1.Diagnosis and Prediction of Clinical Outcomes in Patients with Acute Lacunar Infarction: Usefulness of Perfusion MR Imaging.

PURPOSE: To correlate the findings of perfusion-weighted imaging (PWI) with clinical outcomes in patients with acute lacunar infarction. MATERIALS AND METHODS: Eleven patients (7 males and 4 females) with acute lacunar infarction who were examined within 50 (mean, 29) hours of the onset of symptoms underwent conventional MRI, diffusion-weighted imaging (DWI) and PWI. Gadolinium (0.2 mmol/kg) was injected at a rate of 2 ml/sec, and PWI was performed using a gradient-echo EPI pulse sequence and the following parameters: TR/TE, 2000/60; flip angle, 90 degree; matrix size, 128X128. Relative cerebral blood volume (rCBV) maps were derived from gadolinium bolus perfusion-weighted images where rCBV ratios between infarcted areas were detected by DWI, and contralateral control areas were obtained. In each case, the resulting rCBV ratio at a lesion site was compared with the clinical outcome determined on the basis of the difference between National Institute Health Stroke Scale (NIHSS) scores at admission and discharge. RESULTS: With the aid of the time-intensity curve obtained at PWI, the rCBV maps revealed a hypoperfused area in 10 of 11 patients, and there was positive correlation (r=0.81) with clinical outcome. CONCLUSION: Although PWI has a lower detection rate than DWI, it may be a useful modality for helping determine prognosis in cases of acute lacunar infarction.

2.Usefulness of Apparent Diffusion Coefficient in Ovarian Cystic Tumors Using Diffusion-Weighted Magnetic Resonance Imaging.

PURPOSE: To evaluate the usefulness of apparent diffusion coefficient (ADC) values using diffusion-weighted magnetic resonance imaging (DWI) in patients with ovarian cystic tumors. MATERIALS AND METHODS: During past 12 months, we studied 30 patients who were clinically suspected of having ovarian cystic tumors and who underwent DWI using a 1.5 T MR unit. Eight patients with small cystic ovarian lesions of less than 3 cm in diameter and insufficient DWI were excluded from the calculation of the ADC values. The remaining twenty-six cystic ovarian lesions in 22 patients were classified into four groups; ovarian cysts, cystadenomas, other benign tumors, and malignant tumors. DWI was obtained using single-shot spin echo planar imaging and two gradient steps (b values of 0, 800 sec/mm2). The ADC values were measured using regions-of-interest (ROI) in the cystic components of the DWI located in the same section as the T2-weighted image and away from the septation and solid components. RESULTS: The mean ADC values were 0.196+/-0.105x10(-3) mm2/sec in the ovarian cysts, 1.312+/-1.064x10(-3) mm2/sec in the cystadenomas, 0.274+/-0.124x10(-3) mm2/sec in the other benign tumors, and 1.011+/-0.080x10(-3) mm2/sec in the malignant tumors. The differences in the ADC values between the ovarian cysts and cystadenomas, the ovarian cysts and malignant tumors, the cystadenomas and other benign tumors, and the other benign tumors and malignant tumors were statistically significant (p < 0.01). There was no statistically significant difference in the ADC values between the ovarian cysts and other benign tumors, or between the cystadenomas and malignant tumors (p > 0.05). CONCLUSION: The calculated ADC values using DWI should be helpful in the differential diagnosis of cystic ovarian tumors.

3.A Case of Gastric Lipoma with Hemorrhage.

Gastric lipoma is rare submucosal tumor, accounting for less than 3% of all be- nign gastric tumor. Most are usually asymptomatic, but on occasion, they may present with abdominal pain, obstruction, dyspepsia, intussuception and gastrointestinal bleeding. Surgical resection is definitive diagnostic and therapeutic procedure. Surgical removal of gastric lipoma should be considered in the following situations: 1) the lesion is large, 2) the lesion is difficult to differentiate from malignant tumor, 3) the patient is symptomatic or has recurrent bleeding or obstruction. We report a case of gastric lipoma with bleeding in a 67-year-old male. Gastroscopy showed active gastric ulcer with fresh blood clot. Although medical conservative treatment was done, bleeding was continued. We referred patient to general surgical department for open surgical procedure and subtotal gastrectomy was performed. Histopathological examination of surgical gastric segment showed 5 5.5 cm sized ulcerated mass. Microscopic finding of cross section showed uniform and mature adipose cell, finding consistent with lipoma. We confirmed it submucosal gastric lipoma with ulcer bleeding.
