1.Effects of transcranial magnetic stimulation on the recovery of neurological function and the expression of neurofilament protein in rats with cerebral infarction

Objective To investigate the effects of transcranial magnetic stimulation(TMS)on the recovery of neurological function and the expression of neurofilament protein(NFP)-200 in rats with cerebral infarction.Methods The string-fastening method was used to establish focal ischemia-reperfusion models in 60 Sprague-Dawley rats and the rats were divided into the TMS group and the sham TMS group(composed of 30 rats each).Then each group were divided into the treated 1 d,3 d,7 d,14 d and 21 d subgroups(composed of 6 rats each).TMS group rats were administered with 2 sessions of TMS(200 pulses total)daily,keeping on corresponding days in different groups.The recovery of neurological function and the optical density of NFP-200 by immunohistochemistry in the infarct peripheral zone in two groups were detected after therapy.Results At the 14 d and 21 d after treatment,neurological deficit scores of TMS group(2.67?0.82,1.50?0.55) were significantly lower than those of the sham TMS group(3.67?0.52,3.17?0.75)(P
2.Diagnosis and treatment of delayed rupture of the spleen: analysis of 32 cases

Objective To explore effective methods of diagnosis and treatment for delayed rupture of the spleen (DRS) through retrospective analysis of 32 cases. Methods A retrospective study was done in 32 cases with DRS. The effects of CT, ultrasound and diagnostic peritoneal lavage on diagnosis of DRS was observed and compared. Results Splenectomy was performed in 26 cases, among which one died four days after splenectomy because of complicated brain injury and six given conservative treatment under continuous monitoring discharged. The mean hospitalization duration was 18.5 days. Conclusions Diagnostic peritoneal lavage can improve the positive rate of peritoneal puncture. Ultrasound is an important method for diagnosing DRS and closely monitoring the patients receiving conservative treatment.
3.Effects of berberine on fibronectin and p38MAPK signal pathway in rat glomerular mesangial cells cultured under high glucose condition

Aim To investigate the effects of berberine on extracellular matrix in rat glomerular mesangial cells cultured under high glucose and its possible mechanism.Methods Six groups were divided according to the different experimental conditions:① Normal glucose group (NG);② Mannitol group (Mannitol);③ High glucose group (HG);④ SB203580 treatment group (HG+SB203580);⑤ Berberine low dosagegroup (HG+BBR 30 ?mol?L-1);⑥ Berberine high dosage group (HG+BBR 90 ?mol?L-1). The phospho-p38MAPK,phospho-cAMP response element binding protein (CREB) and fibronectin were detected by Western blot analysis. Results Berberine obviously decreased protein expression of fibronectin compared with high glucose group. Phospho-p38MAPK and phospho-CREB level was significantly lowered compared with that of high glucose group. Conclusion Berberine can inhibit extracellular matrix accumulation including fibronectin via p38MAPK signal pathway in treatment of diabetic nephropathy.
4.Anti-exprimental atherosclerosis effects of improved prescription of didang-tang (IPDT) in rabbits and its mechanism

AIM To observe the effects of improved prescription of Didang-tang (IPDT) on exprimental atherosclerosis(AS) in rabbits and study the possible mechanism. METHODS Lipid plaque area of aortic endothelium was measured by image analysis method. lipid metabolism was measured by enzyme dynamics method, one-step method and immunization photoextinction. Plasma SOD and MDA was measured by microcontent fast detecting method and modified Ba MuGuoFu method. Ceramide was measured by thinlayer scannig; apoptosis of foam cell of aorta lipid plaque was measured by TUNEL method.RESULTS IPDT reduced lipid plaque area of aortic endothelium,regulated lipid metabolism, improved antioxidation of organism, ced ceramide content of aorta lipid plaque and apoptosis of foam cell.CONCLUSION IPDT has good effects of anti-exprimental AS,the possible mechanism maybe related to regulation of lipid metabolism and antioxidation and depression of apoptosis of foam cell.
5.Practice and thinking of assessment scale design for simulated clinical skills examination

This article described systematically the design basis,passing standard and influential factor of test score of the assessment scale of simulated clinical skills examination.Through developing and modifying OSCE examination scale design for many years,we have found that the use of 360-degree assessment for designing OSCE assessment rating scale can effectively reduce the impact on the score of examinee owing to the environment,the candidates,raters and question design,which has an important role for scientific development of OSCE assessment scales,as well as improving the reliability and validity for development of a scientific OSCE assessment.
6.Interventional effect of emodin on the changes of sphingomylinase activity and ceramide content in experimental rabbit with atherosclerosis

