1.Effect of astragalus membranaceus on serum free amino acids and chemokines in patients with rheumatoid arthritis

Objective To study and observe the effect of astragalus membranaceus on serum free amino acids and chemokines in patients with rheumatoid arthritis. Methods 60 patients with rheumatoid arthritis in Lishui Hospital of traditional Chinese medicine from October 2014 to May 2016 were selected as the research object,and 60 patients were divided into control group and observation group with 30 cases in each group,according to the method of random number table,the control group were treated with conventional treatment of rheumatoid arthritis,the observation group were treated with astragalus mongholicus on the basic of control group,then the serum free amino acids and chemokines of two groups before the treatment and at 10th,20th,40th and 60th day after the treatment were respectively detected and compared. Results The serum free amino acids and chemokines of two groups before the treatment were compared, while the serum chemokines of observation group at 10th,20th,40th and 60th day after the treatment were all lower than those of control group,the serum free amino acids glutamic acid,asparagic acid and phenylalanine were all lower than those of control group,leucine,γ-aminobutyric acid, glycine and taurine were all higher than those of control group,the difference was statistically significant (P<0.05). Conclusion The influence of astragalus mongholicus for the serum free amino acids and chemokines of patients with rheumatoid arthritis is better,so its application in the patients with rheumatoid arthritis is higher.
2.Application of vacuum sealing drainage in the clinical treatment of destroyed hand injuries

Objective To evaluate the effect of vacuum sealing drainage(VSD)ON the destroyed hand injuries.Methods From January 2006 to December 2008,forty pafients with destroyed hand injuries (surface of wound)were randomly divided into VSD group(20 cases)and control group(chlorhexidine wet compress)(20 cases)after debridement. The outcomes were retrospectively analysed by pain score,swelling score,bacterial positive rate and necrotic tissue.Results Three days after treatment,the pain score of VSD group was(2.62±1.54)degree,and that of control group was(3.39±1.64)degree.There wag significant difference between the two groups(P<0.01).The swelling score of VSD group was(1.51±0.25)degree which was different from control group(2.29±1.26)degree(P<0.05).The bacterial positive rate of control group was 20%(4/20),but that of VSD group was 5%(1/20).There was significant difference between the two groups(P<0.05).The VSD group had no necrotic tissue occurrence,but the control group had 2 cases,one of which had serious necrosis and had escapology.Conclusion VSD Can inhibit bacterial infection,ease pain,exempt changing dressings and stimulate growing of granulation tissue also can maintain a good environment for tissue transplantation later.So VSD combining with tissue transplantation is an ideal therapy in destroyed hand injuries.
3.Clinical efficacy of self-made Bitong decoction in lumbar disc herniation patients with cold-dampness syndrome

Objective To analyze the clinical efficacy of self-made Bitong decoction(SBD)combined with etoricoxib tablets(ET)in lumbar disc herniation(LDH)patients with cold-dampness syndrome(CDS).Methods The clinical data of LDH patients attending Lishui Hospital of Traditional Chinese Medicine affiliated to Zhejiang University of Traditional Chinese Medicine from January 2023 to January 2024 were retrospectively analyzed and divided into the ET group and the ET+SBD group according to the treatment plans.The main observed indicators of this study were quality of life[Oswestry Dysfunction Index(ODI)]and pain level[Visual Analog Scale(VAS)],functional status[Modified Japanese Orthopedic Association,M-JOA)scale],treatment response rate,TCM syndrome score and inflammatory factors(IL-1β,TNF-α,and IL-6)after 4 weeks of treatment.The occurrence of adverse reactions during the medication period was recorded to evaluate the safty.Results A total of 86 patients were included in the study,40 in the ET group and 46 in the ET+SBD group.After treatment,the ODI score,VAS score,M-JOA score,TCM syndrome score,IL-1 β,TNF-α and IL-6 levels were significantly lower in the ET+SBD group than those in the ET group,and the treatment efficiency was higher than that in the ET group(P<0.05).In addition,there was no statistically significant difference between the incidence of adverse events in the ET+SBD group and the ET group(P>0.05).Conclusion SBD combined with ET can significantly improve the quality of life,pain degree and functional status of LDH patients,and improve the treatment effect,which is closely related to the anti-inflammatory and analgesic effect of SBD.
