1.Distribution and drug resistance analysis of common pathogens in neurocranial surgical inpatients

Objective To understand the distribution and drug resistance dynamic status of common pathogens isolated from neurocranial surgical inpatients to provide a basis for clinically rational use of antimicrobial drugs .Methods A total of 413 strains of pathogenic bacteria isolated from neurocranial surgical inpatients from January 2013 to June 2016 were performed the identifica-tion and drug susceptibility test by using the Compact Vitek-2 automatic bacterial identificantion analyzer and the drug susceptibility test results were analyzed by using the WHONET5 .6 software .Results The commonest pathogenic bacteria were 90 strains of Pseudomonas aeruginosa ,69 strains of Klebsiella pneumonia ,52 strains of Escherichia coli ,50 strains of Acinetobacter baumannii and 37 strains of Staphylococcus Aureus .The common bacteria were resistant to many antibiotic drugs .The detection rate of me-thicillin-resistant Staphylococcus Aureus(MRSA) was 54 .1% ,no vancomycin-resistant Staphylococcus Aureus was found .Conclu-sion Clinicians should concern about the common pathogens and their drug resistance in their department ,rationally select antibac-terial drugs ,increase the curative effect and reduce the occurrence of bacterial drug resistance .

Objective To measure von Willebrand factor(vWF)concentrations and von Willebrand factor-cleaving protease(vWF-cp)activity in plasma of lung cancer patients and to evaluate their clinical significance.Methods The vWF:antigen(vWF:Ag)and vWF-cp activity were measured by using ELISA and residual-collagen binding assay(R-CBA),respectively in 78 patients with lung cancer.The serum and pleural fluid(PLF)of carcino-embryonic antigen(CEA)were measured by radioimmunity assay in 23 patients.Results (1)The level of vWF:Ag in lung cancer patients(107.7?43.7)%was significantly higher than that in benign diseases(71.3?49.5)% and normal controls(82.4?41.3)%(P
4.Clinical Comparison of Neuromuscular Pharmacodynamics of Different Dosage of Mivacurium Chloride in Infants during Sevoflurane Anesthesia

Objective To investigate the neuromuscular blocking effects and clinical pharmacology of different dosage of mivacurium chloride in infants during sevoflurane anesthesia. Methods Forty ASA I infants undergoing sevoflurane general anesthesia were randomly assigned to tow groups according to the dose level of mivacurium: group1: 200 μg/kg ( n =20) and group2: 250 μg/kg ( n =20) . TOFs were determined synchronously. The onset time of mivacurium, recovery time of spontaneous breathing and cardiovascular reactions were measured. Results The onset time was significantly shortened in group 2 compared with group 1 (P<0.05) . There was no significant difference in the recovery time of spontaneous breathing between the two groups. 2 minutes after mivacurium was injected, DBP in group 2 decreased significantly compared with baseline and group 1. 3 minutes after mivacurium was injected,SBP in group 2 decreased significantly compared with baseline and group 1. Conclusion In infants undergoing sevoflurane general anesthesia, the onset time of mivacurium can be shortened when 250μg/kg was administered,but the depression of cardiovascular system may occurr simultaneously.
5.Clinical value of N-terminal B-type natriuretic peptide precursor on auricular thrombosis in patients of coronary heart disease combined with atrial fibrillation

