2.Multi-slice spiral CT angiography of the posterior nutcracker syndrome

Objective To assess the value of multi-slice spiral CT angiography (MSCTA) in diagnosis of posterior nutcracker syndrome (NCS). Methods MSCTA of 15 patients clinically diagnosed as posterior NCS (patient group) and 50 subjects with normal renal vessels (control group) were retrospectively analyzed. The anatomy, course and relationship to the adjacent structure of left renal vein (LRV) and its branches were observed. The anteroposterior diameter (d1) and cross areas (s1) of the angle of control group and posterior abdominal artery (AA), the anteroposterior diameter (d2) and cross areas (s2) at the largest lumen near the renal hilar were measured and the ratio of s2/s1 and d2/ d1 were calculated.Results All LRVs posterior AA of patient group were compressed and narrowing with dilating apparently near the renal hilar. The difference of d2 was not significant (P>0.05), but of other values were significant (P<0.05).Conclusion MSCTA can show anatomy and three-dimentional trend clearly of LRV for posterior NCS, and evaluate the extent of narrow and dilating lumen accurately with cross areas measuring.
3.Advances in the study of derivatization of ginsenosides and their anti-tumor structure-activity relationship.

Ginsenosides, belonging to a group of saponins with triterpenoid dammarane skeleton, show a variety of pharmacological effects. Among them, some ginsenoside derivatives, which can be produced by acidic and alkaline hydrolysis, biotransformation and steamed process from the major ginsenosides in ginseng plant, perform stronger activities than the major primeval ginsenosides on inhibiting growth or metastasis of tumor, inducing apoptosis and differentiation of tumor and reversing multidrug resistance of tumor. Therefore ginsenoside derivatives are promising as antitumor active compounds and drugs. In this review, the derivatization methods, ginsenoside derivatives and their anti-tumor structure-activity relationship have been summarized for providing useful information for the research and development of novel antitumor drugs.
4.Regulation of omega-3 fish oil emulsion on the SIRS during the initial stage of severe acute pancreatitis.

The aim of this study was to explore the effects of parenteral supplementation with omega-3 fish oil emulsion (Omegaven) on systemic inflammatory response syndrome (SIRS) during the initial stage of severe acute pancreatitis (SAP). In a prospective, randomized and controlled trial, 60 patients with SAP were randomized either to treat with conventional therapy (Con group, n=30) or conventional therapy plus intravenous supplementation with omega-3 fish oil emulsion 0.2 g/kg every day (FO group, n=30). The effects were analyzed by the SIRS-related indexes. The results showed that APACHE-II scores in FO group were significantly lower, and the gap increased much farther after the 4th day than those in Con group (P<0.05). Fluid equilibrium time became shorter markedly in FO group than in Con group (5.1+/-2.2 days vs 8.4+/-2.3 days). In FO group, SIRS scores were markedly decreased and the SIRS state vanished after the 4th day; Plasma level of TNF-alpha was significantly reduced, while IL-10 decreased markedly, most prominently between the 4th and 7th day, and the ratio of IL-10/TNF-alpha raised as compared with Con group (P<0.05). During the initial stage of SAP, parenteral supplementation with omega-3 fish oil emulsion could efficiently lower the magnitude and persistence time of the SIRS, markedly retrieve the unbalance of the pro-/anti-inflammatory cytokines, improve severe condition of illness and may provide a new way to regulate the SIRS.

5.Effect of antisense oligodeoxynucleotide of vascular endothelial growth factor C on lymphangiogenesis and angiogenesis of pancreatic cancer.

In order to investigate the effect of antisense oligonucleotide (ASODN) of vascular endothelial growth factor C (VEGF-C) on lymphangiogenesis and angiogenesis of pancreatic cancer, antisense and scamble-sense oligonucleotide of VEGF-C were constructed, and the model of nude mice with orthotopically xenografted human pancreatic cancer cells (Panc-1) was established. Thirty nude mice were randomly divided into 3 groups: PBS control group (group A), scramble-sense control group (group B) and antisense group (group C). All nude mice were treated once every 2 days as 3 times per week, for 3 weeks (oligonucleotide 10 mg/kg every time). After treatments were completed, ELISA method was used to examine the concentration of VEGF-C in plasma and immunohistochemical method to examine microvessel density (MVD), lymphtic vessel density (LVD) of pancreatic cancer. The results showed that the expression of VEGF-C was inhibited significantly in group C. The concentrations were 237.5+/-41.5, 221.5+/-52.3 and 108.6+/-14.9 pg/mL in groups A, B and C respectively (P<0.01). LVD in groups A, B and C was 13.8+/-2.1, 12.4+/-1.9 and 4.2+/-1.6 respectively (P<0.01). MVD in groups A, B and C was 27.5+/-8.7, 25.9+/-4.2 and 19.4+/-5.6 respectively with no significant difference among the groups (P>0.05). It was suggested that VEGF-C ASODN decreased the expression levels of VEGF-C in nude mice with orthotopically xenografted human pancreatic cancer, and it could inhibit lymphangiogenesis, but had no significant effect on angiogenesis.
6.Identification of chemical constituents in qiliqiangxin capsule by UPLC-Q-TOF/MS(E).

In order to clarify the chemical constituents in Qiliqiangxin capsule, a rapid ultra-performance liquid chromatography/orthogonal acceleration time-of-flight mass spectrometry (UPLC-Q-TOF/MS(E)) method was established. Forty peaks were identified on line using this method. The herbal sources of these peaks were assigned. The results implied that triterpenoid saponins, flavonoid glycosides, C21-steroids and phenolic acids were included in the main components of Qiliqiangxin capsule. The method is simple and rapid for elucidation of the constituents of Qiliqiangxin capsule and the results are useful for the quality control of Qiliqiangxin capsule.

OBJECTIVETo establish the fingerprints of the active fractions from Tian-wang-bu-xin-wan decoction by HPLC, and to explain the role of Radix Platycodi in the decoction prescription.
METHODThe experimental conditions of the HPLC method were established as follows: Hanbon Lichrospher C18 column (4.6 mm x 250 mm, 5 microm), mobile phase were methanol and 0.012 5 mol L(-1) ammonium acetate/acetic acid buffer, eluted with a linear gradient, flow rate was 1.0 mL min(-1), the photodiode array detector (PDA) and evaporative light scattering detector (ELSD) were connected in series.
RESULTIn the same conditions and used same method, the extract amount of whole prescription was higher than that of excepting Radix Platycodi. The method provided two kinds of fingerprints with satisfied separation, which were the HPLC-PDA (Max Plot) fingerprint and the HPLC-ELSD fingerprint. And, they had good correlation and complementaritiy.
CONCLUSIONThe Radix Platycodi can enhance the decoction yield of the prescription; and increase the dissolution and the contents of some ingredients in the decoction.
