1.Study on the mode of operation for post-radiotherapy esophageal carcinoma

Purpose:To discuss which is the better mode of operation for esophageal carcinoma after radiation therapy. Methods:We divided 232 esophageal carcinoma patients into two groups (group A 116 patients and group B 116 patients) random. Operation mode of group A was posterolateral thoracotomy through the bed of sixth rib in right thorax, retrosternal replacement esophagus with stomach and left neck anastomosis or esophagus exteriorization. Correspondingly operation mode of group B was anterolateral thoracotomy through the intercostal space of fourth rib in right thorax, esophageal pouch replacement esophagus with stomach and left neck anastomosis or esophagus exteriorization. We analyzed the resection rate, the mortality rate, the duration of operation, the volume of bleeding and total incidence of complication in each group. Results:The resection rate was 100% in group A and 92.24% in group B ( P 0.05). Conclusions:Operation mode of group A is a better mode of operation for esophageal carcinoma after radiation therapy which is based on the resection rate, which is higher in group A than in group B and total incidence of complication which is lower in group A than in group B.
2.Effects of Wumeiwan on δ-Opioid Receptor,β-Arrestin1 and Bcl-2 Expressions in Rats with Colitis

Background:Ulcerative colitis(UC)is a chronic and nonspecific intestinal inflammatory disease and its pathogenic mechanism has not yet been clarified. Intestinal mucosal immune function disorder may play a key role in the pathogenesis of UC. Aims:To investigate the effects of Wumeiwan on expressions of δ-opioid receptor(DOR),β-arrestin1 and Bcl-2 in rats with colitis. Methods:Fifty-six Sprague-Dawley rats were randomly divided into model group,Wumeiwan group, mesalazine group and blank group. Rats in model group,Wumeiwan group and mesalazine group were administered intrarectally with 5% TNBS and 50% ethanol to induce experimental colitis. After colitis models were established,rats in Wumeiwan group and mesalazine group were administered intragastrically with Wumeiwan and mesalazine suspension, respectively,and rats in model group and blank group were given intragastrically with 0. 9% NaCl solution,all for 15 days. On day 16,all the rats were sacrificed and colon samples were obtained. Protein and mRNA expressions of DOR,β-arrestin1 and Bcl-2 in colonic tissue were determined by immunohistochemistry and real-time PCR,respectively. Results:The inflammatory injury in colonic tissue of rats with experimental colitis was significantly attenuated when treated with Wumeiwan,Protein and mRNA expressions of DOR,β-arrestin1 and Bcl-2 in colonic tissue of model group were significantly higher than those of blank group(P < 0. 05). Protein and mRNA expressions of DOR,β-arrestin1 and Bcl-2 in colonic tissue of Wumeiwan group and mesalazine group were significantly lower than those of model group(P < 0. 05), however,no significant differences were found between the two groups(P > 0. 05). Conclusions:DOR-β-arrestin1-Bcl-2 signal transduction pathway may play a central role in the pathogenesis of UC. Intervening this signaling pathway may be one of the mechanisms of attenuating UC by Wumeiwan.
3.Expression of SH2B1 adaptor protein in oesophageal cancer and its clinical significance.

OBJECTIVE:
To explore the expression of SH2B1 adaptor protein in oesophageal cancer and its clinical significance.
METHODS:
SH2B1 expression in tissue specimens of 120 primary oesophageal cancers, tissues of 120 paired adjacent non-cancer and another 120 normal tissues was analyzed by immunohistochemical SABC staining and Western blot. SH2B1 expression in the oesophageal cancer tissues was analyzed with clinicopathological parameters. SH2B1 expression of normal human esophageal epithelial cells (HEEC) and 2 oesophageal cancer cell lines, TE-1 and Eca109, were evaluated by RT-PCR and Western blot.
RESULTS:
SH2B1 expression in the normal oesophageal tissues, adjacent non-cancer tissues and cancer tissues was gradually increased (P<0.05), and significantly associated with the depth of invasion, clinical TNM stage, lymph node metastasis (P<0.05), but it did not have significant association with gender, age, drinking, type and grade of tumor (P>0.05). SH2B1 expression was detectable in all cell lines by RT-PCR and Western blot, but the expression in the two oesophageal cancer cell lines was significantly higher than that in the normal HEEC.
CONCLUSIONS
Over-expression of SH2B1 might play an important role in the occurrence and development of human oesophageal cancer and closely correlate with malignant progression of invasion and metastasis of oesophageal cancer.

