Objective To study the clinical characteristics of early esophageal varices bleeding after endoscopic varices liga-tion EVL in advanced schistosomiasis patients. Methods The data of 206 advanced schistosomiasis patients who received VEL were collected and studied retrospectively. Results There were 17 cases of early esophageal varices bleeding after EVL in-cluding 1 died case the early hemorrhage rate was 8.25% and the mortality rate was 0.5%. The early bleeding occurred from the 4th to 12th day and 76%occurred from the 7th to 9th day postoperatively. The direct cause of hemorrhagic was ligation ring falling off and the inducements were the improper diet 10 cases 58.8% and increased abdominal pressure 6 cases 35% . All the cases of early esophageal varices bleeding occurred in the patients whose liver function being Child-Pugh C. Conclu-sions The incidence and mortality of EVL early postoperative hemorrhage are both low and mostly occur from the 7th to 9th day postoperatively. We should pay attention to the diet and nursing and the patients with Child-Pugh C liver function are the high risk group.

ObjectiveTo evaluate the efficacy and safety of bepotastine besilate in the treatment of chronic urticaria.MethodsA randomized,double-blind,parallel-controlled clinical study was conducted in 5 centers.Patients were randomly assigned to 2 groups to be treated with bepotastine besilate 20 mg or loratadine 10 mg once a day,respectively,for 4 weeks.Visits were scheduled before and after 1,2 and 4 weeks of treatment.Itching degree,number of wheals and diameter of the largest wheal were recorded for efficacy evaluation.ResultsTotally,240 patients were enrolled and 227 patients completed the study.The response rate was 74.6％ and 77.9％ respectively in bepotastine besilate- and loratadine-treated patients,respectively(P ＞0.05).No significant difference was observed in the incidence of adverse reactions between bepotastine besilate- and loratadine-treated patients(12.8％ vs.17.9％,P ＞ 0.05).The main side effects were mild to moderate drowsiness,dry mouth,dizziness.ConclusionBepotastine besilate is effective and safe for the treatment of chronic urticaria,with an efficacy and safety profile similar to that of loratadine.

Objective To investigate the etiology,diagnosis and treatment of the delayed gastric emptying after abdominal surgery.Methods From January 2005 to December 2012,the clinical data on diagnosis and treatment of 32 cases of delayed gastric emptying after abdominal surgery were retrospectively analyzed.Results Delayed gastric emptying occurred in 32 cases after 5-8 days after the surgery,which accounted for 40.63％ of gastric surgery.Blood loss was 100-300 mL in 15 cases,9 cases' blood loss was more than 350 mL,accounting for 75％.Thirty cases were cured by conservative treatment,accounting for 93.75％,2 cases on the 20th day after surgery and the 31 th day after surgery to accepted surgery again,accounting for 6.25％.Conclusion The delayed gastric emptying after surgery is closely related to surgical site,methods and surgical sub-injury.Non-occurrence of surgical treatment is the main method to cure this disease.

Objective To investigate the value of human epididymis secretory protein 4 ( HE4 ) combined with CA125 assay in differential diagnosis of endometriosis cyst and ovarian malignant tumor.Methods The level of HE4 and CA125 were measured by enzyme-linked immunosorbent assay (ELISA) in the serum specimens of 46 cases in endometriosis cyst group,36 cases in malignant ovarian tumor group,60 cases in benign ovarian diseases and 50 women in healthy women group.Those results were shown with median level.The normal range were 0-150 pmol/L in HE4 and 0-35 kU/L,which either one was more than the threshold value defined as positive index.The sensitivity of assay was evaluated by receiver operating characteristic (ROC) curve,the relation and value of HE4 or CA125 alone and combination assay in diagnosis of endometriosis was analyzed by Mann-Whitney U test and correlation analysis.Results (1) HE4:the median levels of HE4 were 52.4,51.0,50.0 pmol/L in group of endometriosis,normal control and benign ovarian tumor,which didn't show statistical difference.However,HE4 was 507.5 pmol/L inovarian cancer group,which was significantly higher than those of 3 groups (P < 0.05 ).(2 ) CA125:there were significant different in median level of CA125 was observed as 743.0 kU/L in ovarian cancer,84.9 kU/L in endoemtriosis,15.4 kU/L in benign ovarian disease,and 11.5 kU/L in healthy women (P < 0.05).( 3 ) The single assay:when compared with that in endometriosis group,receiver operating characteristic area under the curve( ROC-AUC) were 0.933 in HE4 alone and 0.821 in CA125 alone assay in ovarian cancer group.The specificity was 95% and the sensitivity was 79.6% and 49.0%.(4) The combination assay:when compared with those in endometriosis group,the ROC-AUC was 0.936,the specificity was 95% and the sensitivity was 81.0% in ovarian cancer.Conclusions Measurement of HE4 could be used in differential diagnosis of endometriosis cyst.And the combination of HE4 and CA125 assay could discriminate ovarian endometriosis cysts from ovarian malignant tumors effectively.

