1.Imaging Manifestations of Pneumocystis Carinii Pneumonia in AIDS(A Report of 18 Cases)

Objective To study the X-ray and CT findings of pneumocystis carinii pneumonia(PCP)in AIDS.Methods 18 cases with PCP in AIDS proved by clinic and pathologically were collected.HIV antibody was positive in all cases.Chest radigraphy and CT scan were taken in all cases.The imaging findings were analysed.Results The typical features of PCP on radiography and CT included unsymmetric diffuse mesh or reticulo-nodular changes,symmetric ground-glass opacification and many more even lobar consolidations which distributed in bilateral hilar regions and middle-lower lung fields.The atypical manifestations included cystic lesions,lobar consolidation,hilar and/or mediastinal adenopathy,pleural effusion,pneumothorax,atelectasis,mass and cavity-mass.Conclusion Plain chest film and CT scan are of important value in diagnosis of PCP.It is possible to diagnose PCP in patients with typical imaging manifestations positive HIV and in combination with clinical data.
2.CT Features of Brain Lesions in AIDS

Objective To discuss the CT characteristics of AIDS's brain common disease,so that to improve the diagnostic level.Methods CT features of brain lesions in 25 patients with AIDS were retrospectively analyzed.Results Acute HIV encephalitis(n=6),CT features included low density at the basis festival area and the ventriculi laterals,no enhancement on contrast-enhanced scan in 1 case.Subacute HIV brain illness(n=5),CT images showed different degree of atrophy.Toxoplasmosis(n=4),CT images showed iso-dense tubercles with edema in the deep part of brain white matter or iso-dense circle at the edema area.Cytomegalovirus encephalitis(n=3),CT images showed symmetrical low density at bilateral cerebral occipital lobe,internal capsule and thalamus.Progressive multifocal leukoencepathy(n=3),CT showed multiple low density at the inferior surface of brain cortex,the gyri close of focus was clearer,and far of tapetum room system.Mixed infectious(n=4),mixed CT characteristics.Conclusion CT is beneficial in the differential diagnosis of brain disease in AIDS.
3.Isolation and antifungal resistance analysis of 10 strains of Candida auris

Objective:Analyze the isolation and antimicrobial resistance of Candida auris.Methods:Using matrix-assisted laser desorption ionization time-of-flight mass spectrometry, 10 clinically isolated strains of Candida auris were identified in Nanfang Hospital of Southern Medical University between September and December 2023. Additionally, their resistance to multiple antifungal drugs, specimen source, and patient prognosis were analyzed. Results:All 10 isolates were identified as Candida auris by mass spectrometry. The drug sensitivity test showed that 10 strains were resistant to fluconazole, 3 strains were resistant to amphotericin B, and all were sensitive to Echinocandins. The samples came from various sources; except for 2 patients who were in good condition, 1 patient with stable condition were discharged, and 1 patient died, the remaining 6 patients voluntarily discharged due to worsening of their condition. Conclusions:Candida auris can be identified by mass spectrometry. It was found that Candida auris had high resistance to fluconazole and no resistance to echinocanthin.
4.Identification and antibiotics sensitivity analysis of Herbaspirillum huttiense in catheter-related bloodstream infection

