2.A Case of Halo Dermatitis around Seborrheic Keratosis.

A case of circular eczematous dermatitis around seborrheic keratosis was presented. A 54-year-old man presented with two weeks history of a round pruritic eruption around a preexisting lesion of seborrheic keratosis. Microscopic findings showed central tumor nest composed mainly of basaloid cells with occasional spongtosis and exocytosis. A diffuse, dense, mononuclear cell infiltrate with increased contents of melanophages was seen beneath the tumor. The surrounding lesion of halo dermatitis showed mild dilatation of the capillaries and perivascular infiltration of mononuclear cells in the upper and mid-dermis. The lesion of halo dermatitis disappeared gradually over a three week period following excision of the central lesion. This peculiar phenomenon seemed to be different from that of Suttons halo nevus, and might be a manifestation of an eczematous condition which might have a dermatitis provoking factor.

3.A case of twenty-nail dystrophy associated with chronic thyroiditis and universalis.

Twenty-nail dystrophy is a rare entity in which all 20 nail are uniformly and simultaneously affected with excess longitudinal ridging and loss of luster. This condition is thought to be idiopathic but, many cases are associated with alopecia areata, lichen planus, psoriasis, and ichthyosis. In such cases, it has been hypothesized that immunplogical disorders could play a role in pathogenesis. A 29-year-old male patient presented with dystrophic nail chatge on his all finger and toe nails for 3 years. On phisical examination, all nails were uniformly affected with longitudinal ridging and loss of luster and all body hairs were lost. Thyroid scan showed a finding of chronic thyroiditis. Herein we report a case of twenty-nail dystrophy associated with chronic thyroiditis and alopecia areata.

4.A case of amyopathic dermatomyositis.

A 50-year-old female, who had been suffered from diabetes rielitus, showed typical cutaneous findinhs of dermatomyositis with only transient and mild proximal muscle weakness at sometime during the course of disease. She had Gottrons papules, Gottrons sign, heliotrope rash, macular violsceous erythema involving neck, anterior chest, back, extensor site of forearms and arms and dorsa of hands. However, he serum CPK, LDH and sGOP levels were within normal limits. Her fasting and 2 hours potprandial blood glucose levels were elevated. There were no histological abnormality in muscle biopsy. Histopathologic findigs of the skin lesion on her back were that of poikiloderma atrophicans vasculare. We diagnosed our patient as Rebeccas type 2 amyopathic dematomyositis which is chracterized by skin disease with subjective myalgias and musle weakness, but without laboratory evidence of muscle disease.

Erythema multiforme can be induced by various causes, including infections like virus or fungus, drugs like sulfonamides and penicillins, internal malignences, connective tissue diseases, pregnancy, menstruation, and exoessive sun exposure ramoxone, one of the paraquat dichloride compounds, is the most commonly used weed-kiIler in Korea. It is sometimes used in suicides due to its fatal toxicity, and even weak transutaneous absorption can lead to death. 6.Sclerosing lymphangitis of the penis: a case report.

Sclerosing lymphangitis of the penis occurs as a skin-colored or dusty red, cord-like thickening of tissue at the corona or within the coronal sulcus. The thickened, elongated lesion is firm and relatively nontender. The cause is unknown: because it occurs primarily in those who are sexually very active, however, it is likely that chronic trauma plays an important etiologic role. We experienced a case of pathologically proven sclerosing lymphangitis in a 31 year-old man. Ultrasonographic finding showed circumferential cord like hypoechoic band with irregular, but well demarcated margin.

8.Clinical Observation on Urolithiasis.

Clinical observation was made on 272 cases of urolithiasis who admitted in the department of Urology, Seoul Adventist Hospita1, during the period from Jan. 1976 to Dec. 1980. The results of this observation may be summarized as follows: 1. Among total admissions (969 cases), those with urolithiasis were 272 cases (28.2%) and the sex ratio of male and female was about 9.2 : 5.9. 2. The highest incidence on age distribution showed in 20 to 40 years (61.0%). 3. On seasonal distribution, occurence was prevalent in summer (3O.8%). 4. The ureter was he most favored predilection site of urinary stone (84.9%) and upper urinary stones were almost noted (95.6%). The most common location of ureteral stones was lower third of ureter (60.2%). 5. In the aspect of number of urinary calculi, the single was in 257 cases (91.8%) and the most common size was below 1.0cm in diameter (64.8%), weight in 0.6-1.0gm (38.4%). In shape, round or oval stones were more prevalent (38.9%) and the 2 cases of staghorn calculi were noted in renal region. 6. The clinical symptoms of upper urinary tact showed flank pain in 91.8%, nausea & vomiting: etc. in 57.8%. hematuria in 47.8% but in lower urinary tract, hematuria was in 83.3%, vesica1 irritability in some cases. 7. Microscopic hematuria showed in 88.8% and pyuria in 65.3% Positive urine cultures above ever 100,000 colony/ml were obtained in 158 cases (58.O%) and most common organisms were E. coli (56.8%), staphylococcus aureus (21.5%), Proteus mirabilis (8. 2%). 8. It upper urinary stones, hydronephrosis with or without functional deterioration were detected in 91% on excretory urogram or other X-ray films, Among them, moderate hydronephrosis showed in 34.9% 9. The 142 cases of urolithiasis (58.0%) were treated with surgical intervention. Among them, ureterolithotormy was made in 40.7%. 10. On chemical analysis of urinary calculi (107 cases), calcium phosphate and calcium oxalate in mixed type was most common (39.8%). On the chemical component of urinary calculi, most prevalent types were phosphate stone (72.2%). oxalate stone (61.7%), etc.
