1.The Treatment of Grade 3 Acute Acromioclavicular Joint Injuries with Modified Neviaser Technique

There has been considerable controversy in the treatment of acute grade 3 injuries of acromioclavicular joint. Neviaser reported that the possibility of a latent degenerative arthritis occurring in the acromioclavicular joint as the result of passing Kirschner wire through the joint was more theoretical than real and any changes that may take place in the acromioclavicular joint were the result of trauma. The acromioclavicular joint and the articular disc were not disturbed at the time of operation in Neviaser technique, but we excised the articular disc that were torn for the purpose of preventing acromioclavicular joint from degenerative arthritis. We had treated 26 cases of grade 3 injuries of acromioclavicular joint with modified Neviaser technique from June 1990 to May 1994 at the Pohang St. Mary’s Hospital. The results were as follows. 1. The clinical results by Weitzman were consisted of 18 cases(69%) of excellent, 6 cases(23%) of good, 2 cases(8%) of fair and 0 case(0%) of poor. 2. Loss of reduction was 1.6mm by stress views of final reviews and didn’t affect to functional result. 3. There were no cases of redislocation of acromioclavicular joint after removal of Kirschner wires. 4. There were no cases of degenerative arthritis of acromioclavicular joint in final review. There result suggest that excision of the articular disc that were torn may be helpful in preventing acromioclavicular joint from degenerative arthritis.

2.A study on the effects hypothermia in the initial phase and hyperthermia in the recovery phase on ischemia-induced testicular injury.

Even though the ischemic change induced by the testicular torsion has been studied extensively, there is a few available information about the prevention or reduction of testicular injury during vascular insufficiency. We studied the effect of hypothermia on prevention of the ischemia-induced damage and the effect of hyperthermia on the recovery of the damage during re-perfusion of blood after ischemia ended. The four groups of Sprague-Dawley rats were subjected to varying duration of bilateral testicular ischemia. In the first group, both testes underwent ischemia for 2, 4 and 6 hours at room temperature. The second group underwent ischemia for 30 minutes at room temperature and then for 1.5. 3.5 and 5.5 hours at 4 degrees C ice bath. In the third group, testes underwent the procedure described in the first group and then were immersed in 41 degrees C water for 30 minutes during re-perfusion of blood. The fourth group underwent the procedure described in the second group and then were immersed in 41 degrees C water for 30 minutes during re-perfusion of blood. Bilateral orchiectomy was performed after lapse of 2 weeks. Histology of testes of all groups was compared. Neither group revealed significant destruction of germinal epithelium after two hours of ischemia. However, the preservation rates of germinal epithelium in the first group were 25% and 8.3% at four and six hours or ischemia, when compared with the control group, which were statistically significant(p<0.05). In contrast, the rates of the second group which received hypothgmia were 87.5% at four hours 85.4% at six hours of ischemia. Comparing the first group with the fourth group (hyperthermia) and the second group with the fourth group(hyperthermia), authors could not find any statistical significance(p>0.05). In conclusion, when ischemia was induced by testicular torsion. the damage of testicular tissue could be prevented or arrested to a certain degree by external ice application. However, it might be proposed that hyperthermia did not enhance the recovery of the damaged testes after re-perfusion of blood.

4.Scintigraphic evaluation of renovascular hypertension.

ACE inhibitor challenged renal scintigraphic studies offer noninvasive means of evaluating patients for renovascular hypertension, and provide help in selecting patients who will benefit most from interventional procedures designed for alleviation of renal artery stenosis. These studies provide functional assessment of each kidney which also helps the vascular surgeons to plan which renal artery to repair first, when bilateral renal arteries are stenotic, prior to an abdominal aortic aneurysm repair. Vasotec challenged Tc99mMAG3 renal scintigraphy is one of such tests with several advantages over other similar methods, and appears to have a great potential of being a preferred scintigraphic study for evaluation of renovascular hypertension.

5.Effects of Decision Making Competency, Nursing Professionalism, and Job Satisfaction on Turnover Impulse among Nurses.

