Journal of Clinical Surgery 2017;25(4):277-279
Objective To compare the cure rate, recurrence rate, complication and length of hospital stay in conservative treatment versus laparoscopic appendectomy for uncomplicated acute appendicitis in adults(>18 years-old),we designed this experiment. Methods Seventy-two patients in our department with uncomplicated acute appendicitis between May 2013 to May 2015 were included in our study,in which 43 cases as the experimental group accepted appendectomy, and the other 29 cases as the control group received conservative treatment, all of the patients were followed-up one year out of hospital. And there clinic data about cure rate, recurrence rate, complications and hospitalization time were redcorded. Results As a result,4 cases in the experimental group showed worsening of symptoms under conservative treatment and received laparoscopic appendectomy.2 cases in the experimental group showed recurrence after discharge. In the experimental group, cure rate was observed to be 86.2%,recurrence rate 8.0%(excluding the 4 cases that received laparoscopic appendectomy following aggravation). Conclusion In comparison, control group showed cure rate of 100% with 0% recurrence rate.In terms of complications,no apparent complications were observed in both groups. The mean length of hospital stay were 4.1 days and 5.1 days for experimental and control group, respectively. Our research showed that laparoscopic appendectomy remains to be the most effective treatment for uncomplicated acute appendicitis with a cure rate of 100%;and conservative treatment exhibited cure rate of 86.2%,resulted in less suffering of the patients, less occurrence of complications, and shorter duration of hospital stay.Although may recur, conservative treatment is still an effective treatment worth recommending.

Chinese Journal of General Surgery 2021;36(4):286-289
Objective:To evaluate the mid-term follow-up results of arteriovenous graft(AVG) for patients on maintenance hemodialysis. Methods:The clinical data of 131 patients who implanted AVG from Jan 2014 to Dec 2016 at Department of Vascular Surgery, Nanfang Hospital, Southern Medical University were retrospectively analyzed. Results:The mean follow-up time was 22.8 months (ranging from 2 to 61 months). The average primary patency time after AVG was (22.20±1.97) months, and the primary patency rates at 1 year, 2 years, 3 years were 61.5%, 36.6% and 23.2%, respectively. The average secondary patency time after AVG was (38.30±2.30) months, and the secondary patency rates after 1 year, 2 years, 3 years were 85.6%, 68.6%, and 55.8%, respectively. Sixty five (49.6%) patients had thrombosis after operations, 50(38.2%) had access stenosis, 13(9.9%) had graft infections, and 2(1.5%) had pseudoaneurysm, 2(1.5%) had hemodialysis access-induced distal ischemia, 2(1.5%) had seroma. Conclusions:Though the primary patency rate of AVG is worse than that of arteriovenous fistula (AVF), a satisfactory secondary patency rate can be achieved through repair treatments. Regular follow-up, early detection of stenotic lesions and treatments are vital for long-term patency of AVG.

Objective To explore the effect of panaxadiols saponin (PDS) on the oxidative damage in lung tissue in hemorrhage-lipopolysaccharides (LPS) two hits rats. Methods Forty Wistar rats were divided randomly into 4 groups: sham operational group(S group); two hits groups with hemorrhage-LPS(HL group);dexamethasone preventive therapy group (HLD group) ;panaxadiol saponins preventive therapy group(HLP group). The rats were made first-hit by hemorrhagic shock,LPS were administered intraperitoneally to make endotoxic shock and induce two hits in rats, then the model of acute lung injury (ALI) induced by two hits had been built up successfully. NO-2/NO-3,MDA content and NOS,iNOS,SOD activities were measured by 722S spectrophotometer. Results The results of pathological observation showed that there was obvious inflammatory reaction in lung tissues after two-hits,and the structure was destroied.Compared with HL group, the inflammatory reaction was reduced in HLD group and HLD group.NOS,iNOS activities and NO-2/NO-3,MDA contents were increased significantly, and SOD activity was significantly lower in HL group compared with S group(P