4.Effect of Jianpi Shugan Tongru prescription combined with staged medicinal diet therapy in treating postpartum hypogalactia of qi and blood deficiency pattern

Objective To analyze the efficacy of Jianpi Shugan Tongru prescription combined with staged medicinal diet therapy in the treatment of postpartum hypogalactia of qi and blood deficiency pattern.Methods Patients with postpartum hypogalactia of qi and blood deficiency pattern who gave birth in Jinhua People's Hospital from March 2022 to March 2023 were selected as the research subjects,and were divided into control group(conventional treatment)and test group(treated with Jianpi Shugan Tongru prescription combined with staged medicinal diet therapy).The clinical efficacy,TCM syndrome scores,lactation-related indexes,prolactin(PRL)and estradiol(E2)levels were compared between the two groups.Results A total of 100 patients were included in the study,including 50 patients in the test group and 50 in the control group.After treatment,the total effective rate and the TCM syndrome scores in the test group were better than those in the control group(P<0.05),the time to initiation of lactation,breast filling score and milk production in the test group were better than those in the control group(P<0.05),the PRL level in the test group was higher than that in the control group,the E2 level was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Jianpi Shugan Tongru prescription combined with staged medicinal diet therapy has a good effect on the treatment of patients with postpartum hypogalactia of qi and blood dificiency pattern,and can improve the TCM syndrome scores and lactation-related indexes of patients,and can regulate the levels of PRL and E2.
5.Clinical study on simvastatin treatment of pulmonary hypertension in patients with coal worker's pneumoconiosis.

OBJECTIVETo observe simvastatin treatment of pulmonary hypertension in patients with coal workers pneumoconiosis (CWP).
METHODS96 CWP patients with pulmonary hypertension were randomly divided into treatment and control groups. The control group was treated with 2.5 mg warfarin, once a day for four months; the treatment group was treated with 20 mg simvastatin, taken in evening, for 4 months. 6 min walking distance (6MWD) test and inspection pulmonary artery pressure were measured by echocardiography before and after treatment.
RESULTSIn the treatment group, the 6MWD were (258 ± 26) m after treatment and (225 ± 19) m before treatment, respectively. Compared with control group, pulmonary artery pressure was (41 ± 9) mm Hg in the treatment group before treatment, (36 ± 3) mm Hg in the treatment group after treatment, and (39 ± 5) mm Hg in control group, respectively, the difference was statistically significant (P < 0.05).
CONCLUSIONSSimvastatin can improve pulmonary hypertension in coal workers pneumoconiosis, and shows a definite curative effect.

6.Effects of mercury on the structure and activity of BLM642-1290 recombinant helicase.