PURPOSE: The study was undertaken to investigate effects of decision making competency, nursing professionalism and job satisfaction on turnover impulse, and identify factors contributing to turnover impulse among nurses. METHODS: The study was a descriptive study with 231 nurses from 4 general tertiary hospitals participating. Data were collected during March, 2013 using a structured self-report questionnaire. Decision making competency, nursing professionalism, and job satisfaction were measured with validated instruments, and turnover impulse was measured with one question using a 5-point Likert scale. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficients and multiple regression. RESULTS: The significant predictors of turnover impulse among nurses were professionalism (beta=-0.18, p=.015) and job satisfaction (beta=-0.18, p=.009). These variables explained 12% of the variance in turnover impulse among nurses. CONCLUSION: The results indicate that nursing managers need to put efforts in improving nurses' job satisfaction and to develop plans to increase nursing professionalism in order to reduce nurses' turnover impulse.

9.Preoperative Evaluation of Non-Mass-Like Enhancement on Magnetic Resonance Imaging for Measuring Tumor Extent and Affecting Surgical Margin Status in Breast Cancer Patients

Purpose:
This study investigated the correlation between non-mass-like enhancement (NME) observed on preoperative breast magnetic resonance imaging (MRI) and the actual pathological size of breast cancer. We further examined the effect of NME on the positive resection margins during partial mastectomy.
Methods:
We retrospectively collected data from breast cancer patients who underwent surgery between January 2018 and September 2020. Patients were divided into two groups based on their MRI findings: NME and no-NME (mass-like lesion only) groups. The medical records, including MRI findings and clinicopathological information of patients, were collected retrospectively, and correlations with pathologic results were analyzed. Propensity score matching was applied to develop comparable cohorts of the NME group and no-NME group.
Results:
This study included a total of 317 patients, with 66 and 251 patients in the NME and no-NME groups, respectively. The mean pathologic size of invasive lesion was significantly smaller than the mean lesion size in the NME group (1.55±1.39 cm vs. 3.45±1.81 cm, p<0.001). The mean pathologic size of ductal carcinoma in situ (DCIS) lesions was larger than that in the NME group but without statistical significance (3.91±2.67 cm vs. 3.50±1.79 cm, p=0.326). In the NME group, NME estimated DCIS size to within 1 cm in 20 patients (30.3%) and overestimated invasive lesion size by more than 1 cm in 31 patients (46.9%). NME (vs. no-NME; odds ratio [OR], 2.967; 95% confidence interval [CI], 0.878-10.025) showed a tendency to predict positive resection margins, but this was not statistically significant (p=0.080).
Conclusion
NME findings on MRI showed a similar extent of DCIS lesions. NME findings on preoperative MRI should be considered an important factor for measuring the extent of tumors, especially in DCIS patients.
10.A comparison of postoperative pain after conventional open thyroidectomy and single-incision, gasless, endoscopic transaxillary thyroidectomy: a single institute prospective study.

PURPOSE: The aim of this study was to compare postoperative pain between single-incision, gasless, endoscopic transaxillary thyroidectomy (SET), and conventional open thyroidectomy. METHODS: From March to December 2015, patients with thyroid disease underwent total thyroidectomy or lobectomy. Patient's clinical and pathological characteristics, postoperative pain score using visual analog scale (VAS) were compared between the 2 groups. The primary endpoint was postoperative pain evaluated by VAS score and postoperative analgesic use. Operation time and length of postoperative hospital stay were secondary outcome measures. RESULTS: Conventional, open cervical surgery was performed on 30 patients (group O) and SET was performed on 27 patients (group E). Pain scores in shoulder area, which is the ipsilateral side of the tumor location at 1 hour and 24 hours after surgery, were higher in group E patients (P < 0.05). Pain scores 7 days after surgery did not differ between the 2 groups according to the locations (P < 0.05). CONCLUSION: In conclusion, endocrine surgeons should be concerned about immediate higher postoperative pain scores in patients who undergo SET.