BACKGROUND: It is reported that ceramide signal pathway may play an important role in the apoptosis of vascular endothelial cell and then lead to the progress of atherosclerosis, such as the formation of foam cells and the proliferation of smooth muscular cells.OBJECTIVE: To study the changes of the sphingomylinase activity and ceramide content in aorta of rabbits with experimental atherosclerosis and investigate the regulative effects and mechanism of emodin on them as compared with positive fenofibrate.DESIGN: Completely randomized controlled design.SETTING: Laboratory of Pharmacology & Toxicology, School of Pharmaceutical Science of Sun Yat-sen University.MATERIALS: The experiment was completed at the Laboratory of Pharmacology & Toxicology, School of Pharmaceutical Science of Sun Yat-sen University from July to December 2003. Totally 48 New Zealand male rabbits were selected. Forty animal models of atherosclerosis were made with high cholesterol feed, and the other 8 rabbits were selected as the normal controls. Model animals were divided randomly into model group, 5 mg/kg emodin group, 10 mg/kg emodin group, 20 mg/kg emodin group and 25mg/kg fenofibrate group with 8 in each group.METHODS: At the seventh weeks of model duplication, 5 mg/kg, 10 mg/kg and 20 mg/kg emodin were perfused in rabbits of emodin groups respectively, and 25 mg/kg fenofibrate was perfused in rabbits of fenofibrate group. Emodin and fenofibrate were diluted or suspensed with 2 mL saline once per day respectively. Rabbits in normal control group and model group were administrated with the same volume of saline for 4 weeks. The rabbits were raised separately and were fed with 135-150 g food per day.MAIN OUTCOME MEASURES: [1] The area of the lipid plaque in aortal intima; [2] the content of serum TC and TG; [3] SOD activity and MDA content; [4] SMase activity and CER content in aorta.RESULTS: Totally 48 rabbits entered the final analysis. [1] Area of the lipid plaque: Area of the lipid plaque was (48.87±15.5) % in the model group, which was larger than that in each emodin group (P < 0.05 or 0.01),especially larger than that in the 10 mg/kg emodin group (22.19±12.9)%while that in the fenofibrate group was similar to that in the model group (P > 0.05). [2] Content of serum TC and TG: The anrtal intima of control was smooth. Content of serum TC and TG in each emodin group were similar to those in the model group (P > 0.05), but those in the 25 mg/kg fenofibrate group were lower than those in the model group (P < 0.05). [3]Content of SOD and MDA in plasma: SOD activity of rabbits in each emodin group was higher than that in the model group (P < 0.05, P < 0.01),but the MDA activity in the 10 mg/kg and 20 mg/kg emodin group was lower than that in the model group (P < 0.05). The MDA activity in the25 mg/kg fenofibrate group was similar to that in the model group (P > 0.05).[4] Content of SMase and CER: Those in the model group were higher than those in the normal control group, but those in the 5, 10 and 20 mg/kg emodin groups were lower than those in the model group; those in the 25 mg/kgfenofibrate group were similar to those in the model group (P > 0.05). [5]Analysis of correlation: Content of SMase was in positive relation with blood cholesterol (r=0.542, P < 0.01), in positive relation with blood MDA (r=0.789, P > 0.01), and in negative relation with blood SOD(r=-0.936, P > 0.01); content of CER was in positive relation with blood cholesterol (r=0.433, P > 0.05), in positive relation with blood MDA (r=0.673, P > 0.01), and in negative relation with blood SOD (r=-0.876, P > 0.01).CONCLUSION: The study finds that emodin, despite its insignificant effects on decreasing TG or TC, can protect vascular endothelial cells and reduce the area of lipid-laden plague of aortal intima by antioxidation, inhibition of the sphingomyelinase activity and reduction of the content of ceramide. It is suggested that moderate dosage of emodin employed in the study may be most appropriate to atherosclerosis treatment.
7.Application of PDCA circle model in medical college students' clinical practice training and evaluation

In order to strengthen the management of clinical teaching, improve the teaching quality of clinical practice, the advantages of the traditional medical education, teaching reform and medical simulation were further integrated, using PDCA (Plan-Do-Check-Action) cycle model in medi-cal college students' clinical practice training and evaluation. The skills of training and evaluation were optimized and practiced by thechain model of teaching-assistance-guidance to improve the train-ing quality of interns, and make continuous improvement from the angle of teaching, learning and management and with PDCA circulation mode. Practice shows that the application of PDCA circulation management has greatly improved teachers' teaching quality and their own quality, and the students' satisfaction of practice teaching management and clinical teachers has also significantly increased. The effect is obvious.
8.Oligodendrocyte Degeneration and Alzheimer's Disease

Alzhelmer's disease (AD) is an important neurodegenerative disease. Recent evidence has indicated that the production and loss of the myelin,sheath are associated with AD because the particular vulnerability of oligodendrocytes produced in the late stage makes the loss of the myelin sheath take a core position in the changes of the earliest stage of AD. The loss of the myelin sheath disrupts synchronization of impulses on which normal brain functions highly depend, and ultimately results in the function disruption of cortical association regions and subsequent neuronal loss. Meanwfiile, there are diverse mechanisms that make oligodendrocytes degeneration exist in the brains of AD. Therefore, elucidating its specific mechanism may help better understanding of AD, and thus provide some help for its treatment.
9.Renal vascular angiography with interpolated finite impulse response with 1.5T MR

Objective To evaluate the feasibility of renal artery angiography with interpolated finite impulse response (IRFR) sequence on 1.5T MR. Methods A total of 122 subjects underwent MR scan with IFIR sequence. On axial image of IFIR, bilateral signal-to-noise ratio (SNR) and breath artifact of renal artery were measured and 3D maximum intensity projection (MIP) images were reconstructed on workstation ADW 4.3, and each branch was scored according to its displaying quality. Influence of SNR, age and breath artifact on the displaying of renal artery were compared and analyzed. Results Renal segmental artery could be seen in 112 subjects (91.80%). Among them, segmental artery was showed clearly in 78 subjects (63.93%), post-segmental artery was showed in 54 subjects (44.26%) and branches of post-segmental artery was showed in 22 subjects (18.03%). There was statistical difference of renal artery scores mean rank between ≥60 years group and <60 years group (49.25 vs 67.48, P=0.006), as well as between the group without and with breath artifact (66.66 vs 43.35, P=0.002). No statistical difference of SNR was found among renal artery scores from 1-5 (P=0.177). Conclusion IFIR sequence is simple and safe for renal artery angiography on 1.5T MR, and different grades of renal artery branch can be showed clearly.
