1.The Effect for Intracranial Pressure during Laryngoscopy and Endotracheal Intubation.

It is well known that intracranial pressure (ICP) and mean arterial pressure (MAP) are increased by laryngoscopy and endotracheal intubation during induction of general anesthesia, and it may be very dangerous in neurosurgical patients who had increased ICP. Therefore, this study was performed to know the range of ICP increase during induction of the conventional general anesthesia with intubation following thiopental and succinylchohne injections. Intracranial pressure and MAP were measured in 13 patients who underwent craniotomy. All the patients were monitored cerebral epidural ICP and intraarterial pressure preoperatively. The results were as follow: 1. Intracranial pressure was increased of 7.1±7.23 mmHg. 2. Arterial pressure was increased of 43.5±25.46 mmHg. 3. Cerebral perfusion pressure was increased of 33.3±27.53 mmHg. It is stressed that certain procedures are necessary to prevent from further increase of ICP due to induction of general anesthesia in patients with increased ICP.

2.Evaluation of the Phototoxic Potential of Some Quinolone Antibiotics.

BACKGROUND: The photsensitizing effect of quinolones has been recognized since their introdulation as an antibacterial agents. Recently several new second eneration antibacterial agents of this pharmacological class have become available for therapy, and are gaining increasing impotance. OBJECTIVE: To reveal the phototoxic potentials of some new quinolones by photohemolysis test, estimation of fluorescenc spectra, and Candida albicans test. METHODS: Nalidixic acid and four second-generation quinolones(ciprofloxacin, enoxacini, norfloxacin, and ofloxacitid were examined by fluorescence spertra which measured t.he phototoxc potentials by photochemial instability, photohemolsis test for the phototoxic properties against cell membranes and Candida tlbicans test for phototoxic properties against DNA. RESULTS: All drugs showed a fluorescence spectra within 360 nm to 450 nm, and in the photohemolysis test, all studied drug except ofloxacin got above 5% hemolytic value, and all drugs showed clear zone. in Candida albicans test after 48hours. CONCLUSION: These results suggested that all tested drugs were photochemically unstable. According to the mechanisris of cellular phototoxicity, ciprofloxacin, enoxacin, and norfloxacin was phtototoxic to nucleus and cell membrane, whereas ofloxacin was phototoxic to nucleus only.

6.Schilder’s disease in a young child with tumefactive demyelinating brain lesion

Schilder’s disease is a rare sporadic demyelinating disease of the brain. We report a girl with Schilder’s
disease who had undergone Kasai operation for biliary atresia. The patient presented with acute right
hemiparesis. Brain magnetic resonance imaging (MRI) showed a single large tumefactive white
matter lesion. A diagnosis of Schilder’s disease was based on clinical features and MRI findings. The
patient showed dramatic clinical improvement and significant regression of the lesion in response to
high-dose intravenous methyl prednisone, and remained free of relapse of other neurologic problems
during the 3-year follow-up.

9.Interleukin-4 as a New Index of Disease Severity in Atopic Dermatitis.

BACKGROUND: Interleukin-4(IL-4) from the Th2 subset of lymphocytes has been known to have a key role in the pathogenesis of atopic dermatitis. Blood levels of IgE, often reflecting the severity of allergic diseases, are not always elevated in patients with atopic dermatitis due to its homocytotrophic characteristics. OBJECTIVE: We propose that IL-4 can substitute or at least complement the role of IgE as an index of disease severity of atopic dermatitis. METHODS: Eleven patients with atopic dermatitis and five normal controls were included in this study. Before treatment, we evaluated the clinical severity of atopic dermatitis by the method described elsewhere with some modification which employed extent, duration, and intensity of skin lesions. Based on this criteria, we divided patients into 'severe and mild groups. With blood samples drawn from patients and normal controls, we measured IL-4 and IgE values. After a treatment period of I to 2 months, when all of the patients became free of symptoms and signs of atopic dermatitis, patients in each group were re-evaluated for ehanges of IL-4 and IgE values. IL-4 values were determined with a Predicata human interleukin-4 kit from Genzyme diagnostics (Predicta IL-4 kit) and ELISA reader (CRES UV900, Biotek). IgE values were measured .with IMx total IgE assay system from Abott laboratories (model 8389-0). RESULTS: l. Of eleven patients, six were mild and five were severe in clinical severity. 2. IL-4 levels were decreased from the pre-treatment average value of 57.149+/- 40.079 pg/ml to a post-treatment average value of 32.072+/- 16.912 pg/ml (p<0.05) which was similar to average normal control value, 36.690+/- 24.451 pg/ml. 3. IgE levels were similarly decreased from an average value of 230.2 86.4 IU/ml before treatment to one of 171.S+/-79.4 IU/ml after treatment (p<0.05). 4. This tendency of post-treatment lowering of values became more apparent in severe groups (p<0.05) but less obvious in mild groups both in terms of IL-4 and IgE (p>0.1). CONCLUSION: We conclude that IL-4 is not only involved in the pathogenesis but can also be used as a main index of disease severity in atopic dermatitis especially when the clinical severity is more than mild.

10.A Case of Leiomyosarcoma of the Scrotum.

Scrotal leiomyosarcoma arising from dartos layer not involving the testis, epididymis or spermatic cord is very rare tumor. We report a case of scrotal leiomyosarcoma in 29 year old man who present with right scrotal painless growing mass. The lesion had been for 3 months but there was no related symptom such as pain or tenderness. The excised lesion was 3 x 3cm sized ovoid mass with smooth surface and it had no relationship with the testis, epididymis and spermatic cord. Its pathologic finding was compatible with leiomyosarcoma. So additional radical orchiectomy was done.
