1.A Case Report of Cervical Prolapse Complicating Pregnancy.

Uterine cervical prolapse concurrent with pregnancy is a rare situation. Large variety of management options for pregnancy, labor, and delivery to this situation had been reported. We have experienced one case of third-degree cervical prolapse combined with pregnancy. This woman visited our hospital in the late first trimester. We had managed the patient successfully with a vaginal pessary to maintain cervical placement. hereby, We report this case with a brief review.

3.Oxygen Consumption at Different Treadmill Speed and Grade in Athletes and Nonathletes.

This study was aimed to elucidate the relationship between combinations of treadmill speed-grade and oxygen consumption(Vo2). Twenty athetic and 20 non-athletic male college students aged 19-24yr were employed to exercise on a treadmill using 4 speeds(4.02, 5.47, 6.76 and 8.05km/h) and 5 grades(0, 8, 12, 16 and 20%). A fixed speed was selected for each session with the grade increased every 3 min. The Vo2, heart rate, stride frequency and stride length were measured during the last min of each 3-min stage. Vo2increased linearly with increasing speed and grade showing significant multiple correlations in nonathletes(Vo2=3.64x+0.831y+0.031xy-7.03, R=0.98, P<0.01) and athletes(Vo2=3.48x+0.324y+0.112xy-5.74, R=0.99, P<0.01). Stride frequency and length tended to increase with increasing speed except for the transition from walking to runnig at 8.05Km/h at which the stride frequency ran up much higher with the stride length getting lower than at 6.76Km/h. Heart rate increased linearly with increasing Vo2. The rate of increase was higher during walking than during running. These results indicate that athletes have higher rate of increase in Vo2than nonathletes at near-maximal exercise and may be used as a guideline in predicting maximum oxygen comsumption and in prescribing exercise intensity.

4.Expression of E-cadherin according to the Presence of High Risk Prognostic Factors, Clinical Stages and Pathologic Types in Cervical Cancer Patients Treated by Radical Hysterectomy.

OBJECTIVE: To analyze the relationship between the degree of expression of E-cadherin and presence of high risk prognostic factors (lymph node metastasis or parametrium involvement), clinical stages and pathologic types in invasive cervical cancer. METHODS: An immunohistochemical technique has been applied to formalin-fixed, paraffin- embedded samples from 20 radical hysterectomy without risk factors and 16 radical hysterectomy with risk factors. The degree of expressions of E-cadherin immunostaining was compared with the International Federation of Gynecology and Obstetrics (FIGO) stage, presence of high risk prognostic factors, and pathologic types. RESULTS: The difference of the degree of expression of E-cadherin was not statistically significant between high-risk group (lymph node metastasis or parametrium invasion) and non-risk group. The difference of the degree of expression of E-cadherin was not statistically significant according to clinical stages and variable pathologic types either. CONCLUSIONS: These results suggest that the degree of the expression of E-cadherin has no relationship with known high risk prognostic factors, clinical stages and pathologic types in invasive cervical cancer.

5.Relationship Between Degree of Spinal Canal Stenosis and Neurologic Deficits and Recovery in Thoracolumbar Burst Fractures.

STUDY DESIGN: A retrospective study was designed in 56 consecutive thoracolumbar burst fractures. OBJECTIVES: To evaluate the relationships between the spinal canal stenosis and neurologic deficits and between the amount of decompression and the neurologic recovery. SUMMARY OF LITERATURE REVIEW: Some authors have reported that spinal canal stenosis correlated with the neurologic deficit, others have reported that there was no relationship. MATERIALS AND METHODS: Using preop. and postop. CT scans of 56 burst fractures, we calculated the canal patency(CP) and the midsagittal anteroposterior diameter residue(MDR) as spinal canal stenosis. We categorized the patients with Frankel grade and scored according to the motor index score at trauma and on last follow up as neurologic status. And then, we correlated the spinal canal stenosis(CP, MDR) with the neurologic status(Frankel grade, motor index score) RESULTS: There was significant correlation between the spinal canal stenosis and the neurologic deficit at trauma. but there was no significant correlation between the amount of decompression and neurologic recovery on last follow up. CONCLUSION: The spinal canal stenosis was correlated with the neurologic deficit, but neurologic recovery might be correlated with the complex factors(ie, cord injury, biochemical change, blood supply etc.), not only the anatomical reduction.

6.Homotransplantation of Cultured Chondrocytes to prevent bony bridge formation and to help repair of the Damaged Growth Plate: An Experimental Study in a Canine tibial model

The purpose of this study was to investigate the ability of cultured chondrocytes to prevent formation of bony bridge and possibly to repair of the damaged growth plate. Growth cartilage cells were obrained from the new born canine epiphyseal plates and was culture-expanded in high density. It took 14 days until formation of micro mass of cartilage cells which was easily removable from the culture flask. Twenty dogs were divided into two groups: group I (10), the medial side of growth plate of right proximal tibia was destroyed and cultured chondrocytes were homografted into the defect: and group II (10), the medial side of growth plate of right proximal tibia was destroyed and was left as it was. Left leg was served as a control. Serial radiological and histological observation were made until 16 weeks after homografting to determine the growth parrern. Following results were obtained. 1. In group I, 8 of 10 dogs had near normal growth with little angular deformity of the tibia, averaging 8° at post-op 16 weeks. Two dogs had 20° angulation at 16 weeks post-surgery. In contrast in group II, angular deformity was obvious at 4 weeks post-surgery, reaching 31° at 16 weeks post-surgery. 2. In group II, bony bridge was consistently formed on the medial side of the proximal tibia. In group I, the cultured chondrocytes initially appeared to be an amorphous cartiagenous mass, which, however, remained to contribute to matrix formation as time went on. 3. This study showed the ability of cultrued chondrocytes to prevent formation of bony bridge and possibly to repair the damaged growth plate. To prove the effectiveness of homografting of the growth cartilage cells for reconstruction of the growth plat, further studies should be followed.

9.Fumarase Deficiency with Spastic Quadriplegia: A case report.

Fumarase catalyzes the conversion of fumarate to malate in the Krebs cycle. Fumarase deficiency is a rare inborn error of metabolism and is inherited in an autosomal recessive manner. It causes mitochondrial encephalomyopathy. The symptom is characterized by developmental delay and hypotonia. We report here a case of a 32-month-old child who was initially refered because of spastic quadriplegia, delayed development and poor feeding.

10.Congenital Coronary Arteriovenous Fistula in Adult.

Five cases of coronary arteriovenous fistula(CAVF) were diagnosed with coronary arteriography during recent 3 years in Seoul National University Hospital. In one patients, the CAVF was from right coronary artery to left ventricle at posterior aspect. In another, the CAVF was from left circumflex to left atrium. In the remaining three, it was from conal branch of left anterior descending to pulmonary artery. The correct diagnosis for the appropriate treatment of CAVF is important since the incicence of the ischemic coronary heart disease is increasing recently.
