1.Psoriasis vulgaris and Helicobacter pylori infection

Objective To explore the relationship between Helicobacter pylori (Hp) infection and psoriasis vulgaris, and investigate the effect of anti-Hp therapy on the patients with psoriasis vulgaris. Methods 13C-urea expiration test and detection of Hp antigen in feces samples were performed to identify Hp infection in 62 psoriasis patients and 35 healthy persons. Detection results of the two groups were statistically analyzed. The 62 psoriasis patients were divided into two groups based on the detection of Hp infection: Hp positive group (32 cases) and Hp negative group (30 cases). Patients in Hp positive group were given anti-Hp triple therapy (omeprazole, amoxicillin, clarithromycin) for two weeks and topical application of tretinoin for six weeks, while the patients in Hp negative group were treated only with topical use of tretinoin for six weeks. The total effective rate, score of psoriasis area, severity index (PASI) and visual analogue scale (VAS) of pruritus were recorded and then analyzed. Results The Hp infection rate in 62 psoriasis patients was 51.6%, and in healthy individuals was 22.9%, the difference between the two groups was statistically significant (P

The paper analyzes some of the major causes of the current confusion in medical service prices. These include: ①irrationality of the mechanism of medical service pricing; ②lag of the management of medical service prices; ③irrationality and limitations of the distribution system in hospitals; and ④confusion resulting from the traditional medical security system of free medical care. The countermeasures put forward to deal with the confusion include: ①establishing a rational pricing mechanism; ②strengthening the supervision and management of medical service prices; and ③further speeding up relevant medical systems.
3.Expression and localization of CB2 (cannabinoid receptor) in rat skin

Objective To study the expression and distribution of CB2 in rat skin tissue. Method Immunohistochemistrial technics were employed to study the expression and distribution of CB2 in rat skin tissue. Results CB 2 receptors mostly presented in suprabasal layers of the epidermis and hair follicles in dermis by CB2 like immunoreactivity with a N terminal anti CB2 receptor antibody. The positive expression of CB2 appeared in rat spleen and a negative result occured in liver. Conclusion The CB2 receptors distributed mainly in the rats, epidermis and hair follicles, may involve in some of the dermic physiological and pathological process, such as the communication between central neural system and skin.
4.The therapeutic efficacy of continuous blood purification for severe acute renal failure after cardiac surgery

Objective To evaluate the efficiency and the timing of continuous blood purification(CBP) in patients with multiple or-gan dysfunction syndrome(MODS) and acute renal failure(ARF) following cardiac surgery. Methods From November,2003 to July,2007,thirty-one patients with MODS and ARF following cardiac surgery were treated with CBP. They were divided into two groups, alive group ( group A) and dead group ( group B). Clinical data of the two groups were reviewed. Before CBP, duration of extracorporeal circulation, du-ated. Mean arterial pressure (MAP), heart rate (HR), oxygenic index (PaO2/FiO2), white blood cell count (WBC), platelet count (PLT) and renal function (blood urea nitrogen, BUN, ercatinine, Cr) were observed before and after CBP. Results The number of im-paired organs of patients in group B was significantly more than that in group A before CBP ( P<0.05). MODS scores ( 12.9±3.2 vs 6.9 ±2.3, P<0.05) and APACHE Ⅱ scores (26.3±10.4 vs 17.2±5.1, P<0.05)of group B were significantly higher than those of group A before CBP. After treatment of CBP for 24 hours, APACHE Ⅱ scores and MODS scores only significantly decreased in group A (P<0.05 ). After treatment of CBP, Cr and BUN significantly reduced, while MAP and PaO2/FiO2 increased, in all patients, but HR was signif-icantly lower than that before CBP in group A (P<0.05). The duration of ARF (34.67±32.79 hours vs 13.05±14.09 hours,P<cantly higher than those of group A. Conclusion MODS scores and APACHE II scores can be used to evaluate the severity of patients with MODS and ARF after cardiac surgery. CBP is an effective treatment for these patients. It is suggested that early CBP therapy is important for reducing the chances of the multiple organ dysfunction syndrome and mortality of these patients.

6.Localization of dopamine- and cAMP-regulated phosphoprotein in mouse kidney

Objective To investigate the expression and localization of dopamine- and cAMP-regulated phosphoprotein of 32 000 (DARPP-32) in mouse kidney tissue. Methods The cellular localization of DARPP-32 in mouse kidney tissue was detected by immunoblot and immunohistochemistry. Results DARPP-32-like immunoreactivity was detected in the medullary thick ascending limb of the loop of Henle, the cortical proximal convoluted tubule, and collecting ducts in medullary rays. The renal tubules were enriched of Na+, K+-ATPase for sodium reabsorption. Conclusion The participation of DARPP-32 is a likely crucial step of the signal-transduction pathway of dopamine regulation on sodium reabsorption in renal tubule cells.
7.Bioartificial kidney alters cytokine response and survival time in acute uremic pigs with multiple organ dysfunction syndrome