Objective:To analyze the identification and antibiotics susceptibility of Herbaspirillum in catheter-related bloodstream infection, and improve the awareness and attention of the rare pathogenic microorganisms for clinicians and microbiologists. Methods:The bacterium was isolated from a positive blood culture of a hemodialysis patient with chronic renal failure. The smear of isolate was prepared, stained and observed by microscope. The single colonies were identified by mass spectrometry and VITEK 2 Compact identification and antibiotics sensitivity analysis system, respectively. Then, 16S ribosomal DNA (rDNA) was amplified and sequenced, and bacterial genome was sequenced.Results:The gram-negative bacilli was found in the positive blood culture bottle. After incubated on blood agar for 16 hours, milky white, bulging and non-haemolytic colonies were observed. The identification result was Burkholderia cepacian by VITEK 2_Compact and antimicrobial susceptibility testing showed resistance to aztreonam and polymyxin but sensitive to other drugs in N335 card. The isolate could not be identified by VITEK MS with clinical database. However, it was identified as Herbaspirillum huttiense/Herbaspirillum aquaticum with research database. The 16S rDNA of the strain was consistent with Herbaspirillum huttiense and Herbaspirillum aquaticum (more than 99%). High-throughput bacterial genome sequencing revealed that the isolate in this case shared 100% homology with Herbaspirillum huttiense subsp putei IAM 15032 in Genbank database, which confirmed that the isolate was Herbaspirillum huttiense. Conclusions:There are more and more environmental microorganisms evolved into human pathogenic bacteria. Herbaspirillum species are easy to be misidentified because its biochemical characteristics are similar to other strains.
5.Clinical values of 16S rRNA and ITS gene sequencing for pathogenic identification of renal transplant recipients with urinary tract infection

Objective:Objective To explore the clinical values of next-generation sequencing (NGS) in bacterial 16S rRNA region and fungal ITS region for diagnosing and treating urinary tract infection (UTI) in renal transplant recipients.Methods:A total of 90 mid-stream clean-catch urine samples were collected from renal transplant recipients who were diagnosed with UTI at Hospital from January 2017 to December 2019. Each sample was equally divided and tested via NGS method and traditional urine culture separately. The results of pathogen test and detection rate were analyzed and compared.Results:And 21/90 sample were considered to be contaminated due to the identification of three or more kinds of microorganisms by culture. And among the remaining 69 samples, 36 (52.17%) cases tested positive by 16S rRNA sequencing, 25 (36.23%) positive by urine bacterial culture; meanwhile, 34(49.28%) tested positive by ITS sequencing and 4(5.80%) positive by urine fungal culture.Conclusions:The detection rate of both bacteria and fungi in NGS microorganism testing is higher than that in traditional urine culture ( P< 0.05). For renal transplant recipients with UTI, NGS microorganism testing is an effective supplement for traditional urine culture. Improving the detection rate and accuracy of etiology may enable an optimization of individualized treatment.
6.Research on Optimization of Drug Supervision Strategy of Guangdong-Hong Kong-Macao Greater Bay Area Based on Tripartite Evolutionary Game

OBJECTIVE:To provide reference for improving the mechanism of drug supervision in Guangdong-Hong Kong-Macao Greater Bay Area. METHODS :Based on the evolutionary game theory ,the stakeholders of Guangdong-Hong Kong-Macao Greater Bay Area in the stage of drug safety supervision were identified ;the evolutionary game model was established so as to obtain the behavioral selection and evolutionary stability strategy of the each party through and put forward the suggestions for improving the mechanism of drug supervision in Guangdong-Hong Kong-Macao Greater Bay Area. RESULTS & CONCLUSIONS:Based on evolutionary game theory ,the three-party evolutionary game model of drug administration ,drug inspection and manufacturing enterprises was established. According to the model established in this study ,it could be concluded that:when the compliance cost of the production enterprise was less than the sum of the violation cost and the bribery cost minus the additional income ,or less than the sum of violation cost and punishment and reputation lossminus the additional income ,the manufacturing enterprises could achieve the equilibrium state of compliance production ;the supervision probability was inversely proportional to the supervision cost ,and strict inspection was inversely proportional to inspection cost ;compliance productivity rate and strict detection rate were in direct proportion to the intensity of supervision and penalties ;the higher the supervision probability was,the higher the strict detection rate was ,and the compliance productivity would gradually increase. It is proposed to link enterprise information files ,lay the foundation of four party cogovernance ;introduce the “cost-benefit”supervision system ,make reasonable use of the third-party inspection institutions ;clarify the boundary of personnel responsibility ,and use the intelligent system for real-time supervision so as to promote the improvement of drug safety supervision in Guangdong-Hong Kong-Macao Greater Ba y Area.
