasonography make the rapid diagnosis of acute appendicitis and its complications, when physical examination is difficult in small children and diagnosis of their illnesses is obscure in patients with fever and abdominal pain.

8.A Case of Iliac Artery Aneurysm Masquerading as Pelvic Sarcoma.

Iliac artery aneurysm is rare disease. We report a 60-year-old man who was referred to our hospital because of right hydronephrosis and pelvic mass. A intravenous pyelogram, a computerized tomography scan and a magnetic resonance imaging revealed a pelvic sarcoma. We experienced a case of right internal iliac artery aneurysm masquerading as pelvic sarcoma.

Although genitourinary leiomyoma may arise in any anatomic structure containing smooth muscle, primary leiomyoma of the bladder is rare. This tumor is usually asymptomatic unless urinary tract function is affected. We experienced a case of leiomyoma of the bladder with back pain in a 42 year old woman.

10.Detection of p53 Gene Mutations in Bladder Cancer using PCR-SSCP Analysis and Immunohistochemistry.

We carried out an immunohistochemical study and DNA analysis of 14 surgical specimens of primary bladder carcinoma and paired sample of normal bladder mucosa to evaluate p53 over expression, point mutation by polymerase chain reaction single-strand conformation polymorphism analysis(PCR-SSCP). The immunohistochemical study demonstrated immunoreactivity for p53 protein in ten cases out of 14(71%) with monoclonal antibody (NCL-p53-DO-7, Novocastra Lab. UK). Mobility shifts were detected by PCR-SSCP in p53 gene exons 4-8 in six cases out of 14(42.9%). p53 gene mutations were observed in invasive cases(85.7%) and superficial cases (50%) by immunohistochemistry. Correlation was not observed between the presence of p53 gene mutations and the histological differentiation and invasiveness of the cancer. Considering frequent p53 alterations in invasive cases, it will be necessary to reevaluate p53 alterations in bladder cancer and the correlation to the prognosis in bladder cancer.
