1.Influence of LTC_4 on the microcirculation of the gastric mucosa and its effect on acute gastric mucosal injury

The author explored the influence of exogenous LTC_4 (0.1 ng. 100g~(-1),iv) on the microcirculation of gastric mucosa and its effect on acute gestric mucosal injury. Male Wistar rats underwent a fast of 24 hours before the experiment and could drink water freely during this period. The results showed: 1. The caliber of the gastric mucosal superficial capillaries thinned and part of the capillaries disappeared. The caliber of collecting venule thinned also. After 10 minutes there were remarkable venous stasis and heamorrhage. 2. It was confirmed by the method of hydrogen clearence that the gastric mucosal flow reduced significantly. 2.Screening for congenital syphilis with TPPA and TRUST in 7800 neonates

Objective To assess the value and clinical applicability of non-treponemal antigen serologic test and Treponema pallidum antigen serologic test in the diagnosis of congenital syphilis. Methods Totally,7800 newborns were collected from the out-patient and in-patient department. Toluidine red unheated serum test (TRUST) was conducted to quantitatively detect non-treponemal antibodies and Treponema pallidum particle agglutination assay (TPPA) to qualitatively detect specific treponemal antibodies in these subjects. The results of first serology test for syphilis were assessed for neonates with confirmed congenital syphilis. The sensitivity and specificity in the diagnosis of congenital syphilis were compared between TPPA and TRUST.Results Of the 7800 newborns undergoing syphilis serology testa, 60 were positive for TPPA and TRUST,including 15 cases of congenital syphilis, 47 were positive for TPPA but negative for TRUST, including 5 cases of congenital syphilis. The sensitivity and specificity were 75% (5/20) and 99.41% (7734/7780) respectively for TRUST in the diagnosis of congenital syphilis, 100% (20/20) and 98.88% (7693/7780) respectively for TPPA. Conclusion TPPA seems more applicable than TRUST to the screening for congenital syphilis.
3.Hereditary spastic paraplegia with SPG30 mutation: A report from North East China

Hereditary spastic paraplegia is a heterogeneous group of genetic neurodegenerative disorders of the
nervous system. It is classified into four subtypes based on the mode of inheritance; and among them,
most autosomal recessive hereditary spastic paraplegia cases are due to type SPG11 and SPG15 gene
mutations. Autosomal recessive hereditary spastic paraplegia cases with SPG30 gene mutation have
never been reported in China. Herein, we present our experience with a case of hereditary spastic
paraplegia with SPG30 gene mutation in our hospital from North East China. In this patient we detected
a missense mutation of c.499 C>T (p.Arg167Cys) in gene KIF1A, a causative gene of type SPG30.

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leucoencephalopathy(CADASIL)is a common hereditary disease caused by NOTCH3 gene. The major clinical manifestations includerecurrent small-vessel ischaemic events, migraine, dementia and mood disturbance. Herein, wereport a 32-years-old male presented with right leg weakness and persistent migraine. We carried outneurological exams, genetic testing, blood and cerebrospinal fluid analysis (CSF) as well as magneticresonance imaging (MRI) for the brain and spinal cord. There were no anti-aquaporin-4 antibodiesand oligoclonal bands in the CSF and blood investigations were within the normal range. MRI scansrevealed multiple hyperintense regions in the brain and longitudinally hyperintense signal in spinal cord.Further, we identified a c.383G>A(p.Cys128Tyr) mutation in NOTCH3 gene. Therefore, the patientwas diagnosed with CADASIL concurrent with spinal cord lesion. The patient’s condition slightlyimproved after two weeks treatment with daily dosage of 0.5 g citicoline and 75 mg clopidogrel.
5.Study on the predictive value of preoperative peripheral blood inflammatory related indexes in Fuhrman grade of clear cell renal cell carcinoma

Objective To explore the predictive value of systemic immune inflammatory index(SII),systemic inflammatory response index(SIRI),neutrophil to lymphocyte ratio(NLR),lymphocyte to monocyte ratio(LMR),platelet to lymphocyte radio(PLR),albumin to globulin(AGR)and heat shock protein 90α(HSP90α)in Fuhrman grade of clear cell renal cell carcinoma(ccRCC).Methods From October 2019 to August 2022,212 patients who underwent surgical treatment for ccRCC were divided into low-grade tumor group and high-grade tumor group according to Fuhrman grade.The independent influencing factors of Fuhrman grading were determined by univariate and multivariate Logistic regression analysis,and the predictive value of each inflammatory index to Fuhrman grading was evaluated by drawing Receiver operating characteristic curve(ROC).We constructed the line chart prediction model and evaluated the effectiveness of the model.Results The preoperative levels of SII,PLR,AGR,HSP90α and the maximum diameter of tumor were significantly different between high-grade group and low-grade group(P<0.05).Logistic regression analysis showed that the maximum diameter of tumor,PLR,AGR and HSP90α were independent influencing factors of Furhman grade.By drawing the ROC curve,it was found that the area under the curve(AUC)of PLR,AGR and HSP90α to predict Furhman grade were 0.641,0.675 and 0.696.In addition,the Furhman grade line chart prediction model had good prediction ability,the AUC was 0.789(95%CI:0.717~0.862),the sensitivity was 61.80%,and the specificity was 85.40%.Conclusions There was a significant correlation between inflammation-related indexes in peripheral blood and Furhman grade of clear cell renal cell carcinoma.The Furhman grade line chart prediction model based on the maximum diameter of tumor and peripheral blood inflammation index has good predictive ability.

Post-stroke depression often seriously affects the prognosis and quality of life of patients and many
clinical trials had shown that Chai Hu Shu Gan San (柴胡疏肝散) combined with selective serotonin
reuptake inhibitors (SSRIs) had good efficacy and minor side effects. We aimed to conduct this metaanalysis to evaluate the efficacy and safety of Chai Hu Shu Gan San as an adjuvant drug for SSRI in
treating post-stroke depression. We searched PubMed, EMBASE, Cochrane Library, Wanfang, China
Biology Medicine disc (CBM), Chongqing VIP, and CNKI (China National Knowledge Infrastructure)
from their date of foundation to December 15, 2018. Literature screening, data extraction and quality
assessment were conducted by two authors independently. The data synthesis and analysis were
performed by using Review Manager (RevMan) 5.3 software and sensitivity analysis was conducted
to assess the robustness of the results. Finally, a total of 22 articles were included. The meta-analysis
confirmed the advantages of the combination of SSRI and Chai Hu Shu Gan San, mainly from four
aspects: the Hamilton Depression (HAMD) scale score (MD=3.66; 95% DI=2.33-4.98; p<0.001),
the Modified Edinburgh Scandinavian Stroke Scale (MESSS) score (MD=4.87; 95% CI=2.32-7.43;
p<0.001), the efficacy rate (OR=3.50; 95% CI =2.61-4.69; p<0.001) and the incidence of adverse
reactions (OR=0.28; 95% CI=0.17-0.46; p<0.001). No significant publication bias was observed, and
sensitivity analysis suggested a good stability of the results. According to the present evidence, we