OBJECTIVEBloom's syndrome is an autosomal recessive disorder characterized by genomic instability and a predisposition to many cancers. Mutations of the BLM gene (encoding a BLM helicase) may form a structure of the etiology of this disease. As a global pollutant, mercury poses a major threat to human health. The current study was conducted to elucidate the effects of Hg(2+) on the structure and activity of BLM642-1290 recombinant helicase, and to further explore the molecular mechanisms of mercury toxicity to the DNA helicase.
METHODSThe effects of Hg(2+) on biological activity and structure of BLM642-1290 recombinant helicase were determined by fluorescence polarized, ultraviolet spectroscopic, and free-phosphorus assay technologies, respectively.
RESULTSThe helicase activity, the DNA-binding activity, and the ATPase activity of BLM642-1290 recombinant helicase were inhibited by Hg(2+) treatment. The LMCT (ligand-to-metal charge transition) peaks of the helicase were enhanced with the increase of the Hg(2+) level. The LMCT peaks of the same concentration of helicase gradually increased over time.
CONCLUSIONThe biological activity of BLM642-1290 recombinant helicase is inhibited by Hg(2+) treatment. The conformation of the helicase is significantly altered by Hg(2+). There exist two binding sites between Hg(2+) and the helicase, which are located in the amino acid residues 1063-1066 and 940-944 of the helicase, respectively.

7.Hepatitis B virus genotypes and alanine aminotransferase levels in HBeAg negative patients with chronic hepatitis B and liver cirrhosis.

OBJECTIVETo investigate genotypes of the hepatitis B virus (HBV) and alanine aminotransferase (ALT) levels of HBeAg negative patients with chronic hepatitis B and liver cirrhosis.
METHODSHBV serological markers and ALT levels were detected in 62 patients with chronic hepatitis B and 41 cases with liver cirrhosis, using enzyme linked absorbent immunoassays and an enzyme method, respectively. A polymerase chain reaction of S region was used for HBV genotyping.
RESULTSOf the 62 patients with chronic hepatitis B, 21 (33.9%) were HBeAg negative, and 41 (66.1%) HBeAg positive. Among 41 cases with liver cirrhosis, 28 (68.3%) were HBeAg negative, and 13 (31.7%) HBeAg positive. Of these 62 patients with chronic hepatitis B, 53 (85.5%) were infected with HBV genotype C, and 9 (14.5%) with genotype B. Thirty-nine (95.1%) of the 41 patients with liver cirrhosis were infected with genotype C, and 2 (4.9%) with genotype B. The proportion of HBeAg negative chronic hepatitis B patients with ALT level > 40 U/L was lower than that of the HBeAg positive group (47.6% and 85.4%, respectively) (P < 0.01). The percentage of ALT levels > 40 U/L of the negative patients with liver cirrhosis was also lower as compared to that of the HBeAg positive patients, but there was no statistical difference between the two groups, because of the small sample size (P > 0.05).
CONCLUSIONThe proportion of HBeAg negative patients is high in the group of chronic hepatitis B and liver cirrhosis. These patients have relatively low ALT levels, and mainly have HBV genotype C infection.

8.Effects of celastrol on the proliferation and apoptosis of human leukemia cells HL-60 and Jurkat

To determine the effects of celastrol on the proliferation and apoptosis of cell lines HL-60 and Jurkat in human leukemia. Human leukemia cell lines were treated with celastrol at different concentrations. The inhibitory rate of cell proliferation was detected by MTT assay; the apoptosis rate was detected by AnnexinV-FITC/PI double staining; cell cycle was observed by PI staining; cell morphology was observed by transmission electron microscope(TEM). Cell proliferation was inhibited by celastrol, with IC50 of(0. 46±1. 05)μmol/L and(0. 88±1. 13)μmol/L at 24 h. Celastrol induced cell apoptosis in a dose-dependent manner. The cell cycle distribution of G1 phase rate increased, S phase rate decreased(P< 0. 05)with typical cell apoptosis-induced morphological changes. Results showed that celastrol could significantly inhibit cell proliferation and induce apoptosis in human leukemia cell lines of HL-60 and Jurkat.
9.Expression and significance of JNK1, Raf-1 and Livin in the carcinogenesis of sporadic colorectal tubular adenoma.

