1.Acute fibrinous and organizing pneumonia: one case report and literature review

Objective To further improve the awareness of the clinical feature of acute fibrinous and organizing pneumonia (AFOP).Methods One case of AFOP treated in our department was described in details.The relevant cases were searched in Wanfang database using key words acute fibrinous and organizing pneumonia [in Chinese].Results Four reports containing 9 cases were identified from published literature.A total of 10 cases (including this one) were analyzed.The patients included 7 males and 3 females (43 to 78 years of age).Eight patients presented with cough.Chest tightness and dyspnea were reported in 8 cases,and fever in 9 cases.Velcro crackles were heard in 4 patients.Laboratory tests showed WBC increased in 2 cases,increased neutrophil count in 6 cases,elevated C-reactive protein in 9 cases,and faster erythrocyte sedimentation rate in 8 cases.The partial oxygen pressure (PO2) ranged from 54 mmHg to 69 mmHg.Chest CT scan showed unilateral lesions in 3 cases and bilateral lesions in 7 cases initially.The main CT findings were patchy,consolidation and ground-glass opacities,sometimes associated with air bronchogram.The diagnosis was confirmed by lung biopsy in all the 10 cases.Bronchoscopy biopsy was conducted in 6 cases,and percutaneous lung biopsy in 3 cases.One patient received both bronchoscopy biopsy and percutaneous lung biopsy.Methylprednisolone was used in all cases.No patient received mechanical ventilation.One patient died.Conclusions AFOP is a new type of interstitial lung disease,the etiology of which is unknown.AFOP often occurs in middle-and old-aged patients.AFOP is easily misdiagnosed due to its unspecific clinical manifestations,which are similar to common pneumonia.The confirmation of AFOP diagnosis depends on pathological biopsy.Corticosteroids treatment is appropriate,but the dosage,duration,and long term effect of corticosteroids are not established.

type of extreme heat with yin asthenia.But discrepancy of CO and EF between type of deficiency of both QI and YIN and type of QI-stagnation and blood stasis all had not statistical significance.Conclusion: In clinic to aim directly at left ventricular contractile dysfunction of senile diabetics,no matter syndrome differentiation belonging to type of deficiency of both QI and YIN,type of QI-stagnation and blood stasis or type of deficiency of both yin and yang,all can benefit after treatment with supplementing QI and activating blood circulation method;but syndrome differentiation belonging to type of extreme heat with yin asthenia advocates not to use supplementing QI and activating blood circulation method.
3.Prostate cancer cell vaccine transfected with 4-1BBL induces anti-tumor immunity in vitro.

OBJECTIVETo explore the anti-tumor immunity in vitro induced by prostate cancer cell vaccine transfected with recombinant adenovirus encoding 4-1BBL in mice.
METHODSThe replication-deficient adenovirus AdEasy-1 system was used to construct recombinant adenovirus Ad-m4-1BBL and Ad-eGFP. The prostate cancer cell RM-1 of mice was transfected with Ad-m4-1BBL and Ad-eGFP, and treated with mitomycin (MMC) to produce TCV, TCV-Ad-eGFP and TCV-Ad-m4-1BBL, followed by co-culture with syngeneic murine spleen cells. Then the cytotoxic activity of the lymphocytes against RM-1 cells was analyzed with CCK-8 solution, and IL-2 and INF-gamma were detected by ELISA.
RESULTSThe 4-1BBL protein was highly expressed in the TCV-Ad-m4-1BBL of the 4-1BBL-transfected mice. TCV-Ad-m4-1BBL significantly increased the expressions of IL-2 ([180.24 +/- 2.22] pg/ml) and INF-gamma ([1512.46 +/- 23.64] pg/ml) as compared with TCV and TCV-Ad-eGFP (P < 0.05), and induced higher RM-1 cell specific cytotoxicity ([34.24 +/- 2.64]%) than the latter two ([9.82 +/- 1.48]%) and ([14.65 +/- 3. 21]%), (P < 0.05). But none of them exhibited significant cytotoxicity against hepatocellular carcinoma Hepal-6.
CONCLUSIONThe m4-1BBL-expressing prostate cancer cell vaccine can effectively induce anti-tumor immune responses.

4.Systematic review on the curative effect of total hip arthroplasty through direct anterior approach versus posterolateral Approach

Objective To systematic evaluate the curative effect of total hip arthroplasty (THA) through direct anterior approach versus (DAA) posterolateral approach (PSA).Methods All the randomized controlled trial (RCT) articles and observational research articles about the curative effect of THA through direct anterior approach (DAA group) versus posterolateral approach (PSA group)for treatment of hip disease that published at home and abroad included from database establishing to December 2017 were retrieved from Cochrane Library,Pubmed,Web of Science,CBM and China national knowledge internet (CNKI) through computer.The articles were screened and the information was extracted independently by two authors.A Meta-analysis was performed utilizing the RevMan 5.3 software.Results Two hundred and forty-nine articles were searched out in the initial stage.After screening,7 Chinese articles and 2 English articles (942 cases)were included in the final analysis,including 8 RCT articles and 1 observational research article,and 463 patients in direct anterior approach and 479 patients in posterolateral approach.The results of Meta-analysis demonstrated that the incision length,operation time,intraoperative blood less,Harris hip function scores at 1 month after surgery,visual analogue scale (VAS) scores at 24 hours after surgery,postoperative bed rest time and the hospital stay were better in DAA group compared to PSA group (SMD =-2.51,95% CI:-4.84,-0.19,P=0.03);(SMD=0.84,95% CI:0.10,1.59,P=0.03);(SMD=-1.52,95% CI:-2.98,-0.05,P=0.04);(SMD=2.58,95% CI:0.65,4.51,P=0.009);(SMD=-0.65,95% CI:-0.91,-0.38,P<0.01);(SMD=-1.85,95% CI:-3.00,-0.70,P=0.002);(SMD =-1.12,95% CI:-1.62,-0.61,P <0.01).There were no statistical difference in drainage (SMD =-1.45,95% CI:-3.40,0.49,P =0.14),Harris hip function scores at last follow-up (SMD =0.91,95% CI:-0.31,2.14,P =0.15),VAS scores at 48 hours after surgery (SMD =-1.21,95% CI:-3.03,0.61,P =0.19),acetabular anteversion angles (SMD =-0.50,95% CI:-1.06,0.05,P =0.07),acetabular abduction angles (SMD =-0.01,95% CI:-0.32,0.30,P =0.97) and incidence of complications (OR =1.10,95% CI:0.59,2.07,P =0.77) between the 2 groups.The result of analysis of publication bias according to Harris hip function scores at last follow-up and incidence of complications showed that the funnel-plot was essentially bilateral symmetry.Conclusions THA through DAA can significantly reduce the soft-tissue damage,and it is beneficial for patient's quick recovery,also it conforms to the current concept of minimally invasive treatment that worthy of promotion.
5.New Progress of MALDI-TOF-IMS in the Study of Proteomics.

