1.Transphyseal extension of osteosarcoma: MRI and pathologic correlation.

We retrospectively analysed plain radiographs and MR imaging of 16 conventional osteosarcoma cases(8 children, 8 adults) which underwent amputations or limb salvage operations. Pathologic correlation was performed with gross and microscopic sections to evaluate 1) whether the open epiphyseal plate can function as a barrier against transphyseal spread of osteosarcoma and 2) the diagnostic value of MR imaging for the detection of the detection of the epiphyseal involvement of osteosarcoma. In children with open epiphyseal plates, conventional radiographs suggested transphyseal tumor growth in one of eight cases(12.5% and MR imaging in seven cases(87.5%). Pathologic examination confirmed epiphyseal involvement in six of seven cases noted with MR imaging(75%). On the the other hand, in adult patients with closed epiphyseal plates, conventional radiographs showed transphyseal tumor growth in six of eight cases(75%), while MR imaging and pathologic exam demonstrated tumor invasion in all cases(100%). We conclude that open epiphyseal plate does not function as and effective barrier against tumor extension, and MR imaging is an excellent method in detecting the extent of transphyseal tumor growth.

3.Efficacy and Safety of Commerical Used Viscoelastics: Healon(R), Provisc(R), Viscorneal(R), Hyal 2000(R), Biolon(R), Viscoat(R).

Several different viscoelastic substances are widely used in many ophthalmologic fields. In our study, the efficacy and safety of the different kinds of viscoelastics -Healon(R), Provisc(R), Viscorenal(R), Hyal-2000(R), Viscoat(R) and Biolon(R)-was evaluated in undergoing phacoemulsification and posterior intraocular lens implantation, corneal transplantation and ocular trauma surgery. We evaluated and the facility of the injection the foreign materials during injection of viscoelastics through the microscopy. We measured the changes of intraocular pressure at the postoperative 1 day and corneal endothelial cell count at the postoperative 2 month. The mean endothelial cell loss after phacoemulsification and posterior intraocular lens implantation was 13.71% at postoperative 1 day and there was no significantly different among groups. The incidence of the foreign materials including air bubles in viscoelastics in the operating microscope was Provisc(R), Hyal 2000 Viscorneal(R), Biolon(R), Healon(R) in order. The mean incidence of the intraocular pressure above 25 mmHg after cataract surgery at postoperative 1 day was 3.94%and was Hyal 2000(R), Biolon(R), Viscorneal(R), Healon(R), Provisc(R) in order. The incidence of the additional use of the antiglaucoma drugs in keratoplasty patients was 76%in Healon(R) group, 26.7%in Viscoat(R) group, 69.2%in Provisc(R) group respectively. From the results of this report, we conclude that it is desirable to use the proper viscoelastics according to the chemical and physical properties in special situations, but there is no significant difference in the same viscoelastic substance.

5.Neonatal Retinal Hemorrhages and Influence of Perinatal Factors.

Retinal hemorrhages in the newborn infant were first described shortly after the invention of the ophthalmoscope(Jaeger. 1961). Frequency of the retinal hemorrhages in the newborn infant has been reported to be between 2.6 and 50%. We postulate that the retinal hemorrhages are caused by maternal factors and fetal factors. Maternal factors include delivery type, maternal age, duration of pregnancy, parity, duration of labor, and maternal blood pressure. Fetal factors include color of the amnionic fluid, body weight, and sex. We examined the fundus of 161 newborn infants and investigated the above factors that influenced the retinal hemorrhages. The results were as follows: 1. The overall incidence of the retinal hemorrhages were 35.4%. 2. According to the delivery types, retinal hemorrhages were found in 57.9% after vacuum extraction, in 41.7% after induction delivery, in 33.8% after spontaneous delivery, and in 13.5% after Cesarean section. 3. As the duration of pregnancy increased in complicated delivery, retinal hemorrhages also tendted to increase. 4. The incidence of hemorrhages were greater in the hypertensive mother and abnormal color of amnionic fluid in spontaneous delivery. 5. The incidence of retinal hemorrhages were greater in the high body weight, multipara, and normal duration of labor. 6. Retinal hemorrhages were not related the maternal age and fetal sex. 7. Retinal hemorrhages were absorbed within four weeks.

6.A Case of Gangrenous Ischemic Colitis Associated with Rectal Carcinoma.

The ischemic coitis proximal to obstructive colon cancer was firstly reported by MacCallum and Kaisch in 1947. Nowsdays, its clinical importance and the study of pathogenesis is reported frequently. Because of the rare inciedence of this complicated condition this lesion could be overlooked which then could result anastomotic leakage, fecal fistula and peritonitis. Recently, we have examined a 61 year-old Korean male patient who had the complaints of acute abdominal pain and bloody diarrhea. The diagnosis of gangrenous ischemic colitis associated with obstucting rectal cancer was confirmed on the basis of sigmoidoscopic and operative and associated histopathological findings. After prompt surgical intervention, the patient was discharged with good improvement. The literatures are reviewed here with the case presentation.

8.Efficacy of Sotalol and Amiodarone for Atrial Fibrillation.

BACKGROUND AND OBJECTIVES: Atrial fibrillation(AF) is the most frequently encountered arrhythmia in clinical practice. Pharmacologic therapy has been advocated for both immediate restoration of sinus rhythm and prevention of recurrent AF. Because conventional antiarrhythmic therapy is often ineffective in maintaining sinus rhythm or is associated with adverse side effects in patients with AF, recent interest has focused on the use of class III antiarrhythmic agents. This study investigated the efficacy and safety of sotalol and amiodarone for conversion of chronic AF and prevention of recurrent AF. MATERIALS AND METHOD: Thirty six patients with AF were firstly received sotalol by prospective study protocol. The patients were classified as having paroxysmal AF(PAF, N=2) or chronic AF(CAF, N=4) based on AF pattern. If the patients with CAF did not convert to sinus rhythm or the patients with PAF recurred in AF, the patients were received second agents(amiodarone). Patients were followed up for one year. RESULTS: Among the 12 patients with PAF receiving sotalol, 10(83.3%) patients remained in normal sinus rhythm for average 9.4+/-3.6 months. Sotalol was replaced by amiodarone in the remaing 2 patients with arrhythmia recurrence and 1 of the 2 patients remained in sinus rhythm during the follow-up period. In the case of 24 patients with CAF, conversion to sinus rhythm was achieved in 5(20.8%) patients with sotalol. Among the patients with CAF who were not respond to sotalol, 17 patients received amiodarone subsquently and 3 patients successfully converted to sinus rhythm. There were no proarrhythmic effects related to both agents during the study period. CONCLUSION: Both sotalol and amiodarone appear to be less effective in the termination of CAF, but sequential use of these two agents seem to be very effective for the prevention of recurrence of PAF.

9.A Case of Spontaneous Rupture of Renal Angiomyolipoma.

We experienced a case of spontaneous rupture of renal angiomyolipoma appeared unilateral and solitary tumor without stigma of tuberous sclerosis in 49 years old women. Renal angiomyolipoma which are not associated with tuberous sclerosis appear at an average of 48 years and are more common in women with a ratio of 6:1 and is a benign tumor. Angiography may be helpful in the differentiation of an angiomyolipoma from renal cell carcinoma. The first symptoms are often due to sudden spontaneous perineal hemorrhage. The patient was managed by left total nephrectomy with good result.