Objective To analyze the predictive value of N-terminal B-type natriuretic peptide precursor(NT-proBNP) on auricular thrombosis in patients of coronary heart disease (CHD) combined with atrial fibrillation (AF).Methods The clinical data of 124 patients of CHD combined with AF were analyzed retrospectively.All patients were divided into research group (60 cases,compliance with auricular thrombosis) and control group (64 cases,non-compliance with auricular thrombosis),according to the occurrence of auricular thrombosis.The values of NT-proBNP,fasting plasma glucose (FPG),postprandial plasma glucose(2 h PG),total cholesterol (TC),high density lipoprotein cholesterol (HDL-C),oxidized low density lipoprotein (ox-LDL),high sensitivity C reactive protein (hs-CRP) and left ventricular ejection fraction (LVEF),left ventricular end diastolic diameter (LVEDd) and left atrial diameter (LAD) were detected and compared between two groups.The relative factors to the occurrence of auricular thrombosis were confirmed by multivariate Logistic analysis.The best cutoff point of NT-proBNP was confirmed by the areas under the receiver operating characteristic curve (ROC).Results The values of NT-proBNP,ox-LDL,hs-CRP,LVEDd and LAD in research group were higher than those in control group [(4 312.6 ± 209.1) pmol/L vs.(3 421.6 ± 156.8) pmol/L,(4.0 ± 0.9) mmol/L vs.(3.4 ± 0.8) mmol/L,(7.4 ± 1.3)mg/L vs.(5.8 ± 1.0) mg/L,(74.3 ± 6.8) mm vs.(58.1 ± 5.5) mm,(39.6 ± 4.3) mm vs.(32.5 ± 3.8) mm],LVEF and HDL-C were lower than those in control group [(48.2 ± 3.1)% vs.(57.3 ± 3.8)%,(0.72 ± 0.16)mmol/L vs.(1.08 ±0.27) mmol/L],and there were significant differences (P <0.05).There were no significant differences in the values of FPG,2 h PG and TC between two groups (P > 0.05).Multivariate Logistic analysis showed that NT-proBNP and LAD were independent factors to the occurrence of auricular thrombosis (P =0.009,0.028).There was significant difference in the occurrence of auricular thrombosis between patients with NT-proBNP > 4 250 pmol/L and patients with NT-proBNP≤4 250 pmol/L (P =0.028).Conclusion NT-proBNP is an independent predictor for the occurrence of auricular thrombosis to patients of CHD combined with AF.
6.Investigation on influences of vitamin C,KCL and dicynone on 3 kinds of blood cross matching method

Objective To comprehend the influences of vitamin C ,KCL and dicynone on blood cross matching of saline and mi-crocolumn gel and to compare them with polybrene crossing matching .Methods Under the conditions with and without the specific antibody ,KCI injection ,dicynone injection ,vitamin C injection and normal saline were added for conducting the immunological reac-tion with corresponding RBC .The influences of 3 kinds of drug on different medium cross matching methods of different mediums . Results The low concentration of vitamin C and KCL does not affect polybrene cross matching ,dicynone makes the experiment to generate pseudoagglutination ;the high concentration of these 3 kinds of drug can cause the test to fail .The sensitivity of the saline medium method is slightly lower ,the influence of vitamin C and KCL on the test is inapparent ,while in the KCL method ,with the dilution of antibody ,the agglutination intensity is weakened and even disappeared .Conclusion 3 kinds of drug all have influences on the mierocolumn gel cross matching method ,especially KCL .
7.Expression of Glu、mGluR 5 and EAAT 1 in bone tissues of ovariectomized osteoporotic rats and the effects of Total Flavonoids of Rhizoma Drynariae on it