Objective:To develop health-related physical fitness measurement scale and assess its reliability and validity among older adults.Methods:The health-related physical fitness measurement scale was developed through Delphi method and field investigation method.And the authority coefficient, Kendall's coefficient and variation coefficient were analyzed.A multi-stage random sampling technique was adopted to select 410 older adults to investigate the reliability and validity of health-related physical fitness measurement scale with test-retest reliability, Cronbach's α coefficient and Guttman Split-Half coefficient and structural validity by IBM SPSS 20.0 and AMOS 22.0.Results:The health-related physical fitness measurement scale consisted of 3 dimensions (organic function, motor function, physical adaptive capacity) and 15 indexes.The Cronbach's α coefficient was 0.84.The Spearman-Brown correlation coefficient and Guttman Split-Half coefficient were both 0.82.The test-retest reliability between the first measurement and retest after 1 week was 0.84.Three factors were extracted by exploratory factor analysis, which were basically consistent with the theoretical concept of the scale, but the items of the theoretical conception were different from the actual ones.Confirmatory factor analysis based on the theoretical conception showed that all the indexes met the requirements of reference value as well(χ 2/ df=2.38, GFI=0.94, AGFI=0.91, CFI=0.92, TLI=0.91, TFI=0.92, RMSEA=0.06). Conclusion:The health-related physical fitness measurement scale has good reliability and validity in measuring the health fitness level of the elderly population in Guangzhou.

Abstract? AIM: To evaluate the efficiency of two different operation ways, vitrectomy and vitrectomy combined with inferior scleral buckling, for a complex kind of retinal detachment.?METHODS:The complex kind of retinal detachment were diagnosed in 100 cases ( 100 eyes ) , with the common features:1)the course of more than 1mo;2) at least one retinal hole located in the inferior marginal retina;3) the detachment of retina was found proliferated, there was at least 1 retinal fold in retinal detachment area. The patients were randomly divided into two groups: the treatment group ( 50 eyes ) who received vitrectomy combined with inferior scleral buckling;the control group ( 50 eyes ) who received vitrectomy without scleral buckling. The retinal anatomic reattachment, visual function recovery, macular central fovea thickness and complications in two groups were observed.? RESULTS: There were 49 eyes ( 98%) with retinal anatomic reattachment in the treatment group while 42 eyes(84%)in control group(χ2=4.2605, P<0.05);the best corrected visual acuity ( BCVA) was 0.254 ±0.238 in the treatment group while it was 0.219±0.229 in control group (t=1.8879,P>0.05);the macular central fovea thickness was 272 ±32.21μm in the treat group while it was 316 ± 33.46μm(t=12.597,P<0.01).Intraocular pressure in 12 eyes ( 24%) was more than 30mmHg in the treatment group while 4 eyes ( 8%) in control group within 1wk postoperation (χ2=4.7619,P<0.05); intraocular pressure in 100 eyes were controlled below 21mmHg 1mo postoperatively.? CONCLUSION: It is a more effective method to vitrectomy combined with inferior scleral buckling than the single vitrectomy for the special kind of retinal detachment.It can increase the rate of retinal anatomic reattachment and reduce macular edema.