Objective To study the effects of bioartificial kidney(BAK)treatment on cytokines interleukin-10(IL-10)and tumor necrosis factor-α(TNF-α),biochemistry indexes,and survival time in acute uremic pigs with multiple organ dysfunction syndrome(MODS).Method Pigs with MODS and acute renal failure(ARF)were treated with BAK(group A,n = 6)or sham BAK containing no cells(group B,n = 6)or received no treatment(group C,n = 5).Data on blood pressure,hepatic and renal functions,serum IL-10 and TNF-α levels,arterial blood gas and survival time of all the pigs were recorded.Comparisons of values were done using Student's t-test or repeated-measures analysis of variance(ANOVA).ResultsMean arterial pressure(MAP,mmHg)responsed more rapidly and reached higher values in group A (91.82±5.73)compared with group B and C at 24 hours(P<0.01).The peak level of serum EL-10(pg/mL)ingroupA(249.57±43.51)was significantly higher than that in group B and C(132.06± 17.53,104.25 ±13.42,P<0.01).Serum TNF-a level(pg/mL)in group A dropped gradually to(402.91 ± 32.47)at 24 hours,and had significant discrepancy compared with that before the treatment(537.16 ± 38.45)and that in groupB(P<0.05).The average survival time(hours)in group A(113.01 ± 14.32)was significantly longer than that in the group B and C(P<0.01),which was prolonged by 35.93%and 63.90%,respectively.ConclusionsTreatment with BAK can prolong the survival time of uremic pigs with MODS,possibly through ameliorating the MAP,increasing the level of IL-10 and reducing the concentration of TNF-α.
8.Comparison of healthcare expenditures and self-payment among patients with lung cancer in Wenling City before and after implementation of diagnosis-related groups (DRGs)

Objective:
To investigate the healthcare expenditures and self-payment among patients with lung cancer in Wenling City before and after implementation of diagnosis-related groups (DRGs), so as to provide the evidence for controlling medical costs and relieving burdens of patients with lung cancer.
Methods:
The basic data and healthcare expenditures of lung cancer patients that were definitively diagnosed from 2015 to 2019 and covered by medical insurance were captured from the cancer registration database of Wenling Center for Disease Control and Prevention and the database of chronic and specific diseases in Wenling Bureau of Medical Insurance. The changes of outpatient expenditures, inpatient expenditures and self-payments were compared before (2015-2016) and after implementation of DRGs (2018-2019) among lung cancer patients.
Results:
Totally 4 947 lung cancer patients covered by medical insurance were enrolled in this study, including 3 052 males (61.69%) and 1 895 females (38.31%), with a mean age of (64.88±11.64) years. The annual mean healthcare expenditure was 56 675.85 Yuan per capita during the period between 2015 and 2016, in which 14.48% were outpatient expenditures and 85.52% were inpatient expenditures, and the annual mean healthcare expenditure was 38 702.94 Yuan per capita during the period between 2018 and 2019 (a 31.71% reduction as compared to that in 2015 and 2016), in which 24.49% were outpatient expenditures and 75.51% were inpatient expenditures. The proportions of outpatient expenditures, inpatient expenditures and total self-payments consisted of 25.38%, 32.49% and 29.67% of total healthcare expenditures in 2018 and 2019, which were significantly lower than those (50.84%, 50.96% and 50.95%, respectively) in 2015 and 2016 (χ2=13.741, P<0.001; χ2=7.015, P=0.008; χ2=9.340, P=0.002).
Conclusions
The annual mean healthcare expenditures per capita and the proportion of self-payment reduce among lung cancer patients covered by medical insurance following implementation of DRGs.
9.Application of long-term cuffed catheter in patients with chronic hemodialysis

Objective To investigate the technique and effect of tunneled cuffed catheter as long-term hemodialysis access and the prevention of its related complications.Methods Permcath cuffed dual lumen catheter wag implanted in patients by the standard Seldinger technique,that a guidewire wss inserted into deep central vein, following by the insertion of the cuffed catheter through a peel-away sheath.The catheter related complications were observed.Urea reduce rate(URR),the Kt/V,the normalized protein catabolic rate(nPCR)and the time-averaged BUN concentration(TACurea)were measured as the index of the effect of hemodislysis.The effects were compared to those of arteriovenous fistula(AVF)within the same period(in 20 cases).Results In the total of 25 patients, 23 catheters were implanted in the internal jugular vein (21 cases at right,two at left side),one in the subclavian vein and one in the external jugular vein.Except for that one case died of cerebral infarction,all patients were using the catheter in well condition.Catheter related thrombosis or underfed occurred in 4 cases for 9 times, which became fluent after thrombolytic therapy or the adjustment of catheters.Catheter related infections occurred in 2 cases,and were successfully treated with anti-inflammation therapy.The average URR was 71.03%,Kt/V was 1.29,TAC urea was 12.74 mmol/L,and nPCR was 0.93 g/(kg·d)in all 25 patients,which was not significant different compared to patients with AVF((URR 72.46%,Kt/V 1.31,TACurea 12.86 mmol/L,nPCR 0.94 g/(kg·d)).Conclusions Cuffed dual lumen catheter for long-term aecegs in persistent hemodialysis has good effect on hemodialysis. Good catheter implanting technique and prevention of complications may improve the application of the catheter.
10.Reflection on and practice in setting up "the green hospital

In view of the challenges and opportunities presented in the new century and after rational deliberations on five occasions, the completely new model of "hi tech+humanistic solicitude=the green hospital" was put forward and the overall framework of "one line of thought", "two cornerstones", and "three goals" was carefully formulated. In the meantime, all staff members of the hospital were called on to be involved in the "six major activities". As a result, great changes have taken place in the appearance and development of the hospital. It has been proved through practice that "the green hospital" is a successful model conforming to the trends of the times.