OBJECTIVETo study the possible role of JNK1, Raf-1 and Livin in the carcinogenesis of sporadic colorectal tubular adenoma.
METHODSImmunohistochemical staining was used to detect the expression of JNK1, Raf-1 and Livin proteins in 65 sporadic colorectal tubular adenomas with dysplasia of varying degrees and 22 colorectal tubular adenoma with cancerous area.
RESULTSIn normal colorectal mucosa, colorectal tubular adenoma with dysplasia and colorectal tubular adenoma with cancerous area, the positive rate of JNK1, Raf-1 and Livin expression was increased gradually. The positive expression of JNK1, Raf-1 and Livin was all significantly higher in the cases of colorectal tubular adenoma with dysplasia or with cancerous area than that in normal colorectal mucosa (P < 0.05), and the positive expression of JNK1, Raf-1 and Livin was significantly higher in colorectal tubular adenoma with cancerous area than that in colorectal tubular adenoma with dysplasia of different degrees (P < 0.05). In the cases of colorectal tubular adenoma with dysplasia of varying degrees, the positive expression of Raf-1 was increased along with the increasing dysplasia degree of colorectal tubular adenoma (P < 0.05). Coexpression of JNK1, Raf-1 and Livin increased gradually in the carcinogenesis of sporadic colorectal tubular adenoma, while positive correlation was found among the expressions of JNK1, Raf-1 and Livin.
CONCLUSIONJNK1, Raf-1 and Livin may be involved in the carcinogenesis of sporadic colorectal tubular adenoma.

10.Coordination and function of a laparoscopic assistant in laparoscopic pancreaticoduodenectomy

Objective To study the coordination and function of a laparoscopic assistant in laparoscopic pancreaticoduodenectomy (LPD).Methods A retrospective analysis was conducted on 101 patients who underwent LPD at the Department of Hepatobiliary Surgery,Hunan Provincial People's Hospital,from January 2014 to March 2017.The study aimed to study the coordination and function of a laparoscopic assistant.Results LPD was successfully completed in all the 101 patients.There was no conversion to open surgery.The operation time was (326.0 ± 55.6) min,and the resection time was (174.4 ± 42.5) min.The digestive tract reconstruction time was (101.0 ± 21.4) min.The time of pancreaticojejunostomy was (40.5 ± 8.7) min.The time of gastrointestinal anastomosis was:(26.3 ± 5.5) min.The time of biliary anastomosis was (24.4 ± 6.5) min.The intraoperative bleeding was (175.6 ± 41.1) ml.Postoperative pathological data showed that 27 patients (26.7%) had distal common bile duct cancer,23 patients (22.8%)ampullary carcinoma,39 patients (38.6%) duodenal papillary carcinoma,and 12 patients (11.9%) pancreatic ductal adenocarcinoma.The tumor diameter was (2.3 ± 1.3) cm,and the number of resected lymph nodes was (16.7 ±4.2).The number of positive lymph nodes was 1.3 ± 1.1.The length of postoperative hospital stay was 14.8 (8 ~ 29) d.Twenty-three patients developed postoperative pancreatic fistula,including 17 patients (16.8%) with a biochemical fistula,5 patients (5.0%) with a grade B pancreatic fistula,and 1 patient (1.0%) with a grade C pancreatic fistula.There were 2 patients (3.0%) with bile leakage,7 patients (6.9%) with intra-abdominal bleeding,4 patients (4.0%) with delayed gastric emptying,6 patients (5.9%) with abdominal infection,3 patients (3.0%) with pulmonary infection,2 patients (2.0%)with intestinal obstruction,3 patients (3.0%) required a repeated operation,and 1 patient (1.0%) with death in hospital within 30 days after surgery.Conclusions The laparoscopic assistant should have the perspective of "one axis,two sides and four regions" in LPD,and warn the operator to ensure the safety and fluency of the operation by clearly exposing important blood vessels and organs when performing the Kocher incision and when dissecting the key parts such as the dangerous triangle of the uncinate process.During anastomosis,the laparoscopic assistant should appropriately adjust the distance of vision,clearly reveal the surgical field of the anastomotic area,and help the surgeon in improving the precision of the suture and the quality of the anastomosis.
