1.Clinical Analysis of Femoral Hernia in Adult.

PURPOSE: Compared with other types of hernia, femoral hernias in adults are less common and we have had little literature about femoral hernia. The purpose of this study was to evaluate the incidence of femoral hernias in adults and to analyze the characteristics of femoral hernias. METHODS: This retrospective study was based on the medical records of 60 patients who underwent herniorrhaphies for treating femoral hernia in adults from January 2001 to December 2007. We analyzed the incidence of femora hernias, the symptoms, the operative techniques, the complications, the mortality and the postoperative recurrence rates. RESULTS: The incidence of femoral hernia was 4.6% in all cases of inguinal hernias. The incidence in females was about 4 times higher than in males (P<0.001). The most common symptom was bulging. The operative technique was changed from plug technique to bilayer technique. Seroma including swelling and pain were prominent complications. We experienced one case of mortality. CONCLUSION: Femoral hernia is a rare type of inguinal hernia in adult. Femoral hernia is more frequent in females and in the aged; it is an important surgical pathology with high rate of incarceration or strangulation. In consideration of the pathology of hernia and difficulty of the peri-operative diagnosis, the Prolene Hernia System herniorrhaphy is a superior method to other methods in femoral hernia.

4.The use of freeze-dry bovine pericardium (FDBP) in diaphragmatic herniorrhaphy in dogs.

A study was conducted to investigate the effectiveness of freeze-dried bovine pericardium (FDBP) as a biomaterial in diaphragmatic herniorrhapy in dogs. Eight adult dogs were randomly selected and divided into two equal groups. In FDBP group, a diaphragmatic defect was induced and repaired with an identical size of FDBP. In the control group, a diaphragmatic wall was incised at three-side border creating a flap and sutured. Grossly, only mild intrathoracic adhesion was observed for most of the animals, and no herniation occured. Microscopically, the biomaterial incorporated into the host's tissue by ingrowth of young muscle fiber and massive new blood vessel formation in between the fibrous tissue.

5.Giant Meckel's Diverticulum Associated with a Congenital Diaphragmatic Hernia.

Giant Meckel's diverticulum is a very rare lesion and its association with a congenital diaphragmatic hernia has not been reported previously. We report a case of newborn with a giant Meckel's diverticulum and congenital diaphragmatic hernia. A large round atypical air-filled bowel segment was found by chest radiography preoperatively, and a giant Meckel's diverticulum was located within the left hemithorax during surgery.

7.Surgical treatment for different forms of hernias in sheep and goats.

Sheep and goats are frequently presented with different forms of hernias to veterinary clinics. The aim of this study is to investigate the outcome of the surgical treatment of abdominal, umbilical, inguinal and scrotal hernias in sheep and goats. Fifty-eight clinical cases (sheep = 44, goat = 14) were presented to the Veterinary Teaching Hospital, College of Agriculture and Veterinary Medicine, Qassim University, Saudi Arabia from September, 2003 to September, 2006. These animals had abdominal (sheep = 30, goat = 10), umbilical (sheep = 6, goat = 4), inguinal (sheep = 7) and scrotal (sheep = 1) hernias. All the cases of hernias in sheep and goats were subjected to full study including the history of the case, classification of hernias, the size of the hernial ring, surgical repair of the hernias, adhesions between the hernial sacs in each case, the postoperative care and follow up of the cases. The results revealed that gender had an effect on the incidence of hernia. The incidence of abdominal hernias was higher in females and the incidence of inguinal hernia was higher in males. There was a positive correlation between the history of hernia and the degree of adhesion. For the sheep, 26 out of 30 cases of abdominal hernia had good outcomes and the healing was excellent. There were postoperative complications in 4 ewes. For the goats, there were slight swellings at the site of operation in 2 out of 10 cases of abdominal hernia, while the remaining 8 cases had good outcomes. There was one case of umbilical hernia with an umbilical abscess that had broken down with sepsis formation at the surgical site. In conclusion, the success rates of surgical treatment for all types of hernias were very high and there were no significant differences in the success rates among the different types of hernias in both sheep and goats. The types of suture materials and the types of hernias had no significant effect on the outcome of the surgical treatment.

8.Traumatic Abdominal Wall Hernia.

A traumatic abdominal wall hernia (TAWH) is a rare type of hernia, which follows blunt trauma to the abdomen, where disruption of the musculature and fascia occurs, with the overlying skin remaining intact. The case of a sixty five year old female that developed a TAWH, following the collapse of the roof of her house, is reported. She underwent a laparotomy for suspected liver injury, followed by repair of the hernia using a fascia lata graft taken from the thigh. The etiology, pathogenesis and management of this rare hernia are discussed.

9.Left Paraduodenal Hernia Presenting with Atypical Symptoms.

Paraduodenal hernias are a rare congenital malformation, but they are the most common internal hernias. They develop secondary to a failure in midgut rotation, which may lead to small bowel obstruction or other clinical manifestations. The authors recently experienced a case of a left paraduodenal hernia presenting with unusual symptoms of left flank pain and vomiting.

10.Polyorchidism.

Polyorchidism is an uncommon congenital anomaly, which is defined as the presence of more than two testes, and may be associated with urological pathologies, such as an undescended testis, inguinal hernia, testicular torsion and testicular tumor. In the absence of any concomitant disorder, and if testicular tumor can be ruled out, using magnetic resonance imaging and ultrasonography, then a surgical exploration or biopsy is unnecessary. We report a case of polyorchidism in a 3 year-old-boy, with three testes in the right scrotum.
