We report a case of a photosensitivity reaction in a 76-year-old inale induced by chlorpropamide ingestion. The patient had erythematous scaly patches on the sur. -exposed areas, A phototest revealed the decreased minimal erythemal dose(MED) to UVA(5J/cm). A photopatch test and photo-scartch test with 1% chlorpropamide ointment and 0.1% chlorpropamic!e .olution were all negative. An oral provocation test was performed, which showed a positive result with marked decrease of MED to UVA (5J/cm). After the cessation of chlorpropamide, his skin lesions were improved markedly with complete loss of photosensitivity. Macular hypopigmentations (leukcme.anoderma) appeared on the previous erythematous patchy ereas, but disappeared during the follow-up period.

Partial Unilateral Lentiginosis (PUL) is a rare pigmentary disorder characterized by the numerous lentigines confined to a body segment, with a sharp demarcation at the midline. We report two cases of PUL. A 38-year-old woman had asymptomatic discrete small hyperpigmented macules that were scattered on the T7~L1 dermatomes on the left side of her trunk and were clearly demarcated in the midline on both anterior and posterior sides. In the other case, an 18- year-old woman had hyperpigmented macules that were scattered on the left neck, shoulder, and anterior chest. Other anomalies including neurofibromatosis, neurologic anomalies, and multiple lentiginous syndrome were not related in both cases. We report two cases of PUL having no other anomalies.

A 49-year-old male had erythematous to rusky red papules, indurated plaques and lichenified patches with hyperpigmentation on sun-exposed areas for 6 years. Phototest revealed the decreased rninimal erythemal dose to UVA(10J/cm. Photopatch test with 5% Trandate ointment, 5% hydrochlorthiazide ointment and vaselin. as a control were all negative. Two weeks after cessation of Trandate, an oral challenge of hydrochlorthiazide followed by phototest was perfrirmed resulting in exacerbation of skin lesions and photosensitivity with a decreased MED to UVA(10J/cm) again. After the cesation of Trandate containing hydrochlorthiazide, the skin lesions were improved with complete loss of photosensitivity. But, improvement of the infiltrated or licheified plaques were delayed. Presenile cataract previously noted in the patient seemed to be related to his longstanding intake of hydrochlorthiazide.

We report a case of a phototoxic reaction in a 48-year-old female induced by griseofulvin ingestion. The patient hac! erythematous papules, vesicles and patches on the sun-exposed areas. Phototest revealed a decreased minimal erythemal dose to UVA (10J/cm). Photopatch tests with 1%, 5%, 10% Griseofulvin ointment and vaseline as a control and photoingestion tests with Griseofulvin (50mg b.i.d.) were all negative. After the cessation of Griseofulvin, her skin lesions were markedly improved with complete loss of photosensitivity.

Erythromelalgia is a rare disorder characterized by intense hun ing pain in the distal extremities associated with erythema and increased skin temperature. The patient was a 42- year-old woman with a 5 year history of burning pain, erythema, and warmth of the skin on both hands and feet. She had been suffering from bronchiali astalima since childhood. The onset of the symptoms were temporally related to the attacks of hei bronchial asthma. Physical examination revealed mottled dusky red patches on her both ha ids and feet, and purplish acrocyanosis on the finger and toe tips. We present a case of secondary erythromelalgia associated with bronchial asthma and acrocyanosis.

OBJECTIVE: Frozen-section evaluation of gynecologic tumors has historically been used to establish a histopatholgic diagnosis and to guide surgeons in appropriate surgical procedure. this study is to assess the clinical appropriation and accuracy of frozen-section biopsy in gynecologic tumors. METHODS: This is a retrospective quality assurance study all of frozen-section biopsy done at Chungnam National University Hospital during a three year period (1995 to 1997). There were 125 cases out of 2310 surgical specimens for gynecologic tumors, and the results were compared with final diagnoses based on fixed tissue specimens. RESULTS: Frozen section biopsy was accurate in 95.2% of all cases, in 96.3% (104/108) of ovarian tumor cases, and in 88.2% (15/17) of uterine tumor cases. Analysis of 6 inaccuracy cases revealed that most cases were sampling errors and occurred in mucinous tumor, belonging to all cases of ovarian tumors. Sensitivity, specificity, positive predictive value, and negative predictive value of frozen-section biopsy in ovarian tumors were 0.94, 1.00, 1, 00, 0.97. CONCLUSION: Frozen section biopsy is an appropriate method as a basic guideline during operations of gynecologic tumors and this method can obtains the highest accuracy when there is cooperation between experienced surgeons and reliable and careful pathologists. The accuracy of frozen section diagnosis has important implications regarding the type and extent of the primary surgery for gynecologic tumors.

Calcifying aponeurotic fibroma is a rare benign tumor found primarily on the hands or wrists of children and adolescents. It has a tendency of local invasion, and a high recurrence rate after excision. An 11-year-old boy had a hard indurated. 3 x 3cm-sized, nodule on the right sole for 3 years. A biopsy specimen showed heavy focal calcification within areas of densely paked stromal fibroblastic cells. Some fields contained multinucleatec giant cells and chondroid metaplasia. There were infiltrative growth patterns of fibroblastic cells extending into the adjacent adipose tisse and blood vessels. There was no evidence of reccurence at 3 months after the operation.

Based on our experience with 23 frontotemporal reconstructions performed, this paper attempts to provide guidelines for appropriate selection among the four most commonly employed materials (e.g., autogenous parietal bone, silicone rubber, methylmethacrylate, and porous polyethylene) so that optimal results can be achieved. Clinical follow-up ranged from 6 to 91 months (mean 32 months). Every patient was analyzed clinically by ordinary scale method. The mean defect size was 134.8 cm2 in the silicone rubber, 36.5 cm2 in the methylmethacrylate, 17.4 cm2 in the autogenous bone graft and 7.3 cm2 in the porous polyethylene. The clinical assessment was excellent (mean, 29.3 points) in silicone rubber, excellent (mean, 28.6 points) in autogenous bone, excellent (mean, 26.8 points) in methylmethacrylate, and good (mean, 24.8 points) in polyethylene. To sum up, a large bony defect of congenital calvarial anomaly produced an excellent result using custom-made silicone implant. A relatively small bony defect with a scarred bed produced an excellent result using autogenous parietal bone grafting. Unexpected and medium-sized defect was reconstructed successfully through a simple procedure using methylmethacrylate. Porous Polyethylene was used at an incidental small defect because of its expensiveness.
