1.A Case of Hyperkeratosis of the Nipple and Areola Aggravated During Pregnancy.

We report a case of hyperkeratosis of the nipple and areola occuing in 33 year old woman. She did not have a history of endocriruipathy, horrnonal therapy and other ceatoses, so this case seems to be the nevoid form by Levy-Franckel classification. Interestingly, however the lesion which had developed before marriage suddenly aggr avated during pregnancy. This may be a unique feature which has not been reported elsewhere. Histapathologic findings are compatible with breviously reported eases. There was no response to topical steroid and retinoic acid treatrnent.

2.Extremity Amputation following Radial Artery Cannulation in Patient with Craniectomy.

The technique of radial artery cannulation and its complications are well documented, but serious complications are rare. This is a report of one case of amputation of wrist due to finger necrosis developed from the radial artery cannulation in patient who had craniectomy surgery. This 52-year-old 79 kg male underwent subdural hematoma removal surgery. Right radial artery cannulation was carried out percutaneously using 22 gauge Teflon extracath needle after modified Allen's test appeared to be positive. It was intermittently flushed by heparinized solution. His arterial blood pressure was maintained 100/70-110/80 mmHg and 5 units of banked whole blood and 1 unit of fresh frozen plasma were transfused during 8-hours operation. Cannula was removed on the 9th hour after operation because that was obstructed. On the 12th hour after removal of cannula, his right hand noted to be cool and cyanotic. So, warm towel and hot bag applied continuously on the right hand and the right stellate ganglion block was carried out every day for 4 times. However, on the 10th day after removal of cannula, necrotic change of all fingers of the right hand became worse and skin of fingers were shrunken. Therefore, disarticulation of the right wrist carried out on the 71th day of his hospitalization.

3.A Case of Cryptococcosis with Cutaneous Manifestation.

Cryptococcosis is a disease caused by the Cryptococcus neoformans, occcuring most frequently in immunocompromised hosts. Cutaneous involvement is seen in 10-15 % of disseminated cases and its manifestation is variable. A 52 year old man presented with a subcutaneous neck mass and severe headache which had lasted for 2 months and 1 month respectively. Initially cutaneous involvement was monomorphic and localized and a CSF study failed to reveal any organisms. After several weeks of herb medication, however, multiple skin lesions occurred with varied morphology and a CSF study confirmed cryptococcosis by culture. The Urine cortisol was markedly elevated, suggesting an exogenous intake of steroid.

4.Decompressive Craniectomy for Acute Cerebral Infarction.

We present a series of 10 Patients(Seven men and three women with an average age of 53 years) who underwent decompressive craniectomy for treatment to massive brain swelling following acute cerebral infarction. Clinical signs of cerebral herniation(anisocoria or fixed and dilated pupil, and/or hemiplegia with decerebrate righidity) were present in all patients. Computed tomography and magnetic resonance imaging showed the mass effect by cerebral edema through midline shift. All patients were treated with an extensive craniectomy and duroplasty. Among them, one recovered without neurological deficit, three were moderately disabled but functionally dependent, three remained in a persistent vegetative state and three died within 9 days after surgery(good recovery=1, moderate disability=3, persistent vegetative state=3, death=3). The results suggest that decompressive craniectomy can be an useful lifesaving procedure for massive cerebral edema following widespread hemispheric infarction.

6.Clinical Observation on Pyelonephritis.

The clinical observation was made on 233 cases of pyelonephritis in the Department of Urology, Seoul Adventist Hospital, during the period from 1978 to 1980. The following results were obtained: 1. The incidence of pyelonephritis was 36.7% of the total number of in-patient admitted to the Department of Urology during this period. 2. The age ranged from 10 to 70 years and most favorable age was from 31 to 40 years for 45.3%. 3. The sex ratio of male and female was about 1 : 4. 4. The most common symptoms were flank pain (89. 0%) fever, chill (81. 1%), abdominal pain (34. 0%), lumbago (29. 2%), hematuria (76.4%) and others. 5. In urine culture. The most common organisms were E. Coli (72.9%). Staphylococcus aureus (19. 6%). Pseudomonas aeruguinosa (46%) In urine A.F.B. Stain, 26 cases (11. 0%) were positive. 6. On the I.V.P. findings of pyelonephritis, 83 (35.6%) cases were normal and 150 cases (64.4%) abnormal. 7. The underlying diseases associated with pyelonephritis were cystitis (31. 3%), diabetes or debilitating illness (6.9%), tuberculosis of urinary tract (11%), calculi of urinary tract (29%), congenital anomaly of urinary tract (4. 7%), instrumentation (1.9%), pregnancy (1%), etc. (7. 2%). 8. On treatment of pyelonephritis, 143 cases were performed with conservative, medical treatment, and other cases ware performed with operation, for examples: ureterolithotomy in 51 cases, nephrectomy 26 cases, nephrolithotomy, pyelolithotomy in 6 cases, T.U.R., partial nephrectomy in each 2 cases, etc. or with combined treatment.

8.Sparganosis Mimicking Cutaneous Larva Migrans.

A 53-year-old man had skin-colored linear serpiginous plaque within brownish patch on his left thigh for one year. He habitually drank natural spring water for 3 years. We diagnosed this case as sparganosis infiltrated within dermis by histologic and parasitologic diagnosis. The skin lesion mimicked the cutaneous larva migrans by showing dermal migration of sparganum to pro-duce itchy linear serpiginous plaque. We report an interesting case of sparganosis mimicking cutaneous larva migrans.

9.Measurement of Blood Retina Barrier in Branch Retinal Vein Occlusion (BRVO).

Vitreous fluorophotometry was used to measure blood retinal barrier permeability to fluorescein in 15 patients with branch retinal vein occlusion(BRVO). Mean posterior vitreous fluorescein concentration(3mm) was 20.0 +/- 11.3(ng/ml) in affected eyes, and 2.99 +/- 1.22(ng/ml) in unaffected eyes. There was a statistically significant difference between the affected eye and unaffected eye(p<0.05). Also there was a correlation between the hemorrhage area and the posterior vitreous fluorescein concentration(r2=0.819). This study revealed that the permeability of blood retinal barrier was increased in BRVO as compared to the contralateral eye, and the higher permeability values were associated with the extent of area involved.
