3.Effect of home-based rehabilitation on functional independence measures in patients with heroin-associated encephalopathy.

OBJECTIVE:
To investigate effect of home-based rehabilitation on performance in a functional independence measure (FIM) in patients with heroin-associated encephalopathy (HE).
METHODS:
A total of 56 patients with HE were divided into a family rehabilitation management group (treatment group, n=30) and a control group (n=26). The patients in the home-based rehabilitation management group were put on a rehabilitation program and then followed-up; those in the control group were only followed-up. Both groups were evaluated with FIM at discharge and 3, 6, and 12 months after discharge.
RESULTS:
FIM in the treatment group was remarkably better than that in the control group at 3, 6, 12 months after discharge (79.5±16.3 vs 62.3± 14.6, 88.2±20.0 vs 65.5±13.4, 99.6±21.2 vs 68.7±15.8; all P< 0.05). In the rehabilitation group, FIM of the patients receiving 3, 6, and 12 months of rehabilitation therapy was significantly higher than that at the time of discharge (P<0.01).
CONCLUSION
Home-based rehabilitation management has significant effect on the FIM in patients with HE.

4.Factors associated with retention in a community-based methadone maintenance treatment among heroin addicts.

OBJECTIVETo explore factors associated with retention in a community-based methadone maintenance treatment (MMT) among heroin addicts in Xichang of Sichuan province.
METHODSAll 206 heroin addicts were first admitted to MMT community-based program between March to September 2004. Baseline data of patients characteristics, social function, drug using behaviors, sexual behaviors, dose of methadone and retention were collected.
RESULTSUp to Oct, 2005, all 206 patients contributed 8.98 +/- 5.74 person-months of following-up. The retention rates were 58.7% after 6 months and 34.6% after 12 months respectively. Cox proportional hazard regression model indicated that the employed (HR, 0.60; 95% CI, 0.39 - 0.92), helping family to do housework in past 30 days more than once a day (HR, 0.59; 95% CI, 0.42 - 0.82) and previous self-detoxification > or = 3 times (HR, 0.65; 95% CI, 0.47 - 0.91) were independently associated with retention.
CONCLUSIONWe should give individual counseling to help heroin addicts increasing compliance.

5.Change and influence factors of craving for patients after 6 month methadone maintenance treatment.

OBJECTIVE:
To explore the change and impact factors of craving for heroin in patients after 6 month methadone maintenance treatment (MMT).
METHODS:
The questionnaire of craving for heroin was used to measure the level of craving for heroin when patients just entered the study and were treated for 6 months. The influence of MMT on craving in patients were analyzed.
RESULTS:
The total score and score for other factors decreased except the factor "self-control" after MMT. The craving for high dose patients decreased significantly after 6 month treatment (P<0.05). The degree of craving for heroin in males and females all decreased after MMT, but no significant difference was shown (P>0.05). The craving degree for heroin in patients with long drug use was higher than that of patients with short drug use. After the treatment, the improvement of craving was more significant for the long drug users.
CONCLUSION
MMT can decline the craving for heroin in drug users. Dosage for methadone and gender may be the risk factors of craving change.

OBJECTIVETo explore the factors associated with retention in a community-based methadone maintenance treatment(MMT) among drug users in Urumqi.
METHODSWith national MMT guideline( drift) for heroine addicted drug users, local heroine-dependent people were admitted to community-based MMT program affiliated to Xinjiang Uigur Autonomous Region Center for Mental Health. Data on outpatients' social-demo characteristics, baseline behaviors on drug use and daily stabilized dose of drugs were entered to MMT database.
RESULTSUp to 10, Feb. , 2006,353 persons withdrew MMT treatment among 709 heroin dependant drug users. Between 11, Aug., 2005 and 10, Feb., 2006, with median duration as 77 days, cumulative drop-out rate of 90 days and 180 days after first dose of MMT were 73.8% among 455 and 99.4% among 355 drug users, respectively. The incidence of drop-outs was 29.8 per 100 person-month. The median length of stay (days) in MMT was 68.0 (95% CI: 59.0- 78.0). Correlates of retention were found as: Being Uigur(HR = 1.35;95% CI :1.09-1.67), duration of drug use (HR =0.74; 95% CI:0.55-0.99) and stabilized dose(HR = 0.60;95% CI: 0.48-0.74) was found in multiple Cox proportional hazard regression model.
CONCLUSIONRetention of MMT among drug users in Urumqi was low. Uigur people should be given individual counseling to help them increase the compliance rate. Within the ranges of clinic dosage, adjustment of the methadone dose on an individual base might serve as an appropriate approach to increase the effectiveness of the program.

7.Determination of 24 elements in human hair by ICP-MS using microwave digestion.

OBJECTIVE:
To establish an inductively coupled plasma-mass spectrometry (ICP-MS) method for determination of 24 elements in human hair.
METHODS:
The samples were digested with microwave digestion instrument. ICP-MS was applied to determine 24 elements in human hair using indium (115In) as an internal standard. The established method was applied to determine element concentration in normal group (56 samples) and heroin abuse group (10 samples).
RESULTS:
The limits of detection ranged from 0.000 3 microg/g to 10.14 microg/g. Measured value of the standard materials were basically consistent with the standard value. The contents of magnesium, gallium and barium in hair of heroin addicts decreased after rehabilitation treatment.
CONCLUSION
The method is sensitive and accurate for determination of 24 elements in human hair.
