1.Intramuscular gentamicin improves the efficacy of ciprofloxacin as an antibiotic prophylaxis for transrectal prostate biopsy.

INTRODUCTIONInfection-related complications after transrectal ultrasound guided prostatic biopsy (TRPB) could be life threatening. Our centre observed sepsis after TRPB despite prophylactic oral ciprofloxacin. We reviewed all cases of post-TRPB sepsis with their bacteriology and evaluated if the addition of intramuscular (I/M) gentamicin to standard prophylaxis before TRPB could reduce its incidence.
MATERIALS AND METHODSIn a single urological centre, we performed an interventional study that compared a prospective group with retrospective control. The latter is known as the "cipro-only" group included consecutive patients who underwent TRPB between 1 September 2003 and 31 August 2004. The addition of I/M gentamicin 80 mg half an hour before TRPB started on 1 September 2004. All subsequent patients who underwent TRPB until 31 August 2005 were included in the "cipro+genta" group. Patients who did not receive the studied antibiotics were excluded.
RESULTSThere were 374 patients in the "cipro+genta" group and 367 patients in the "cipro-only" group with comparable profiles. There were 12 cases of post-TRPB sepsis in the "cipro-only" group and 5 cases in the "cipro+genta" group. Ciprofloxacin-resistant Escherichia coli (E. coli) was the only pathogen isolated in both groups. In the "cipro-only" group, 9 patients had positive blood cultures and 8 were sensitive to gentamicin. In the "cipro+genta" group, the only positive E. coli was gentamicin-resistant. One patient in the "cipro+genta" group was admitted to the intensive care unit with septicaemia.
CONCLUSIONThe addition of I/M gentamicin to oral ciprofloxacin is a safe and effective prophylactic antibiotic regime in reducing the incidence of post-TRPB sepsis.

2.Enhancements of Mouse Hepatic Cytosol Enzyme Activities Involved in UDP-Glucuronic Acid Synthesis, Glutathione Reduction and Conjugation with Butylated Hydroxyanisole (BHA) and Its Structural Analogs.

Activities of hepatic cytosol enzymes involved in UDP-g1ucuronic acid synthesis as well as in glutathione reduction and conjugation systems were determined following administrations of butylated hydroxyanisole (approximately 5 mmol/kg body weight/day) and of equimolar intake doses of its structural anglogs. These compounds included the multi-functional group side chain compounds (t-butyl hydroquinone, 4-hydroxy- anisole, hydroquinone, benzoquinone) and the mono-functional side chain compounds (t-butyl benzene, anisole, phenol). They were administered to mice for 10 days either by mixing them in the diet or by oral intubations. Results showed that glutathione Stransferase activities were markedly increased by all tested compounds except for the t-butyl benzene. Activities of glutathione reductase and glucose 6-phosphate dehydrogenase were increased together on1y by BHA and t-butyl hydroguinone. UDP-glucose dehydrogenase and NADH:quinone reductase activities were significantly elevated by the multi-functional side chain compounds, but not by the mono-functional analogs. The relations between chemical structures of tested BHA analogs and elevations of the measured hepatic cytosol conjugation (detoxification) system enzyme activities for the metabolism and excretion of BHA analogs are discussed.

3.The Impact of Dry Needling on Patients with Neck Pain in a Tertiary Hospital

Neck pain presents as a symptom of dull pain or discomfort mainly along the trapezius muscle. Dry needling is an
invasive procedure which uses acupuncture needle directed at myofascial trigger points. The aim of the study was to
evaluate the effectiveness of dry needling in managing patients with neck pain. A pre-test-post-test interventional
study design was used. Patient education package was provided to 32 respondents who fulfilled the inclusion criteria.
A 13-item Pain Catastrophizing Scale (PCS) questionnaire was used to assess Rumination, Magnification and
Helplessness. Subjective pain intensity was measured by Visual Analogue Scale (VAS). These questionnaires were
given before and after the dry needling intervention. The findings reported that respondents scored high in pre-test
total PCS score (27.41±13.652). Post-test result revealed a significant improvement in total PCS score
(23.06±13.938) (p = 0.000). Post-test VAS score (4.78±1.237) was also significantly better than pre-test
(6.47±1.414) (p = 0.000). There was no significant difference in pre-test PCS in terms of marital status (p > 0.05)
whereas there was significant difference between marital status and rumination in post-test (Z = -2.303, p = 0.021).
There was significant difference between pre-test magnification in terms of respondents’ occupation (p = 0.008) and
race (p = 0.035) but no significant difference in post-test. Respondents’ age group showed no significant differences
between pre-test and post-test PCS and VAS (p > 0.05). In conclusion, patients who received dry needling showed
improvement in pain intensity and catastrophizing towards neck pain.