Objective To observe the expression of Glu、mGluR 5 and EAAT 1 in bone tissues of ovariectomized osteoporotic rats and the effects of Total Flavonoids of Rhizoma Drynariae (TFRD) on it. Methods 45 SPF 3-month-old Sprague-Dawley (SD) female rats were randomly divided into sham operation (Sham, n=15) group and ovariectomized (OVX, n=30) group. The osteoporotic(OP) model was established by bilateral ovariectomy, 14 weeks later, we measured bone mineral density(BMD) by dual-energy X-ray and determined that OP model was successfully replicated, OVX group rats were then divided into OVX group (n=15) and OVX+TFRD group (n=15). The OVX+TFRD group was given TFRD for 12 weeks. Glutamate (Glu), metabotropic glutamate receptor 5 (mGluR 5), and Glutamate/Aspartate Transporter (GLAST/EAAT 1)’s expression of femur was examined in order to clarify the characteristics of bone glutamate signaling pathway and the effects of TFRD on it. Results Glu and ionotropic receptors mGluR 5 mainly distributed in bone marrow cells and osteoblasts closed to the bone marrow cavity walls. There were no significant differences in Glu expression among Sham group, OVX group and OVX+TFRD group. The mGluR 5 expression of OVX+TFRD group was significantly higher than that of Sham group and OVX group(P=0.009), while no significant difference was found between the latter two groups. In addition to large distribution in bone marrow cells, small amount of transporter EAAT 1 was noted to express in bone cells of the bone lacunae. There were no significant differences in EAAT 1 expression among the three groups. Conclusion In bone glutamate signaling pathway, this study demonstrated that TFRD could significantly improve the ionotropic receptor mGluR 5’s expression, but had no inlfuence for Glu and EAAT 1.
8.Surgical treatment for pleomorphic pulmonary carcinoma: a single-center analysis of 60 patients

Objective To investigated the clinicopathological findings and prognosis of 60 patients with pleomorphic carcinoma.Methods 60 cases of pleomorphic pulmonary carcinoma were collected and studied clinicopathologically between 1999 and 2011.All patients underwent pulmonary resections.Results There was no in-hospital death.1 had haemothorax that required reoperation,1 patient had a late cicatricial bronchial stenosis after sleeve lobectomy of the upper right lobe,and treated successfully by the resection of the right two lobes.The 3,5-year survival rates of all patients were 47.2% and 25.6% respectively.For N0 and N1-N2 patients,3-,5-year survival rates were 57.3%,32.5 % and 25.4%,17.1%,respectively.Overall survivals were of significant difference between radical resection and uncomplete resection.Multivariate Cox regression analyses of overall survival of all 60 patients indicated that radical resection (P =0.041) and nodal status (P =0.029) were significant independent prognostic factors.Conclusion Pleomorphic carcinoma is a rare type of lung cancer.Pleomorphic carcinoma of the lung was often found as a large mass peripherally located and was associated with poor prognosis.Radical resection and nodal status were significant independent prognostic factors.
9.Therapeutic antibody: new opportunity for immunity and inflammatory diseases.

With the development of therapeutic antibodies over the past decade, they have become the treatment options for immunity and inflammation diseases. Major limitations of mouse antibodies as therapeutic agents - immunogenicity, lack of effectors' functions and short serum half-life -- were subsequently identified and largely overcome by the advent of humanized and fully human antibody technologies. The therapeutic antibodies for immunity and inflammatory diseases are primarily utilized in the treatment of allograft rejection, autoimmune disease, autoinflammatory syndromes, allergies and other chronic inflammation. The action mechanisms of therapeutic antibody include blocking ligands or receptors, regulating receptor activity, clearing the target cells or activating receptor. Strategies for generating the antibody drugs with high efficacy and low side effects can be realized by modulation of Fc-mediated activities and optimization of antigen-binding domains.
10.Optimizing the host bacteria to make a large naive phage antibody library in the recombination system.

To prepare large naive phage antibody library, the host bacteria with high transformation efficiency is used in the Cre-LoxP recombination system. The variable regions of immunoglobulin light and heavy genes were amplified from lymphocytes collected from adult peripheral blood and newborn cord blood. The genes were spliced to form the single-chain variable fragments (scFv) by overlap PCR, cloned into pDAN5a vector and then transformed into XL2-blue MRF' with the Hte gene. Compared with XL1-blue strain, the size of the primary library was increased by 3.9 times. The primary library infected Cre recombinase-expressing bacteria, and the genes between phagemids created many new VH/VL combinations. The library was calculated to have a diversity of 1.7 x 10(11) and validated by the selection of antibodies against six different protein antigens. This library provides the basis for further selection of antibody-based drugs. It is the first time to report that XL2-blue MRF' can be used to improve the diversity of the library in the recombination system.