Matrix-assisted laser desorption/ionization time-of-flight imaging mass spectrometry (MALDI-TOF-IMS) has been a classical technique for studying proteomics in present and a tool for analyzing the distribution of proteins and small molecules within biological tissue sections. MALDI-TOF-IMS can analyze multiple unknown compounds in biological tissue sections simultaneously through a single measurement which can obtain molecule imaging of the tissue while maintaining the integrity of cellular and molecules in tissue. In recent years, imaging mass spectrometry technique develops relatively quickly in all biomedical domain. This paper based on the relevant data and reviews the present developing level of MALDI-TOF-IMS, the principle of imaging mass spectrometry, methology and the prospect in forensic pathology.

6.Quantitative monomolecular coverage of barium glass by the silane coupling agent gamma-methacryloxypropyltrimethoxysilane.

OBJECTIVETo study the monomolecular coverage of the silane coupling agent gamma-methacryloxypropyltrimethoxysilane (gamma-MPS) on the barium glass filler surface.
METHODSKubelka-Munk (K-M) function values of the isolated OH-groups on the barium glass filler surface, which were based on the change of gamma-MPS concentrations, were measured using the quantitative analysis of the diffuse reflectance infrared Fourier transform (DRIFT) spectroscopy. When all of the isolated OH-groups on the filler surface disappeared (where the K-M function values was zero), the monomolecular coverage of the gamma-MPS molecules on the filler surface was indicated by the linear regression analysis.
RESULTSThe relationship tallied with negative linear correlation between the K-M function values of the isolated OH-groups on the barium glass surface and the concentrations of gamma-MPS. Each gamma-MPS molecule occupied 0.21 nm(2) when the monomolecular coverage was formed on the barium glass surface.
CONCLUSIONSThe result of this study indicated the optimal amount of silane coupling agent on silanated barium glass filler during the production of resin composite.

9.Functional MRI of human brain in musicians and non-musicians.

OBJECTIVETo explore the differences in brain activation between musicians and non-musicians by use of functional MRI.
METHODSTwelve right-handed musicians and twelve right-handed non-musicians were recruited in the study. During a listening task, they were scanned on the Sigma 1.5T scanner (GE) while they were passively listening to several segments of music of "the Butterfly Love" and the white noise with same physical energy.
RESULTBoth musicians and non-musicians demonstrated bilateral transverse gyrus weak activated while listening to the white noise. But when listening to music, they showed bilateral temporal areas strongly activated including superior temporal gyrus, transverse gyrus and some middle temporal areas. Moreover, musicians showed relative left dominance (10/12), whereas non-musicians demonstrated right dominance(11/12). Furthermore,besides bilateral temporal areas, more and stronger activated areas were found in musicians such as cuneus, precuneus,medial frontal and left middle occipital gyrus.
CONCLUSIONThere are different neuro-patterns between musicians and non-musicians.

10.Survival of small-cell lung cancer and its determinants of outcome in Singapore.

INTRODUCTIONThe survival and epidemiology of small-cell lung cancer (SCLC) in Singapore has not been described. We aim to present the characteristics as well as determine the survival outcome and important prognostic factors for SCLC patients.
MATERIALS AND METHODSA retrospective analysis of SCLC patients diagnosed from 1999 to 2002 was conducted at the Outram campus, Singapore. Clinical characteristics and treatment data were obtained from case records and survival data were checked with the registry of births and deaths on 30 May 2005.
RESULTSOne hundred and eleven patients were analysed. There were 38 (34.2%) limited-disease (LD) patients and 73 (65.8%) extensive-disease (ED) patients. The majority were current or former smokers (94.7% among LD and 94.5% among ED). More patients with LD had good performance status (92% versus 63%, P = 0.0003) and were treated with combined chemotherapy and radiotherapy (82% versus 48%, P = 0.012). The median survival time of LD patients treated with curative chemoradiotherapy was 14.2 months (95% CI, 10.96 to 17.44). Those given prophylactic cranial irradiation had a median survival time of 16.9 months (95% CI, 11.83 to 21.97). For ED patients, the median survival time was 8.17 months (95%CI, 5.44 to 10.89). None of the factors analysed were significant prognostic factors for LD patients while performance status and type of treatment given were significant among ED patients.
CONCLUSIONSWe found that the characteristics and survival of SCLC patients in Singapore are fairly similar to that of other countries.
